Hello Lybrate user, what I understand from your query is that there is sensation of a crick in back of neck, do not worry this are early symptoms of ligamental and muscular weakness what you need is gradual and progressive neck muscle strengthening exercise on regular basis which will help you to improve your posture, bad seating and sleeping posture is one of the major cause of your discomfort ,i strongly recommend you consult a qualified physiotherapist who is expert in kinesiology who will be in better position to help, avoid going for unnecessary investigation, in meantime you can start taking homeopathy medicine for speedy recovery,

Swine Flu in Pregnancy

Swine Flu in Pregnancy

Swine Flu in Pregnancy

What is Swine Flu?

It is a respiratory(associated with breathing) disease caused by a virus. This virus is similar to the influenza virus responsible for causing the common cold.

The scientific name of the virus is H1N1 type A influenza virus. This strain was earlier known to infect pigs(swine family), thus the name swine flu.It is a new strain of the virus, previously unknown in humans, hence, there is no known immunity in humans against this virus.

What are the symptoms:

There are three main symptoms :

Fever,
cough,
shortness of breath.
and a combination of one or many of these:

Headache, aching muscles, running nose, sneezing, chills, tiredness, loss of appetite, vomiting, and diarrhea.

It is usually a mild disease for most people. Symptoms appear quickly and last for almost for one week to ten days.

How do I know if I am at higher risk?

Certain categories of people seem to be at higher risk for complications from swine flu. These are:

Asians, especially people living in poorly ventilated colonies.
People having associated ailments like - asthma, diabetes, paralysis, previous strokes, spinal problems, and jaundice.
Extremes of body weight;very thin built or obese people.
Smokers
Teenage mothers, women in their second and third trimesters are at higher risk.
How can swine flu harm my baby?

For the majority of pregnant women, swine flu does not cause any serious damage to the mother or the baby.

Fever in the first three months may increase the risk of neural tube and birth defects.
Fever during labor may cause neonatal convulsions, encephalopathy, cerebral palsy and fetal death.
Pregnant women should always take fever seriously and contact their doctor when the temperature goes more than 100 degrees.

How does swine flu affect me during my pregnancy?

Active infection by swine flu virus may cause miscarriages, preterm delivery, and adverse delivery outcomes.
There is a higher risk of hospitalization/ ICU admission during pregnancy.
Chest infections/ Secondary bacterial infections are common resulting in pneumonia or even brain infections.
What can I do to protect myself?

Protection can be at three levels:

1) Taking general precautions- which is most useful for prevention.

2) Use of vaccines.

3) use of drugs

Use of face masks will not prevent swine flu. Focus instead on hygiene and clean surroundings.

It is safe to consume well cooked pork and pork products.

Diet Drinks vs. Regular Beverages: Which is Worse For Your Health?

M.Sc - Dietitics / Nutrition (Delhi University)

Diet Drinks vs. Regular Beverages: Which is Worse For Your Health?
Why choosing diet drinks over regular beverages is not good for your health.

Have you switched to diet drinks from regular ones considering your weight or calorie intake? it probably is not the right choice. The reason is drinking diet beverages harms your body and will only result in more weight gain.

Although diet beverages are calorie free, they are added with artificial sweetener called aspartame which is reportedly 200 times sweeter than table sugar. This chemical adversely affects many body functions.

Effects of diet beverages on body in this list might suggest why you should continue your regular beverages occasionally, instead of shifting to diet drinks.

1. Increases belly fat
According to studies, the people who drank diet beverages showed increased waist size than the ones who drank the regular sugar drinks. The reason is believed to be the artificial sweetener and changed attitude towards food habits as the diet beverages easily justify high calorie food like burger, fries etc in turn. This is a concern as increased visceral fat, a body fat stored within abdominal cavity, is associated with increased risk of heart diseases and type 2 diabetes, the most common diabetes related to problems with insulin regulating the blood sugar.

2. Damage to brain
Aspartame is also known to damage brain by affecting the nerve cells and nerve endings in the cerebellum which are important for motor skills.

3. Low bone density
Drinking diet drinks lead to low bone mineral density and osteoporosis especially in females. Pelvic bone is most commonly affected in women by regular consumption of diet beverages.

4. Harmful for teeth
The diet beverages also contain large volume of citric acid which causes tooth erosion destroying tooth enamel in the regular users. The risk is comparatively less in the regular sugar drinks users.

5. Side effects
As aspartame may also interfere with different activities of brain, there are many side effects of diet drinks like headache, change in vision, vomiting, abdominal pain, diarrhea, memory loss, depression etc.

Recurrent Miscarriage - Know The Causes And Management

MBBS, DGO
Gynaecologist, Delhi
Recurrent Miscarriage - Know The Causes And Management

Miscarriage refers to the spontaneous loss of the fetus before one completes the 20th week of pregnancy, taking into account the period from the conception to gestation.  It usually happens in the first trimester of the pregnancy; that is within the seventh and the twelfth week of conception. Recurrent miscarriage is when one suffers from multiple miscarriages in a row.

