First twelve weeks of pregnancy will be like that. Give her whatever she likes to eat. Avoid spicy food and carbonated drinks. Unless she is very weak or dehydrated no need to consult for this sake.

Vaginal Douching - Is it Good?

MBBS, DGO, MD, Fellowship in Gynae Oncology
Gynaecologist, Delhi
Vaginal Douching - Is it Good?

Like many systems within the human body, the reproductive system is also a self maintaining one and performs many complex functions on its own. The female reproductive system is one such system and within it, the vagina is an especially good example. It is a self contained system wherein, the balance of the vaginal bacteria is maintained for proper health.

Douching
Douching is a process that involves the cleaning out or washing of an internal cavity of a body, such as the anus or the vagina with the help of water and special nozzles. Vaginal douching has become prevalent among women, mostly due to aggressive marketing campaigns which claim that it helps to keep the vagina clean. It is estimated that in developed nations, one in four women perform douching regularly.

Is it any good?
Vaginal douching has been reviled by most medical professionals around the world. The consensus is that not only does vaginal douching have no benefits; it may also be harmful for your vagina in general. The vagina is an ecosystem wherein multiple types of organisms thrive, including bacteria and fungi. These organisms keep each other in check and maintain the health of the organ. These organisms help maintain the pH Levels (the alkaline to acidic radio) within the vagina. Douching may actually disrupt the balance and cause harm by destroying one type of micro-organisms and letting the other proliferate, thus causing infections and other problems.

Problems caused by vaginal douching
Douches, especially the ones found in the market, contain elements such as iodine, baking soda, vinegar and other chemicals that can make the delicate vaginal balance go haywire. Some of the disorders that can be caused by it are mentioned as follows:

  1. Yeast infections: This is due to the proliferation of the fungi known as ‘candida’ within the vagina. This fungus is present within the vagina, but douching can reduce the bacteria that counteract it, resulting in rapid growth. This is one of the most common infections of the vagina.
  2. Bacterial vaginosis: Similar to how yeast infections occur, in this case, harmful bacteria proliferate within the vagina. This is also a common vaginal infection and douching has been deemed as one of the leading causes for it.
  3. Pelvic inflammatory diseases: Douching pushes the bacteria of the vaginal canal into the cervix and the uterus, resulting in infections such as PID or pelvic inflammatory diseases (an infection in the reproductive organ in females).
  4. Pregnancy related problems: A myth has been floating around that if a woman performs douching after sexual intercourse, it will wash away the sperm and hence prevent her from getting pregnant. In fact, the reverse is true. Douching can often push the sperm up into the uterus causing pregnancy. Not only that, pregnancies caused in such a manner have higher chances of bearing complications such as ectopic pregnancies (a condition wherein the fertilized egg gets implanted outside one’s uterus), birth defects, etc.
  5. Irritation and vaginal dryness: Douching at the very least will result in the vagina turning dry as the natural mechanisms are disrupted. This results in irritation and itching. If you wish to discuss about any specific problem, you can consult a gynaecologist.

My friend wife is planning for iui/ivf. But my friends showed negative signs as he is not willing to sign the paper. His question is; 1. Should the baby born after will look likes his/her mother? 2. Fearing of what will society says if the baby look totally different from parents. please help him.

MBBS, DGO, MD, Fellowship in Gynae Oncology
Gynaecologist, Delhi
If a couple is going for self ivf then the baby will be as the parents only with the same genetics and looks. In case of donor ivf baby may look different but not too much as donor selected is matched with the patient profile, and baby will have dna of the parent. If a couple wants to be parents then there is no such thing to worry about society, people adopt a child as well and it is not a bad thing. To become a parent is ones right, and it is not bad. So don't worry about such things.

These are some pointers you have to follow during pregnancy:

  1. Maintain good hygiene during pregnancy. Take bath regularly. Warm shower or sponge bath is considered better than tub bath.

  2. Brush your teeth regularly and have a regular dental check up. Caries tooth can be extracted after 12 weeks of pregnancy.

  3. Avoid wearing tight clothes and shoes with high heels. Wear loose and light clothes.

  4. Take lots of fluid, fresh fruits & vegetables.

  5. Take a diet which is light, nutritious, easily digestible & rich in protein, less fat, minerals and vitamins.

  6. Stop smoking and drinking alcohol. It can cause miscarriage, defect in the development and growth of the baby.

  7. Avoid jerky travelling, especially in 1st 12 wks and last 6wks of pregnancy. Prolonged sitting in a car or plane should be avoided. Air travel is not advisable to women who have high blood pressure, less hemoglobin (anemia) and low lying placenta.

