Abdominal Pain - Causes Behind It!

Abdominal Pain - Causes Behind It!

There is hardly any individual across the world, who has not experienced abdominal pain. In most instances, these aches are not very serious and can be diagnosed and treated by addressing the underlying condition. It is very crucial to recognize the symptoms which are severe and know when an individual should seek medical assistance.

Most common causes of abdominal pain:

Some of the most common causes of abdominal pain include Indigestion, infection, menstrual cramps. It may also be due to food poisoning and allergies, irritable bowel syndrome, gas formation and bloating, lactose intolerance, pelvic inflammatory disease or ulcers and hernia, gallstones and kidney stones, urinary tract infection, Crohn’s Disease or gastroesophageal reflux disease.

Some patients have abdominal pain but the source of pain lies in the spine. These patients are often seen by multiple doctors and have undergone many tests but their diagnosis is never established. These patients should visit a pain specialist to reach a diagnosis and treatment plan. They have conditions like neuropathies or facet joint arthritis which cause abdominal pain due to a condition known as referred pain.

Symptoms of abdominal pain that are cause for concern:

  1. Abdominal pain along with any sign of dehydration or fever or is unable to pass stools.
  2. Abdominal pain associated with tenderness in the abdomen, painful and frequent urinations or persistent loose motions or bloody/ tarry stools.
  3. Severe or recurrent abdominal pain.

Determining the cause of abdominal pain and treating it.

  1. Detailed medical history and thorough physical examination would be performed. Further tests like USG, Endoscopy etc may be needed to reach a diagnosis.
  2. The treatment of abdominal pain is entirely dependent on the cause of the pain.

Pain Management "Pain can impact your life in many ways. It can limit your physical as well as mental capabilities and make it difficult for you to enjoy life in its fullest form" Pain is defined as “an unpleasant sensory and emotional experience associated with actual or potential tissue damage- The International Association for the Study of Pain (IASP) Pain can be broadly classified in to Acute and Chronic pain. Acute pain occurs for a short interval of time and usuallyis an after-effect of a surgery, any physical trauma, or other condition. Prompt treatment is advisablein such cases because if it is left untreated, can lead to serious, chronic pain later. Acute painshould be taken as a signal from the bodyfor immediate help. Some forms of chronic pain can be detected and treated more easily than others, like the osteoarthritis pain, which is a treatable condition. Surgery and/or medication can serve as the line of treatmentin these cases. However, some other forms of chronic pains like the neuropathic pains demand a more critical analysis before deciding the treatment option which best suits the condition. Fundamental research associated with chronic pain points to the neuro-plastic alterations in the nervous system at the brain or spinal cordlevel of people suffering from such a pain. According to recent survey, about 65% of people suffering from chronic pain of some form or other admit that the pain interferes with their daily routine. They find it difficult to carry out their day-to-day activities and face problems in social and personal relationships. Chronic pain can be explained as a long-term pain that continues even after the expected healing time, subsequent to a surgery, trauma or other event. Trigeminal Neuralgia!

Fellowship in Pain Management, MD, MD - Anaesthesiology , MBBS , Observer post
Pain Management Specialist, Pune

What is trigeminal neuralgia?

Trigeminal neuralgia is a condition affecting the trigeminal nerve, the largest nerve in the head. This nerve is responsible for sending touch, pain temperature signals associated with the facial organs like jaws, gums, and head to the brain. This condition is characterized by shooting shock-like severe pain for an interval of few seconds. The pain is generally on the face around the lips, eyes and so on. The pain is felt by an individual while doing activities like brushing, eating, or touching the face for any reason. Trigeminal neuralgia usually induces pain on a side or in some cases, both sides of the face of the patient. The pain is felt at different times of the day, with repetition in few hours. Sometimes the pain lasts for months together. This condition is more common in women than in men. The pain is described as one of the worst pains on the planet by the people who have experienced the pain.

Reasons -
The definite reasons for trigeminal neuralgia remain unknown; however the effect is disturbance in the function of the trigeminal nerve. The trigeminal nerve is a bundle of three nerves which is responsible for carrying information from the facial region to the brain. When this nerve gets compressed due to any reason, an individual might experience the pain symptoms. Sometimes the outer covering of the nerve, myelin sheath, is destroyed due to wear to cause severe bouts or a continuous pain in the facial muscles. The reasons in most of the cases can be attributed to:

  1. Tumor or a lump which presses on the nerve causing wear of the trigeminal nerve
  2. A cyst, fluid-filled sac which causes pressure on the trigeminal nerve
  3. Arterio-venous abnormality leads to disruption of the nerve to cause pain
  4. Multiple sclerosis or Ms. Which is a disease of central nervous system

Treatment -
After hearing thorough symptoms from an individual, if doctor suspects trigeminal neuralgia, he will suggest a mri-scan of the brain to observe the trigeminal nerve and surrounding area. This will assist the doctor in finding out the underlying cause for the condition, whether a cyst, tumor or Ms. is responsible. If it is none of the considered reasons other causes will be taken in to consideration. The line of treatment for trigeminal neuralgia starts with medications, which have proved successful in most of the cases. But sometimes, medications become less effective as the time passes causing the symptoms to worsen and difficult to handle. In such cases, injections are the preferred choice for alleviation of pain. Medications like anticonvulsants alleviate pain in some patients as these medicines have calming effect on the nerves. Antispasticity agents are employed to relax the muscles and give relief from the pain.

Injections are employed in order to treat the pain symptoms occurring in the trigeminal neuralgia patients. Radiofrequency ablation of gasserian ganglion is a minimally invasive procedure, in which the gasserian ganglion (sensory part of the trigeminal nerve) is ablated or modified through the use of radiofrequency. It is a popular method used world-wide for pain relief in patients suffering from trigeminal neuralgia with encouraging results.
