Hello lybrate-user, greetings if the diagnosis is certain that its a disc prolapse and the pain is not unbearable, you can very well go ahead with conservative treatment. In a study it was found that 10 year result of prolapse intervertebral disc is same, whether operated or not. The only difference is operated patient get rid of the annoying radiating pain faster. Need of surgery? Ask your self for the intensity of pain (scale of 0-10) if its more than 5 you can consider getting operated. Secondly look for red flag signs 1. Loss or weakness in lifting your great toe or forefoot from the ground 2. Urine retention (not of empty bladder fully), passing frequent small amount 3. Left or right tilt of spine while trying to stand straight 4. Abnormal gait 5. Urinary or bowel incontinence 6. Bilateral sciatica just be vigilant if you going with conservative line of treatment. I hope its helpful. Dr. pratyush gupta m.b.b.s. M.s. Occasional hard stool can also result in anal fissure. Surgery is not the right choice for Anal Fissure for the reason of recurrence of problem after surgery and other complications like stool incontinence (inability to hold stool) etc. However fissure can be completely treated by Graded Kshar Karma Treatment. In this treatment; a specialized Ayurvedic medicine is applied in fissure region which debrides the fissure and induces healing. Pain associated with fissure subsides in a week or 10 days time period but for complete healing of Anal Fissure usually 3-5 Graded Kshar Karma sittings may be required at weekly interval. This is an OPD procedure and no hospitalization or bed rest is required in this treatment. Now you should take sitz bath (Hot water tub bath) 2-4 times daily followed by application of Sushruta ointment and Lekhan powder. Put 5 ml of Sushruta oil in anal passage in night. For easy evacuation of bowel movement; you should take 1 tea spoon of Sushruta Churna with warm water in night. To get these medicines and for further support and appointment you may call reception at Sushruta Ano Rectal Institute.

Abscess leading to pus discharge is suggestive of Fistula formation. Anal Fistula can not be managed just by some dietary tips/ Sitz bath etc. Even oral medication - Homeopathic/ Allopathic/ Ayurvedic (what ever it may be) is also not able to completely cure this problem. So usually surgery (Fistulectomy/ VAAFT/ Laser etc.) is advised to manage this problem but it is not a good choice as the chances of recurrence after surgery are very high and other complications like stool incontinence etc. However it can be completely treated by Graded Ksharsutra treatment. Clinical trials conducted by Indian Council of Medical Research (ICMR) at AIIMS New Delhi and PGI Chandigarh have confirmed that Ksharsutra treatment is more convenient and more effective than surgery in patients suffering from anal fistula. In this treatment; Graded Ksharsutra - a specialized medicated thread is placed in Fistula tract. Medicine released from Ksharsutra debrides the fistula tract and induces healing. This medicine works for 5-7 days. So old Ksharsutra needs to be replaced by a new one at a week or 10 days interval. No. Of Ksharsutra changes required for complete healing of Fistula depends on depth, direction and branching pattern of Fistula. It is observed that a fistula tract of 3 cm usually requires 4-5 Graded Ksharsutra changes. No hospitalization or bed rest is required in this treatment. You can continue your normal routine activities as usual during this treatment. For further support and appointment you may call reception at Sushruta Ano Rectal Institute.

Neurogenic bladder is bladder dysfunction (flaccid or spastic) caused by neurological damage. Symptoms can include overflow incontinence, frequency, urgency, urge incontinence, and retention. Risk of serious complications (eg, recurrent infection, vesicoureteral reflux, autonomic dysreflexia) is high. While neurogenic bladder can't be cured, necessarily, it can most definitely be managed. Most cases of neurogenic bladder can be managed with medication and intermittent catheterization. The minority of children with the condition need major reconstructive surgery.

I presume she is lung cancer patient, although you have not mentioned the same (as you wrote she is EGFR positive, ALK4 and ROS1 and PDL1 are done in lung) Geftinib is an EGFR inhibitor given in lung cancer EGFR +ve, with good response rates. It does cause diarrhoea and rash in a few patients though. If she has other symptoms, treat her symptomatically. We try to prolong their life by giving palliative treatment, which may be months to few years in exceptional cases. But cure is not a target.
