First try to lose wt. For pin worms get dewormed. If your chronic typhoid then go to a gastroentrologist for openion at your place.

Please take sitz bath (warm water in a basin put one fist of salt) and sit in tat water twice or thrice in a day.

Your explanation shows the main reason is hard stools. Take more fibre diet n warm water. That will reliev your condition.