Possible Causes:

  1. Abnormally-shaped Uterus: Some miscarriages, particularly late ones, are thought to happen because the uterus (womb) has an abnormal shape.
  2. Polycystic Ovary Syndrome (PCOS): Women with this condition have many small cysts in their ovaries. They also tend to have hormonal problems, including high levels of insulin and male hormone in the blood, which can lead to recurrent miscarriage.
  3. Infection: Some serious infections can cause or increase the risk of single miscarriages. These include toxoplasmosis, rubella, listeria and genital infection. But it is not clear whether infection plays a role in recurrent miscarriage.
  4. Diabetes and Thyroid Problems: Uncontrolled diabetes and untreated thyroid problems can cause miscarriage. But well-controlled diabetes and treated thyroid problems do not cause recurrent miscarriage.

Risk Factors:
Your risk of recurrent miscarriage is higher if:

  • you and your partner are older; the risk is highest if you are over 35 and your partner over 40;
  • you are very overweight. Being very underweight may also increase your risk.

Each new pregnancy loss increases the risk of a further miscarriage. But even after three miscarriages, most couples will have a live baby next time.

Testing After Recurrent Miscarriage:

If you have had three miscarriages in a row, you should be offered tests to try to find the cause. This should happen whether or not you already have one or more children. Testing is usually offered two early miscarriages (up to 14 weeks) because these are often due to chance. But you might be offered tests after two early miscarriages if you are in your late 30s or 40s or if it has taken you a long time to conceive.
If you had a late (second trimester) miscarriage, where your baby died after 14 weeks of pregnancy, you should be offered tests after this loss.

  1. You can opt for blood tests to check for sticky blood syndrome or APS. Tests would look for antibodies that would help treat the condition. Antibodies are chemicals produced by the body to combat infections.
  2. Get an examination done should the doctor suspect chromosomal abnormalities and in case it is diagnosed, both of you can consult a clinical genetics specialist for genetics counseling.
  3. Your doctor will recommend an ultrasound scan to trace any type of abnormality that may make a pregnancy futile, for instance, a short or a fragile cervix.

Your hopes:

It is natural to pin your hopes on testing as the answer to your problems. But there are three reasons why it may not be the answer you’re looking for:

  • A cause may not be found; when this happens your miscarriages are called ‘unexplained’
  • Even if a cause is found, it may not be treatable;
  • Treatment may not lead to a successful pregnancy. This can happen if a pregnancy miscarries for a different reason than the one being treated. If you wish to discuss about any specific problem, you can consult a Gynaecologist.

Hi Shashi. I understand your problem. Do not stress so much over this issue. Stress itself may cause delyed period. As far as your fear of pregnancy is concerned, you can get a home pregnancy test kit and check yourself now itself. You can safely assume the result to be correct as after 45 days the result would be accurate. We can handle the issue from there as per your result. Do not worry so much. All the best

You should have taken I pill. But as you have not, wait for normal periods. You never know you might have missed your ovulation time, don't worry. If you do not have periods on time. Wait for 5 days. Do a pregnancy test and then go to a gynaec. Even stress causes delay in periods.

Self Monitoring of Blood Glucose - SMBG Chart

MBBS, Diploma in Diabetology
Endocrinologist, Mumbai
Self Monitoring of Blood Glucose - SMBG Chart

Self-monitoring of blood glucose (smbg) is very crucial for those diabetics who are on insulin or on certain oral anti-diabetic drugs which can cause hypoglycemia. To achieve best glycemic control it is essential to know how your daily blood glucose levels are fluctuating in an entire day.

It is easier for a diabetes specialist to adjust drugs or to adjust insulin dose if good smbg record is maintained by the patient.

Normally we check fasting and 2 hr post prandial blood sugar levels in the lab for routine follow-up. There is a bright chance of sugar excursions in the other post meal readings which get unnoticed. Even in case of hba1c or glycated hemoglobin test we get an average result of blood glucose of last 3 months.

In practice, I have seen several patients who had to fast and post prandial blood glucose lab tests in the normal range but hba1c levels below 7 is not achieved because they might have high blood sugars in other post meals which have got unnoticed.

To achieve the best blood glycemic control it is important to control blood sugar levels within normal limits during the entire day and not just fasting or post breakfast levels.

Interpretation of smbg chart:

In the above image of smbg chart, I have mentioned 7 point charting system where you can fill your blood glucometer readings which you take on several spots in an entire day.

Seven points includes:

    Fasting or before breakfast reading
    2 hrs after breakfast
    Before lunch
    2 hrs after lunch
    Before dinner
2 hrs after dinner or bedtime
    At mid night 3.00 am

This seven point smbg charting approximately covers entire day glucose fluctuations to some extent (of course not as accurate as cgms).

I have also mentioned a comment column in the above smbg chart where you can note down change in food or change in insulin dose or any other changes that you have made which probably caused unusual readings like for example if hypoglycemia occurs at before lunch reading and you have skipped breakfast or had very little snack at breakfast you can mention that.

You do not need to check daily all the 7 points, you can check any one or 2 points randomly depending upon your sugar control.

More uncontrolled blood sugars require more frequent smbg glucometer monitoring.

Monitoring of blood glucose at home in case of diabetic pregnancy or gestational diabetes is also important where frequent monitoring and smbg charting of glucometer readings helps a lot to the patient.

I hope this article will help you to control your blood sugar levels more efficiently.

If you find that some of the post-meal or pre-meal readings are high or low, please consult your diabetologist with smbg chart to adjust your medication or insulin dosage.

If you like my article please share it with your friends and family.

Awareness is prevention!
