Hi doctor, I am breastfeeding my 1 year old baby girl. I would like to stop it asap. But she asks it more nowadays. She doesn't drink cow's milk either in feeding bottle r in glass. Suggest me some tips how to make her forget my milk.

Madam, You should have started top milk 4 to 5 months back itself. Now at least once you can give top milk. Later you can wean off breast feeding. If child refuse top milk, get back to me for further guidance.

Yellow teeth ruining your smile? see what's the solution it's not always whitening!

January is around the corner and along comes a new year with its numerous resolutions and promises on improving ourselves. Be it the latest diet or the promise to workout we all want to look and feel our best. If your dental resolution is to stop living with a yellow smile then here's a combination of home remedies and dental treatment tips to make you smile bright and white this new year.

Depending on the existing colour of your teeth and how bright you want your teeth to be, we have here some options ranging from the mildest to the most impactful colour change.

#1. Light yellow teeth: clean better!

Plaque and other food deposits can make your teeth look unclean and discolored. Most often the original white of your teeth can be restored by a simple clean up procedure that washes away all the discolored food particles.

#2. Yellow to brown teeth: eat better!

An indian diet with turmeric coupled with our addiction to anything from tea, green tea, coffee, red wine to even smoking can lead to excessive staining of our teeth. While it may seem impossible to avoid what we are habituated to it's equally necessary to compensate by frequent cleaning and whitening procedures based on your frequency and level of exposure to these staining substances.

#3. Natural white: naturally

Home remedies which have no side effects like a pinch of baking soda mixed with crushed strawberries or 3-4 drops of lemon are often used to whiten teeth by scrubbing away the stained particles. Malic and citric acid present in strawberries and lime respectively gives an instant whitening effect without harming the natural layers of the teeth.

#4. Frequent yellowing: brighten at home

If you're looking for more than just a natural clean white colour but want it done with as little chemical exposure as possible then home bleach is the best option for you. Removable, transparent, custom made plastic moulds of your teeth can be made in just one session. You can then put in a few drops of a low concentration gel bleach into these as they snugly fit onto your teeth. Keep them in the mouth for 45 minutes daily for up to 7 days. This procedure is done as directed by a registered dentist. This gives you a bright white colour with minimal or no side effects and the additional benefit is that it can be repeated just before an occasion or every time you feel there is yellowing of teeth.

#5. Brilliant white: in under an hour!

If you want nothing less than super bright then a very standard and recognized procedure is that of the office bleach done in the dental chair in under 45 minutes. Here we use a more concentrated bleach to get you noticeable and long lasting results. Done in cycles every 10-15 minutes to show you the colour improvement and prevent side effects and discomfort. Certain precautions must be observed for 7 days after the bleach and it is recommended once annually to maintain the colour.

A word of caution for the over the counter products that claim to work brilliantly to whiten teeth but overuse or unsupervised use of the same may lead to irreversible tooth damage.

Even with this array of information and multitude of options available for your convenience- a consultation with your dentist for your perfect white smile, will work best before you decide to take matters into your own hands.

You could have yellow teeth due to one of the following reasons:


    Genetically yellowed or dull when formed.
    Age-related reduction or thinning of enamel and exposure of dentin.
    Excessively stained diet or habits.
    Appear yellow due to fairer gums, lips, complexion or lighter eye colour.
    Dental diseases which stain or eat away your enamel.
    Improper oral hygiene habits.

Crying may be due to colicky pain (abdominal pain) due to gas and so burp well after each feed and give Colicaid drops. Frequent loose motions are common in this age.

My baby girl was born on 3rd october premature at 32 weeks. She was in NICU for 3 weeks due to infection. Her OAE test results REFER however the child responds to sound of rattles. But the OAE test detects the loss of hearing to serai frequencies and may repoat it at a later stage he 4 types of otoacoustic emissions are as follows: Spontaneous otoacoustic emissions (SOAEs) - Sounds emitted without an acoustic stimulus (ie, spontaneously) Transient otoacoustic emissions (TOAEs) or transient evoked otoacoustic emissions (TEOAEs) - Sounds emitted in response to an acoustic stimuli of very short duration; usually clicks but can be tone-bursts Distortion product otoacoustic emissions (DPOAEs) - Sounds emitted in response to 2 simultaneous tones of different frequencies Sustained-frequency otoacoustic emissions (SFOAEs) - Sounds emitted in response to a continuous tone. Pure-tone (PT) audiometry measures throughout the outer ear, middle ear, cochlea, cranial nerve (CN) VIII, and central auditory system. However, OAEs measure only the peripheral auditory system, which includes the outer ear, middle ear, and cochlea. The response only emanates from the cochlea, but the outer and middle ear must be able to transmit the emitted sound back to the recording microphone. OAE testing often is used as a screening tool to determine the presence or absence of cochlear function, although analysis can be performed for individual cochlear frequency regions. OAEs cannot be used to fully describe an individual's auditory thresholds, but they can help question or validate other threshold measures (eg, in suspected functional [feigned] hearing loss), or they can provide information about the site of the lesion. Using current technology, most researchers and clinicians find a correlation between frequency-specific analysis of TOAEs/DPOAEs and cochlear hearing loss. However, at this juncture, the correlation cannot fully describe auditory threshold. Naturally, a correlation would not be expected for noncochlear hearing loss.

As of now there is no treatment, it can only be controlled by insulin. Yes there are newer technologies for delivering insulin.

