I am 32 year old and my nose remains close always if I don't take otrivin nasal spray after few hours and even I can't take any thin cold like ice cream, cold water, cold drink or any cold thing. As I consume cold thing my nose blocks and start watering please advice.

Visiting Physicians Programme, Fellowship in Sinus Surgery (Austria), Micro Esr Surgery, MS - ENT
Ear-Nose-Throat (ENT) Specialist, Hyderabad
This condition is called rhinititis medicamentosa, is dangerous. As over use of otrivin causes high blood pressure. You should immediately stop otrivin and see ent doctor.

While coughing an abnormal sound produces from throat. CT scan, x ray, allergy tests, ultrasound etc done. Checked for Pleural effusion, But nothing is seen and not going as well as not worsening. Four years old now. The problem is since his birth. No pain is also there. Swallowing liquids seems to be difficult. This seems barking sound. In CT Scan there is a slight permanent enlargement. Nothing else. Cough is not chronic. What should I do?

I am diabetic, however I am not comfortable with medication. After taking medicines for week long, I start feeling uncomfortable. I feel lots of dizziness and weakness with medication. Please suggest me some better way out to fight diabetes.

PGP In Diabetologist, Fellowship in non-invasive Cardiology, MD - Medicine, MBBS
Endocrinologist, Delhi
The only other way to fight diabetes is through weight loss. Worldwide, bariatric surgery has proven that weight loss is vital to diabetic reversal (a state where you are euglycemic without the need of any oral medication or insulin. A systematic review and meta-analysis of the english literature reported complete resolution of type 2 diabetes (defined as discontinuation of all diabetes-related medications and blood glucose levels within the normal range) in 78.1% of cases. But, there are two problems in this, first of all bariatric surgery is still very expensive in india (starts from rs 3-4 lakhs and above) secondly, it is not known clearly that all the benefits of post - bariatric surgery on diabetes reversal is entirely due to weight loss alone. On the other hand, dietary restriction and exercise (also known as lifestyle modification) can also theoretically cause weight loss and lead to diabetic reversal and has been tried world over. But, the fact remains that around 95% of weight loss achieved through lifestyle changes is short lived and does not translate into any benefit in the long run. Then there are stem cell therapies

I am 18 years old and I have sinusitis problem from a month. How can I get rid of that problem.

DORL, MBBS
Ear-Nose-Throat (ENT) Specialist, Faridabad
Main symptoms for maxillary sinusitis are thick nasal discharge, headache, watering eyes, forehead pain, recurrent infection of nose and mouth, post nasal drip, sinusitis, is nothing but a physiological disorder of nose which led by malfunctioning of aerodynamics of nose (temporary sinusitis, secondly it is because of repeated infections of the sinus (permanent sinusitis) sinuses regulations and air conditioning of the air which we breathe and to get the benefit of above we must have normal, well sized opened sinusitis and hence nose breathing is required, those who breathe from mouth, they have tendency to get more throat and lungs infections. Recurrent sinus infection, pollution, poor diet, spicy food, long term steam inhalation from nose, and allergies leads to thick discharge from nose. This thick mucus if stays in the sinuses, it leads to irritation of the sinus skin which is mucosa and this go on growing hence leads to permanent sinusitis this extra growth is called nasal polyps this is where patient needs surgical correction. If patient comes to nose surgeon at right time, doctor can revert his/her symptoms and surgery can be abandoned. Sinusitis if patient does not take serious steps to correct might lead to cardiac symptoms, lung diseases, etc. Once there are nasal polyps you have to go for surgery immediately.

Few of the very common causes of snoring are 1. Obesity 2. Tonsillar enlargement 3. Adenoid hyper trophy 4. Sleep apnoea 5. Obstruction or growth in the airway the therapy involves treating the above causes. Weight reduction shall help to a great extent. Ent surgeon often operate on cases where snoring is intolerable to the near ones or it is associated with sleep disturbances. Surgical therapy is one of the sure shot way of managing snoring. Almost 9 years ago in 2007, when I was in my vacation and travelling in train, in mid night I had a feeling that train was rolling, Immediately I sat and I was holding the chain firmly and did not sleep the whole night. Very next day when I met doctor, he examined and said whenever I lie down, my pressure goes less than 100 and when I get up it goes above 120, That is the reason for this happening. Then I had some medication. I was told not to get up from bed immediately, but slowly turn one side, put one leg down on the ground and then both leg on ground. I followed for this for almost a month. Then it slowly vanished. I was later told this might be because of some shoulder bone pressing some nerve which stops blood going to brain for fraction of seconds and I was asked to go physiotherapist. He did some IR therapy and suggested some exercises and slowly I forgot that incident till 2015. In 2015, when I was cycling (prior to joining Goqii), I fainted on middle of road and lasted for few minutes and I was admitted to hospital. The discharge summary simply indicated Syncope. After that once in 2 or 3 months, I have giddiness for fraction of seconds, while getting up from chair or bed or coming out of car (basically when I stand up after long sitting). All X-Ray, ECG, ECHO, CT scan and even MRI shows normal.

PGD Maternal Child Health, MBBS
General Physician, Akola
Your initial description falls in the category of benign postural paroxysmal vertigo precipitated by change of postures and neck movements on a particular side. Your small episode of unconsciousness while cycling needs more analysis. Syncope is the most plausible explanation but it could also be due to sudden kinking of the vertibro basilar artery in the neck bones supplying the posterior part of brain causing sudden loss of balance and transient unconsciousness. Because your all investigation reports are normal one has to presume that you are suffering from bppv and you will need special neck rotational exercises called epley maneuver which needs separate description and guidance to do. If the presumed diagnosis is correct then this maneuver may cure you of your problem. If despite this there is no relief then some more investigations like carotid angiography may be needed. Of course all this is postulation on the basis of your narration without going in the details of reports.

1) I am 17 years old and a female. I have undergone laser tonsillectomy a week ago on 17. My doctor prescribed me ROMILAST L10 for 30 days. Is it necessary for me to use? 2) he has also advised me not to take ice creams for a life time and if I will not I'll catch the problem again as tonsils grow again. Is it true? If not until how many days should I resist eating ice creams.

Diploma in Otorhinolaryngology (DLO), MBBS
Ear-Nose-Throat (ENT) Specialist, Kolkata
You should follow the instructions given by your treating physician. If your tosillar tissues are removed competely during operation then there are no chances of tonsils growing again and there is no contraindication of taking ice cream. After tonsillectomy we advise all our patients to take ice cream as their first oral diet.

Blood is coming from nose since a week or more what is the and solution for this problem.

PDDM, MHA, MBBS
General Physician, Nashik
Pinch all the soft parts of the nose together between the thumb and index finger. Press firmly toward the face - compressing the pinched parts of the nose against the bones of the face. Lean forward slightly with the head tilted forward. Leaning back or tilting the head back allows the blood to run back into the sinuses and throat and can cause gagging or inhaling the blood. Hold the nose for at least five minutes. Repeat as necessary until the nose has stopped bleeding. Sit quietly, keeping the head higher than the level of the heart. Do not lay flat or put your head between your legs. Apply ice (wrapped in a towel) to nose and cheeks. Get examined by ENT surgeon in person.

Hello doctor I got little ear pain and my chik got swelling one side. Can you suggest me medicine.

Certificate course in Pain Relief and Palliative care, MBBS
General Physician, Secunderabad
Hello Ms. Lybrate-user. What you are describing may be an ear infection or may be parotitis or even mumps- an infection that raises the earlobe due to swelling of the cheek. You can look up mumps on the internet and consult an ent doctor for ear infection and a general physician for mumps. Hope this helps. Warm regards,