  11. Start taking iron & calcium after 12 wks of pregnancy and continue it throughout pregnancy and 6 months post delivery.

Diving, water sking, gymnastic, netball, hockey, horse riding, cycling, rock-climbing, & scuba diving exercises should be avoided during pregnancy.

  13. If you have any of these symptoms - vaginal bleeding, persistent abdominal pain, persistent vomiting, watery discharge from vagina, less or no fetal movement, severe headache, swelling on hands, face & feet, fever, dizziness, blurring of vision, double vision, spots before eyes, painful urination and diarrhea, immediately consult your gynecologist.

Need some diet plan for PCOS & also want to know how to cure PCOS problem in women? Please advise.

MBBS, MD - Internal Medicine
Internal Medicine Specialist, Faridabad
Keep your weight in check. Obesity makes insulin resistance worse. Weight loss can reduce both insulin and androgen levels and may restore ovulation. No single specific dietary approach is best, but losing weight by reducing how many calories you consume each day may help with polycystic ovary syndrome, especially if you're overweight or obese. Use smaller plates, reduce portion sizes and resist the urge for seconds to help with weight loss. Ask your doctor to recommend a weight-control program, and meet regularly with a dietitian for help in reaching weight-loss goals. Consider dietary changes. Low-fat, high-carbohydrate diets may increase insulin levels, so you may want to consider a low-carbohydrate diet if you have PCOS — and if your doctor recommends it. Don't severely restrict carbohydrates; instead, choose complex carbohydrates, which are high in fiber. The more fiber in a food, the more slowly it's digested and the more slowly your blood sugar levels rise. High-fiber carbohydrates include whole-grain breads and cereals, whole-wheat pasta, bulgur wheat, barley, brown rice, and beans. Limit less healthy, simple carbohydrates such as soda, excess fruit juice, cake, candy, ice cream, pies, cookies and doughnuts. Be active. Exercise helps lower blood sugar levels. If you have PCOS, increasing your daily activity and participating in a regular exercise program may treat or even prevent insulin resistance and help you keep your weight under control. Polycystic ovary syndrome treatment generally focuses on management of your individual main concerns, such as infertility, hirsutism, acne or obesity.Regulate your menstrual cycle. To regulate your menstrual cycle, your doctor may recommend combination birth control pills — pills that contain both estrogen and progestin. These birth control pills decrease androgen production and give your body a break from the effects of continuous estrogen, lowering your risk of endometrial cancer and correcting abnormal bleeding. As an alternative to birth control pills, you might use a skin patch or vaginal ring that contains a combination of estrogen and progestin. During the time that you take this medication to relieve your symptoms, you won't be able to conceive. If you're not a good candidate for combination birth control pills, an alternative approach is to take progesterone for 10 to 14 days every one to two months. This type of progesterone therapy regulates your periods and offers protection against endometrial cancer, but it doesn't improve androgen levels and it won't prevent pregnancy. The progestin-only minipill or progestin-containing intrauterine device are better choices if you also wish to avoid pregnancy. Your doctor also may prescribe metformin (Glucophage, Fortamet, others), an oral medication for type 2 diabetes that improves insulin resistance and lowers insulin levels. This drug may help with ovulation and lead to regular menstrual cycles. Metformin also slows the progression to type 2 diabetes if you already have prediabetes and aids in weight loss if you also follow a diet and an exercise program. Help you ovulate. If you're trying to become pregnant, you may need a medication to help you ovulate. Clomiphene (Clomid, Serophene) is an oral anti-estrogen medication that you take in the first part of your menstrual cycle. If clomiphene alone isn't effective, your doctor may add metformin to help induce ovulation. If you don't become pregnant using clomiphene and metformin, your doctor may recommend using gonadotropins — follicle-stimulating hormone (FSH) and luteinizing hormone (LH) medications that are administered by injection. Another medication that your doctor may have you try is letrozole (Femara). Doctors don't know exactly how letrozole works to stimulate the ovaries, but it may help with ovulation when other medications fail. When taking any type of medication to help you ovulate, it's important that you work with a reproductive specialist and have regular ultrasounds to monitor your progress and avoid problems.
