I am 23 years old male and recently encountered a fracture in my palm. The bone connected to my little finger got fractured in angular manner. Shall I go for a POP or a surgery where a wire will be inserted?

Fellowship in Neurological Rehabilitation, BPTh/BPT
Physiotherapist, Gurgaon
Hi, all dislocations or fracture dislocations should to be put back in joint (reduced) and held in joint until healed. Sometimes the injury will pop back into joint and be stable enough to hold in a plaster cast. If so they need careful supervision with repeat x-rays for 2-3 weeks from injury to make sure they do not slip out again. If the joint is not that stable it will need to be held with 1-3 wires passed through the skin known as k wires. These usually protrude through the skin so they can easily be removed in the outpatient's clinic. Some surgeons will bury the wires under the skin. Only occasionally is open surgery required i. E. A formal cut is made on the back of the hand over the injury. The wires typically remain in place for 4-5 weeks. During this time the fracture and wires will need support in a plaster cast either a full cast or a partial cast. Any fingers or parts of fingers not held by the plaster should be moved to reduce stiffness. Most patients do very well following these injuries. After an immobilization period, rehabilitative exercises should be performed to regain strength and mobility and to minimize chronic pain and stiffness. Thanks.

Dear sir, if this pain is severe with the first step in the morning and gradually reduce after walking a few step, then you might be suffering from plantar fasciitis, with a spur may be. 1. Apply cold pack, 15 min wrapped in cotton bag or handkerchief, for 3-4 times. 2. If you are going for walks, take a break for few days. 3. Don't walk bear feet in the house or outside. 4. There are few stretching and exercise that can help in reducing the pain. 5. If you have foot alignment issue or any other condition then using corrective orthotics and footwear can help. Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. This pain could be due to certain degenerative changes in your shoulder joint or its associated structures, or could be just muscular in nature. If there is no history of any injury, or any s/s of inflammation or infection like swelling, skin redness or local warmth etc, I will advise you initially to do: - ice packs at home 3-4 times in a day - gentle anti inflammatory medication like tab etoshine or tab paracetamol can be taken for 5-7 days or as sos basis - physical therapy with tens or laser under supervision of a physical therapist - analgesic spray like dynapar qps spray for local application - adequate calcium and vitamin d intake as per requirement. Let me know in 2-3 weeks time how you are feeling, otherwise we will get some investigations done for you. Do not hesitate to contact me if you need any further assistance.
