Yes it is harmful, it can cause recurrent ear infections and you can loose your hearing. For precautions you should keep your ears dry, try not to have cough and cold. And you should get it operated.

You need to take antibiotic course or homoeopathic. Avoid playing in water, using shower, keep your ear dry. No ear drops are advised during perforation. Consult privately for medicines.