You have mentioned, he doesn't like but not what he likes. His wt is definitely very low to his age. If he is not suffering from any ailment, give him all home made well cocked nutritious rich in caloric value edibles, he likes. Don't thrust your like on him. If he doesn't like milk, give him milk made products like curd, kheer and alike. Add ghee in his diet.

Sometimes, during consumption of certain food items many children get itchy throat, eyes, ears, asthma, sneezing, rashes, diarrhea and eczema. This can be due to food intolerance in children. It is a form of allergic disease in children. Every 1 out of 10 children suffer from food intolerance. These days, food allergies are becoming more and more common in children who are allergic to nuts, milk and eggs. In some cases it can also be life threatening. This condition is known as anaphylaxis. Milk and egg allergies are more common throughout the world.

Symptoms of food intolerance:

  1. Bloating

  2. Your bowels start becoming irritable.

  3. Stomach starts to ache.

  4. Cough and cold is a common symptom.

  5. Itchy skin rash

Causes of food intolerance in children:

  1. There are enzymes in your body, which help you to digest the food you consume. Some enzymes in your body are insufficient; which further causes food intolerance in children. For instance, lactose intolerance causes spasms, stomach aches, diarrhea and so on so forth.

  2. Food intolerance can also be caused due to chemicals, such as chemicals present in coffee, tea, cheese and chocolates.

  3. Some food products can also cause food intolerance such as undercooked beans containing aflatoxins can cause extreme stomach infections in children.

  4. Some children are also intolerant to certain food products which have salicylates such as majority of fruits, vegetables, herbs and also additives. Processed food contains a high amount of salicylates.

  5. Children can be allergic to antioxidants like artificial food coloring, artificial flavoring and preservatives.

What to do if you think your child has food intolerance?

  1. Take your child for a skin prick test. In this test, the skin is pricked with a thin needle and when the blood oozes out, supplements of different food products are put in that area. If that particular area becomes reddish and begins to itch, your child has allergy to that certain food product

  2. A blood test should be done. It verifies the level of lgE present in your blood and finds out if your child is suffering from food intolerance or not. If you wish to discuss about any specific problem, you can consult a doctor.

If proper dose of deworming is not given some eggs remain which are noticed later too much suger and under cooked non veg can help worms to grow do not give too much sweets hold nonveg exclude fish liq vidangarist 5 ml +liq arvindasav 5 ml=add 10 ml water gice twice a day for 45 days this will remove worms and cyst.

By what method did you do family planning. Tubectomy or vasectomy if tubectomy was done the recanalisation procedure can be done. By surgery fallopian tubes are reconnected. If vasectomy (male sterilisation) was done. Recanalisation is difficult but can be attempted if no anti sperm antibody is present. U can consult me online for further queries.

Oppositional Defiant Disorder in Kids - How to Deal with it ?

A child’s tantrums, especially during teens, are quite common. However, there could be some children who could be exhibiting an extreme version of these symptoms. This is known as oppositional defiant disorder.
Children with this disorder become easily irritable, angry, argumentative, defiant and feel vindictive against most elders (parents, teachers, and others). While this is something very common and can be ignored to be a part of teenage tantrums, the issue is when these symptoms do not seem to end. If they persist for beyond 6 months, it is time to worry. These may then begin to interfere with their daily activities including schooling, where they may not be easy for the teacher to manage.

Diagnosis of ODD: With the changing behaviour of teenagers, it is often difficult to pinpoint and say there is ODD. However, some guidelines for diagnosis are listed below. Angry/irritable, argumentative, defiant and vindictive. If these symptoms are seen for more than 6 months with no inducing reason, happens with non-siblings, and is affecting learning and playing, it is highly likely the child has ODD.

These symptoms can occur at home, at school, or in other settings – seen respectively in one, two, or more settings. Some of the symptoms are listed below.

  1. Repeated temper tantrums
  2. Anger bursts, swearing, using obscene language
  3. Extremely argumentative, especially with people in authority (teachers, parents, etc.)
  4. Annoying others and getting annoyed easily
  5. Noncompliance to rules and regulations at school and institutions
  6. Defending one’s mistakes and blaming others for it
  7. These result in poor academic performance, antisocial behavior, substance abuse, and higher suicidal tendencies.

Treatment

Treatment depends on the presenting symptoms, the age of the child, and supportive care available. The child should be able to actively take part in psychotherapy to reap good benefits. It would otherwise be a task with no results.

  1. Psychotherapy will help the child improve its cope and express and control anger. This also improves problem-solving skills.
  2. Cognitive-behavioural therapy tries to mould the behaviour.
  3. If required, the parents also would be involved to improve family’s involvement in the treatment. Caretakers are given special training if required so that they can support in long-term medical care.
  4. The child also needs to be trained for appropriate behaviour under different circumstances.
  5. Rewards for positive behaviour and punishments for negative behaviour are useful ways.

Prevention: Early identification can help in minimizing distress to the family and help in the early arrest of the disease. The family is also taught basic and simple steps which can help in supporting therapy. Early rejection at school and loss of learning, can happen which can be managed with early intervention. A 3 year old with high fever and vomiting requires ome paracetamol syrup, sponging whole body with cold water, cold water pad over the forehead to reduce the temperature. Small sips of cold water fed by spoon continuosly. I f no relief within few hours, don't delay approach family doctor or a child specialist.

Usually children don't eat well during 2nd year. Try to give foods with different taste. De worm him. Weight also will not increase during 2nd year.
