Coccydynia (Tailbone Pain)- Physiotherapy Treatment Approach

We all have lost our tail with evolution…but did you ever think that the vestigial (redundant) part of that tail which has been left over in us in the form of tailbone can mess up with our daily activities and torture us to the level of not allowing us to sit!

If you are not able to sit or if you are experiencing pain when sitting right at the tail bone region, you may be having ‘Tailbone Pain or Coccydynia’.

What is Coccydynia?

The coccyx, also referred to as the ‘tailbone’ is the last segment of one’s vertebral column. Coccydynia is referred to a disabling pain that occurs in/around the coccyx. This type of pain usually starts when one sits all of a sudden or rises up from the seat after having been seated over a long period of time. Also known as coccygodynia, this condition can mar one’s quality of life. The pain, often described as ‘stabbing’ or ‘piercing’, can radiate to the buttocks, the lumbar spine, and rarely, to one’s thighs.

Anatomy

The coccyx is the final part of one’s vertebral column. The vertebral units are fused together. The frontal portion of the coccyx is the fusion site of the muscles and ligaments which control the functions of the pelvic floor. The coccyx also supports the anus’s position. The gluteus maximus is attached to the posterior part of the coccyx. Ligament or muscle damage, or muscle weakness can cause the coccyx to assume an abnormal position, thus causing pain.

Common Causes:

  1. Sitting for extended periods of time, especially on hard surfaces

  2. Effect of a direct trauma, such as a fall

  3. Giving birth

  4. Bone fracture, tumors or an infection can also cause this condition

How do you know whether it’s Coccydynia?

  1. You will experience pain while sitting, especially on hard surfaces

  2. Localized pain in or around the tailbone that worsens with touch or any pressure applied on it

  3. The pain will become severe when you stand from a seating position after considerable amount of time

  4. The pain might also start during sexual intercourse

  5. The pain increases in women during their menstrual cycles

Diagnosis

  1. Local Coccydynia

    • Traumatic

    • Idiopathic

  2. Radiated or referred Coccydynia
  3. Psychogenic Coccydynia

  4. Chronic proctalgia

Physiotherapy Approach For Treating Coccydynia

Patients diagnosed with Coccydynia are advised to avoid factors which might provoke the pain. The initial line of treatment will include making certain adjustments such as applying gel cushion when one is sitting for extended periods of time. This helps reduce localized pressure and improve his posture. Other modes include:

  1. Mobilizations: This can help realign the posture of the coccyx. Initially, there might be tenderness; hence, it is advised to start with rotational mobilization. To start with, either side should be mobilized first.

  2. Manipulation: This can be done intra-rectal and when the patient is lying in a lateral position. The coccyx is repeatedly extended and flexed with the help of the index finger. However, care must be taken so as to not hurt the rectal mucosa.

  3. Massage: Massaging the coccygeus muscles has also been proven to relieve pain. Biofeedback can also be integrated with it in order to avoid incidences of muscle pull on the coccyx. If you wish to discuss about any specific problem, you can consult a physiotherapist.

Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have mentioned pain in lower back region as your main problem. You are actually very young to have persistent pain. If there is only localized pain in back region with no other associated symptoms like radiating pain, numbness, paresthesias etc, and no systemic symptoms like fever, loss of appetite, loss of weight etc, my advice to you for initial 2-3 weeks will be: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or even tab paracetamol which suits you - intensive physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied 3-4 times in a day - analgesic spray for local application like dynapar qps spray can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc in case you are having back pain. If there is no improvement in 2-3 weeks with this conservative management, we will need to evaluate you further in form of dynamic x rays to look for any instability as a cause for pain and some blood investigations to rule out any deficiencies etc. Do not hesitate to contact me if you need any further assistance.

Sports Taping- stretch the tape from both ends and apply on the affected area Contrast Fomentation (Hot and Cold). Contrast bath---It is a form of treatment knee is applied warm water followed by the immediate application of the knee with ice water. This procedure is repeated several times, alternating hot and cold. Note that the treatment should always end in the ice water, as heat will induce the body's inflammatory response, while cold helps to decrease inflammation.

I am 40 three years old man. Recently there is problem in climbing up in stairs in the knee joints and feeling fatigueness in the two legs below knee joints when sitting couple of hours time in chair in office.

Kindly show me a photograph of the affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful use no pillow under the head. Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back, knee & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, contact me again. Do not ignore. It could be beginning of a serious problem.
