Hello, usually brownish discharge means an old bleed. Shouldn't be a concern of fetal heart is fine now. If it continues, discontinue ecosprin for a few days. But to rule out chromosomal problems you need to get a nt scan and a double marker test between 11 and 13 weeks 6 days. It gives an idea about chromosomal anomalies. All the best.

Cortisone Shots - Are There Any Risks Associated with them?

MBBS, D (ORTHO), DNB (Orthopedics), MCH (Orthopedic), Spine Fellowship â?? DePUY, Medtronics Spine Fellowship

Cortisone Shots - Are There Any Risks Associated with them?

Cortisone shots are injections that help relieve pain and inflammation. They are injected directly into the joints such as the ankle, wrist, knee, hip, shoulder, elbow or spine. These injections are a combination of corticosteroids and local anaesthetics. These corticosteroids are similar to the hormones produced by the adrenal gland. Cortisone injections are often used to treat arthritis, carpal tunnel syndrome, trigger finger, shoulder bursitis and plantar fasciitis. Though they do not cure the disease but they provide temporary relief.

These injections may be slightly painful but provide relief in a day or two that lasts for a few weeks or months at a stretch. Hence, cortisone shots provide relief faster and have longer lasting results than other forms of anti-inflammatory medication. Another benefit of cortisone injections as compared to other drugs that need to be taken in through the mouth is that it avoids side effects of anti-inflammatory medication such as an upset stomach etc. Cortisone injections can also be used to treat backaches and pain that radiates from the spine to an arm or leg.

However, cortisone injections are associated with a number of risks. These include:

  1. Infection in the joints
  2. Nerve damage
  3. Thinning of tissue and skin around the injection site
  4. Pain and inflammation in the joint
  5. Weakening or rupturing of tendons
  6. Osteoporosis or thinning of bones
  7. Lightening of skin around the injection site
  8. Osteonecrosis or death of bones in the injected area
  9. Blood sugar spikes – this is seen mostly in diabetic patients
  10. Weakening of the immune system if the person has an underlying infection

In most cases, these side effects are short-lived. Higher dosages and frequent administration can increase the body’s exposure to the corticosteroid. This increases the risk of long-term side effects such as:

  1. Acne
  2. Easy bruising
  3. Weight gain
  4. Cataract formation
  5. Puffiness of the face
  6. High blood pressure

Cortisone may also be held responsible for the deterioration of cartilage inside a joint. For this reason, doctors do not advise cortisone injections to be had more than 3-4 times a year. The minimum interval between two cortisone injections must be 6 weeks. Doctors also typically limit the number of injections given in one joint.
When used judiciously, cortisone injections can greatly improve the quality of a patient’s life. Hence it is very important to get treated only by a qualified doctor and to completely understand the pros and cons of using this form of treatment. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.

Weight Gain: Muscle Building Part III: Basic Guidelines to Follow

General Physician, Delhi
Weight Gain: Muscle Building Part III: Basic Guidelines to Follow

In this series of sections on Weight gain - We will discuss Basic Rules to follow for Muscle Building

Gradually increase your calorie intake

Eating 5 to 6 meals a day. In order to gain weight, start eating 5-6 meals a day by breaking them into parts. Eating a lot at once may cause indigestion if the body fails to absorb all nutrients. Eat in small amounts, even if you are not hungry and there will be an increase in appetite from day to day.

  1. Eat healthy and timely
  2. Have a balanced diet with heavy foods like - starchy vegetables, dense fruits, added oils and creamy soups
  3. Have foods rich in protein, avoid watery fruits and veggies
  4. Avoid trans-fats
  5. Do not binge, pace yourself through your diet regime
  6. Have dietary supplements and protein shakes
  7. Start weight training, Workout your entire body

If you build your muscles through weight training, it will convert the extra weight into lean body mass and also stimulate your appetite. The extra muscle increases the rate of metabolism.

When you start exercising you will often experience extreme muscle soreness but it goes away in 3-5 days.

A lot of people focus only on training the muscles on their arms, chest, and shoulders and this is a grave error. Firstly, it will give you a disproportionate body; and it will make you prone to injuries. Therefore, it's important to work out the entire body for a complete and balanced look.

Be patient and believe in yourself

Gaining weight quickly in a healthy manner is unrealistic. Every body type is different and it takes months to gain weight. Living healthy by eating the right diet will increase your strength. It is also important to keep following the routine even if you don't see any gains.

