My grand daughter age 4 years after coming from school at 1300 hours and after lunch goes to sleep and doesn't wake up. Inspite of our all efforts she doesn't wake up. She is not taking her food and we give her milk. In morning she wakes up at 0530 and is very fresh and active and goes to school happily. Please advise what is the reason for this.

I presume, she takes home food for her lunch and then she doesn't take dinner instead you give her milk and then she continues to sleep to get up early morning fresh and active. If so try not giving milk, she will wake up with hunger. After normal dinner (solid food) engage her in some sort of play or story telling, thus breaking a behavioral pattern. Then she can go to bed around 9 pm and wake up at 6-7 am. This is a behavioral problem and should be tackled like wise by increasing interaction skillfully.

Nursing improperly often becomes a big reason for tooth decay and caries in babies. The way out is to get the baby tooth cared for from the very first day. This form of tooth decay is associated with nursing, when the baby sucks the pacifier, or takes other sugary juices or drinks, or formula milk from a nursing bottle.

The main causes of nursing bottle caries:
The babies are often fed in nursing bottles or feeding bottles, and the bottle nipple often stays in the mouth for long; sometimes overnight also if not taken care of. Again, the babies are pacified with pacifiers, which are often dipped in honey or a sugar solution, and this again brings in more reasons for dental caries. In any of the situations described, the baby's newly formed teeth which are susceptible to all forms of decay and damages while being gentle and new can get cavities. The sugary liquids and milk can form a coating on the teeth which brings in cavities.

How to prevent the nursing caries:
The best option to prevent is not to leave the baby with the bottle or pacifier for long. Again, the pacifier or the bottle should be always washed properly, sterilized and not left in the mouth of the baby uncared, especially during sleep time, when the baby sleeps while sucking onto them.

There are some steps to follow to prevent the early tooth decay and maintain baby dental hygiene. They are:

1. Even before the first tooth comes out, massage and clean the baby's gums with a clean washcloth or soft cotton everyday, and after every long feeding.

2. You don't need toothpaste to start brushing the first few new teeth that come out. Just brush without a toothpaste and use a very soft gentle brush to form the habit. In areas where no tooth has come up, brush and massage the gums only.

  • When all the teeth have come up, start flossing everyday
  • Try to take the baby for a dental checkup at least every year if not every six months.
  • Try to cut down on the consumption of extra sweet beverages. Add some water to the fruit juice to dilute the concentration and sweetness.

All these steps mentioned are great to start healthy and good dental care and hygiene, and if you follow them well, then dental caries will be away.

Cut down on top feed while continuing breastfeeding feeding. You can add ragi fruit juices vegetable soups dal & rice bread/biscuit/rawa/porridge/chapatis in milk.

Hello doctor I have 3 children 2 boys and 1 girl all of them is by c section after my 3rd c section I had taking birth control pills from last 2 to 3 years Now I'm stop taking it I had stopped that pill from last 9 months now I want to get pregnant but I can't my period is regular my intercourse is also good but I didn't conceive why like this would you please explain. My next thing is I want to conceive twins is it good for me to try so if it is good for me to think about conceive twin pregnancy so you please suggest any medicine to improve the chances.

You have not mentioned your age but based on the information you have provided I believe the risks after three c-sections are high for normal pregnancy and more so for twin pregnacy. Because your uterus is not as strong as initially and there are more chances of uterine rupture and scar dehiscence (separation of layers of muscles at the site of stiches) which can be lethal for mother and the baby. If you still want to go ahead you should consult a gynaecologist for proper examination of your c-section scar and your fertility level if need be.

I hope all the investigations are done and diagnosis is proved. Thyroid swelling in children are uncommon and needs to be properly evaluated. Reactive lymphadenopathy if not responding to antibiotics I suggest you to get a repeat fnac of thyroid and lymph nodes.

Breast milk is best for the baby. Please consider if the milk is not enough or if it is just perceived to be not enough. A baby getting enough milk will sleep from 2-4 hours between feeds, generally passes 5-6 motions if under 2 months and 5-6 times urine per day. You can also get baby weighed to see if growth is adequate. If milk is not truly enough, you can use any of the popular brands of formula like nan pro, similac, enfamil etc.

Investigate to know cause, cbc, hb, cxr, mt. Look for any local pathology if any, improve personal hygeine. Check weight for the age, if low improve diet. Balaced diet+play+hygeine will ensure good health.

Building your child’s confidence and self image is an important part of raising a child. Self image can be defined as a child’s view of himself and his strengths. If a child is comfortable with his self image, he will have confidence in himself. A foundation of self confidence is key to a child’s success in his adult life. In their childhood years, a parent is the main source of building a child’s self worth or self confidence. Here are a few ways you can develop your child’s self confidence.

1. Avoid negative self talk: Children learn by watching their parents. Hearing parents berate themselves will make children do the same. Talking badly about yourself can reinforce a low self esteem. Hence avoid talking negatively about yourself and stop your child from doing it as well. If your child talks negatively about himself, give him evidence to prove the opposite or give them meaningful compliments. Also avoid calling your child names to shame him as these are likely to stay ingrained in his mind.

2. Celebrate uniqueness: Don’t make your child feel like he has to live up to the standards set by siblings or peers. Further, do not pressurize them to fulfill your dreams. Recognize your child’s strengths and celebrate his unique abilities. Help them develop their talents and set their own standards for themselves.

3. Let your child make decisions: Empower your child by allowing them to make decisions about simple things like what game to play or what to eat for dinner. Make them feel that their views are valued and thus teach them to be an active member of the family.

4. Let them work things out for themselves: Spoon feeding your child makes things easier at the time, but in the long run can have a negative effect on their self confidence. Be patient and let your child try and do things for himself. As he meets and overcomes new challenges, his confidence will grow. Give your child age appropriate chores to do around the house. This helps build responsibility and a feeling of competency.

5. Be genuine about your praise: Children are highly intuitive and can tell the difference between sincere praise and something being said merely for the sake of it. For example, if your child has made a drawing, instead of simply telling your child that he is an artist in the making be more specific and praise his choice or colours or ability of colouring within the lines.
