VBI is usually associated with neck pains. Postural hypotension is other condition where BP falls immediately after standing from lying position and patient may feel dizzy. Your initial description falls in the category of benign postural paroxysmal vertigo precipitated by change of postures and neck movements on a particular side. Your small episode of unconsciousness while cycling needs more analysis. Syncope is the most plausible explanation but it could also be due to sudden kinking of the vertibro basilar artery in the neck bones supplying the posterior part of brain causing sudden loss of balance and transient unconsciousness. Because your all investigation reports are normal one has to presume that you are suffering from bppv and you will need special neck rotational exercises called epley maneuver which needs separate description and guidance to do. If the presumed diagnosis is correct then this maneuver may cure you of your problem. If despite this there is no relief then some more investigations like carotid angiography may be needed. Of course all this is postulation on the basis of your narration without going in the details of reports.

Sinus generally do not cause neck pain and the headaches caused by it are frontal. Giddiness could be due to bppv (benign paroxysmal postural vertigo) which occurs when the neck moves or tilts in a particular direction and position. If the side on which this occurs can be known then the treatment becomes a little simple. Your neck pain may be due to cervical spondylosis. Avoid using pillows. Hi lybrate-user Since you have symptoms from two months you must be having some medicines. Kindly share the treatment you had /having. Vertigo is a medical term which describes a sense if rotation even while stationary. It can often be confused with lightheadedness, generalised weakness, dizziness. Vertigo may be caused by problems in your peripheral (Ear &balance mechanism inside ear known as the vestibular system) or central nervous system. A complete checkup is required to isolate the cause. Peripheral vestibular system causing vertigo symptoms may need initial medications to stabilise followed by Vestibular Rehabilitation exercises. A good quality of life can be achieved if properly managed. A Pure tone audiometry and tympanometry can detect any problem in the ear nerves and tympanic membrane pressure (both of which can be abnormal in case of ear diseases) An Xray of the Cervical Spine (AP /Lat/Lat-oblique to check for cervical spondylitis CT brain and a complete haemogram are some of the baseline tests needed for a good evaluation of vertigo. Another cause of vertigo often not diagnosed by CT /MRI BPPV Benign Paroxysmal positional vertigo BPPV can be treated with Epleys manoeuvre and Vestibular Rehabilitation exercises. With regular vestibular exercise you can be symptom free and lead a normal life. Also if you still ha e symptoms with your current dose of Vertin the dose will gs e to be adjusted. *Tips for starting vestibular exercise: No. 1: Sit on your bed with legs hanging. Keep your head straight and just by moving your pupils first look to the right. Hold this position. Count to 10. If you have dizziness with right gaze still hold that gaze. Then move your pupil to the left. Do this for 10 minutes for two weeks. When you can move your pupil to either side without symptoms, you can start another exercise. Share your reports. Common causes of vertigo are-- Head injuries, high blood pressure, diabetes, sone medicines eg aspirin can cause vertigo. Meniere's disease, infections of the inner ear, migraine, are other causes. Commonest being BPPV. Treatment depends on the cause of it. BPPV needs repositioning maneuvers eg epley's maneuver. Antibiotics r required for infections.
