Hello. You are having obstetric cholestasis of pregnancy. Your doctor has prescribed you udiliv which contains ursodeoxycholic acid (udca) which is absolutely safe in pregnancy. I am mentioning in detail about obstetric cholestasis: onset: 2nd or 3rd trimester symptoms: severe pruritus that starts on the palm and sole, then quickly becomes generalised. Jaundice occurs in 10% of cases as is due to concomitant extrahepatic cholestasis. Diagnosis: when unexplained pruritus occurs in pregnancy associated with abnormal lft or raised bile acids. Once diagnosis is done lft should be repeated every week. (other causes of pruritus and abnormal lft should be excluded - 1) viral screen for hepatitis a, b, cmv and epstein barr virus 2) liver autoimmune screen for chronic active hepatitis and primary biliary cirrhosis i.e. Anti smooth muscle and anti mitochondrial antibodies 3) liver usg should also be done) fetal outcome: has been associated with adverse fetal outcome like: preterm labour, meconium staining, fetal distress and sudden fetal demise. Treatment: topical emollients, antihistamines and udca which is considered as the drug of choice and is given at 12 -15 mg/ kg body weight in divided doses. Elective delivery beyond 37 completed weeks is advisable after antenatal corticosteroids as stillbirths tend to cluster around 37 to 39 weeks.

The reference values for OGTT during pregnancy are like as shown:" According to these criteria the diagnosis of GDM is made if there is at least one abnormal value (≥92, 180 and 153 mg/dl for fasting, one-hour and two-hour plasma glucose concentration respectively), after a 75 g oral glucose tolerance test (OGTT: From those it is clear that your OGTT values after 1 hour and 2 hours are higher, suggesting presence of GDM (Gestational diabetes). It is necessary to maintain glucose values between 90 to 120 mg through out the day, for having a healthy baby, to prevent large baby and to get the desired outcome of pregnancy. So there is a need for looking at your diet and also there may be a need for Insulin injections to achieve lower glucose levels, till the completion of pregnancy. Thanks.

Firstly stretch in the skin can cause itching, other less harmful conditions include small eruptions around belly button called Polymorphic eruption in pregnancy - this is not worrisome and can be managed medically. Last but not least is the serious one called obstetric cholestasis. This causes itching on hands, palms, feet and tummy. There will be sleepless nights due to itch.In this case management starts with a pile of blood tests to rule out other causes as well - full blood count, liver function test, bile acids. Screen for hepatitis A B C, cyto megalo virus and ebstein barr virus, screen for primary biliary cirrhosis & chronic active hepatitis. Then get a scan of the baby for growth and umbilical artery doppler to assess flow. Do a scan of your liver / gall bladder. I appreciate its a costly affair in India but you child is important.
