Dear lybrate-user, the patient has two manifestation or major neurologic symptoms dystonia and now paranoia and hallucinations. As he was started on pacitane (trihexiphenidyl) for dystonia then he developed neuropsychiatric complaints. I would like to explain all the symptoms due to a single cause rather than bring in a new diagnosis. Pacitane has side effect of hallucinations and paranoid behaviour. That may be the cause rather than schizophrenia, I suggest taper pacitane and see the results.