Things to remember

  1. Stop eating junk food, eat healthily
  2. Hydrate yourself well
  3. Rapid weight gain may cause stretch marks
  4. Be careful when choosing fatty foods as overindulging in them can lead to heart diseases and type 2 diabetes
  5. Overeating may cause bloating, stomach aches and cramps

Remember, all through the process of changing your body to what you wish requires great hard work, dedication, and motivation. With the right attitude and the overall aim of building a healthy body rather with patience pays out well than short term gain or synthetic means.

General Physician,
7 Causes For Irregular Periods And Why You Should Not Ignore Them

A missed period is one of the most common worries for women. When her menstrual cycle dates don?t match her marked calendar dates, there are a dozen of fears she is burdened with. Most women find a missed menstrual period occasionally. But when it happens twice in a row, it means it?s time to get yourself checked.
Taking a little extra time to check in with your body gives you the chance to recognize abnormalities. You can then take steps accordingly to effectively address them.
A normal menstrual cycle is of 28 days, plus or minus seven days. An irregular menstrual cycle is characterized either as an earlier than expected period or a later than expected period. It can also mean a skipped period or bleeding in between regular periods, heavier/ lighter menstrual flow than usual. The most common reasons for irregular monthly cycle are as below.
Excessive weight loss or gain: Weighing too much or too little affects a woman?s menstrual cycle. Too much weight gain can cause excess estrogen storage in fatty tissues. This then gets partially converted by the body to androgens leading to thinning of the uterine lining and decreased ovulation. Excessively low body weight interrupts many hormonal functions in the body, potentially halting ovulation. Strive for a healthy weight.
Stress: Stress can affect hormones and induce irregular periods. Extreme fatigue, worry or anxiety cause the hormones to become unbalanced. Excessive stress plays a role in suppressing the functioning of the hypothalamus. Hypothalamus, controls the pituitary gland that in turn controls the thyroid and adrenal glands and the ovaries, all of which work together to manage hormones. The side effects of ovarian dysfunction leads to irregular periods. Monthly periods are quite susceptible to dips and spikes in our emotions and our health. A sudden change in the way your brain reacts to stressed situations interferes with the hormonal productivity, leading to irregular estrogen and progesterone levels.
Intense Exercise: Regular exercise is good to maintain a healthy life. However, too much exercise can result in estrogen dominance thereby leading to irregular periods. Girls who are athletic and very active in sports normally face problems with their regular menstrual cycle. Athletic amenorrhea tends to be seen in women who participate in rigorous training exercises like long distance running, ballet, gymnastics, and figure skating.
Birth Control Pills ? they can make one?s periods lighter, or cause one to miss periods. The body takes sometime to adjust to the new levels of hormones delivered by hormonal birth control therefore leading to irregularity.
Chronic Illness ? Chronic illnesses such as Celiac disease, Crohn?s disease, cystic fibrosis, Tuberculosis, heart disease, uncontrolled diabetes may postpone menstrual periods by delaying ovulation.
PCOS ? It is a genetic condition that results from abnormal levels of certain hormones. Girls with PCOS typically have irregular menstrual cycles and may experience fewer than 9 menstrual cycles a year. Due to PCOS, the ovaries do not regularly ovulate and there are also a number of under-developed follicles in the ovaries. They have high levels of androgen (male hormones) which leads to excess hair growth, acne and often overweight.
Alcohol Abuse ? Long-term heavy drinking can cause women to have irregular periods or stop ovulating. Drinking alcohol causes hormonal imbalances and can affect the way the liver processes the hormones that are responsible for ovulation and menstrual regularity.
Many women miss periods now and then. If you?ve skipped a period, try to relax and firstly restore the emotional and physical balance of yourself. Unless you are pregnant, chances are your cycle will return to normal next month. However, if you miss it the second time or show symptoms of irregular periods along with symptoms like flushing, sweating, heat intolerance, heart palpitations, and sleeplessness immediately check with the doctor. Many menstrual problems could indicate undiagnosed thyroid conditions. They call for a thyroid evaluation by your physician.
Never ignore menstrual changes. Talk to your doctor without delay!

The chance is there but lower. Again at this age, there is increased chance of having birth defects in the baby. So, you should consult your doctor to find out, if she is having normal ''eggs". If not, she may need IVF with ovum donation. And after pregnancy confirmation, you should maintain contact with doctors as there are numerous methods to detect the birth defects during pregnancy. All the best.
