I am 76 old. I have unhealed incision done for removal of bunion on the ankle of right leg. It has healed about 90% but balance of 10% formed as tough skin over the incision. There are no pus or blood woos from the crest. Only problem is the crest is not falling off. But every evening pain appears every day to quench the pain I have to apply some antiseptic cream and pain disappears after about one to one half hours. The question is why it is not healing? and what should be done? Due to this I have sceitic pain on the left leg & buttocks. I am in Bangalore, India. Can you kindly suggest a doctor who can help me in curing this. I have already visited respective 3 to 5 leading specialization. They have done skin grafting twice and it has failed. There is evidence that certain dietary supplements and vitamins can help with certain types of pain. Fish oil is often used to reduce pain associated with swelling. Topical capsaicin, derived from chili peppers, may help with arthritis, diabetic nerve pain, and other conditions. There's evidence that glucosamine can help relieve moderate to severe pain from osteoarthritis in the knee. Do reply back for private consultation. And detailed treatment plan.

You will need to deal with your depression with a counselor, and if that person advises you to meet with a doctor you will need to do so too. You need to cooperate with both therapies to the best success. In the meantime do some of these recommendations very faithfully: Daily exercise of at least half an hour is a must. Even if you go to a gym, ask for aerobic and/or callisthenic exercises with whatever else you are doing. A healthy body harbors a healthy mind. With regard to memory, it is very important that your brain and body is ideally rested to be able to recall whatever is required, rather comfortably. Puzzles pose problems to the brain that help it to use new pathways and neurons, which give the brain considerable exercise. It taxes the left brain to use logic to solve the myriad possibilities which other activities do not stimulate. Crosswords are excellent for vocabulary learning and use. Jigsaws and Rubik cube stimulate different permutations to finally settle on the most likely one. Picture completion and anagrams help approach problem solving from several angles. Do Sudoku, and memory co-relation activities and skills. Have a good night’s sleep, have a good breakfast of more proteins, meditate often, remain free of stress, eat a lot of fiber (whole grains, fruits and vegetables), nuts, avocado, eat dark chocolate, consume less of fat and use olive oil instead, do Yoga meditation exercises, etc. You need to check out if you are stronger visual or auditory. The visual is a better mode than the auditory. However, if you combine the two modes, you will get the best concentration. Have a special place of learning, which should be well lit, with soft painted walls, well-ventilated, with no distractions. When you get bored, study by writing. If you repeat learning at least five to seven times, you will apparently remember for a longer time. Sit comfortably but do not slouch. The reading material should be of a fairly large print. Study at small intervals of about 40 minutes and then take a break or change the subject. Short-term memory is a faculty of the left brain, and long-term memory is a feature of the right brain. When people are stressed, they tend to favor the right brain and abandon the left brain, where short-term memory resides. So, it is really very simple: deal with the stress and activate left brain functions. Here are a few suggestions to activate left brain function: shut your left nostril and breathe, move your eyes from right to left and vice versa for at least half a minute at a time, and do callisthenic exercises with some form of counting, regularly. There is a new exercise called Super Brain Yoga, which is done by holding the right earlobe with your left thumb and index finger, and the left earlobe with your right hand’s thumb and index finger. In this position you must squat down and rise up and do this for five minutes every day. There are some memory enhancing techniques and study methods that your teacher will be able to guide you with. If your home life is full of distractions and stress, it is likely to affect your memory, adversely. In that case, I suggest that the family goes for counseling too. The following foods do help too: Blueberries, walnuts, turmeric, Spinach, tomatoes, broccoli, acorn squash, green tea, oily fish, boiled egg, turkey, apples, oatmeal, leafy greens, lentils, pumpkin seeds, avocado, cinnamon, thyme, sunflower seeds, and red wine. Avoid sugar and junk food.

What should one do who is suffering from severe depression for 18 years and finding his surrounding meaningless, gloomy, goal-less, excitement-less, unsuccessful & alone; due to the negligency, unpardonable & life-spoiling mistakes, less-co-operation & misguidance; in spite of showing so much talent & possibility during his teen-age?

Reparenting Technique, BA, BEd
Psychologist, Bangalore
Yes, 18 years is a long time for depression and it will definitely not feel good. But that is an obvious statement, what is not is the fact that you must always combine medication with therapy. It is so much better and you would have developed a lot of coping mechanisms by now and may have been able to conduct yourself to the point of even enjoying life to a certain extent. Even now it is not too late, although it might be a little more difficult. Nature and the way we have been made, have fortunately built-in certain structures that let us live close to normal lives, especially up to our teens. After that, if the little signs that there is some problem were not noticed and dealt with, then the issues will come up for the last time seeking a resolution in late adolescence. But for some reason it was not handled well or in time, and therefore this situation where it has now become a permanent negative trait that has to be dealt with more aggressively. Now it is time to seek counseling in tandem with the medication; and you are to keep with the counseling until there is a resolution. Be cooperative and follow all the directives and requirements and you will see a gradual but sure improvement. You must be active and work towards your reformation. Many lifestyle changes need to be made to progress away for the condition. When you are depressed, you will feel lazy though I would like to call it lethargy. There is an inertia and lack of energy to even think, let alone act. You are required to literally do the opposite of what you feel like doing. Depression makes you want to withdraw, lie down, sleep, become lethargic, slow down, make you too lazy to even think etc: exactly like you described. So when I suggested doing the opposite, you will need to fight this disease. You must become active; stay upright during the daylight time; meet people; never sleep during the day, wake up by 6 am every day, play some active games, especially contact games, do physical exercises, talk to people and join some social clubs, eat more of proteins and vegetables, attend yoga classes etc. Watch sitcoms on tv or comedies and cheer yourself up. Go for excursions in groups, for outings, camps, conferences, and religious conventions. Get a pet dog and spend time training it, exercising it and relating to it. Expose yourself to some sunlight every day, at least 30 minutes but not in the scotching heat. Whatever happens, please incorporate these three important adaptations in your life: always be responsible, be respectful, and be functional. If you did these three things, lots of things will go well in life. Please pray and have faith in god to alleviate your sufferings. Don? t wait for others to help. Use your own motivation, which might be at its lowest, but persevere and win this battle. Now go out there and just do it: nothing is too late in life. Leave the past and don? t relate to the future too much: the present is where your solutions are and you should stay focused in the here and now, always.

Bipolar Disorder - 7 Signs You Should Be Aware of

MBBS, MD Psychiatry, DNB Psychiatry
Psychiatrist, Nagpur
Bipolar Disorder - 7 Signs You Should Be Aware of

Symptoms for Bipolar disorder varies from patient to patient. Bipolar 1 and bipolar 2 can vary in terms of signs and symptoms. Both diseases have a different set of symptoms and they are diagnosed according to the merit of the symptoms.

Bipolar 1
Bipolar 1 is characterised by a manic episode. This may be followed by a hypomanic or depressive instance. Symptoms of mania can cause major impairment in life and extreme cases may require hospitalisation. Bipolar 1 patients can suffer from psychosis as well.

Bipolar 2
Bipolar 2 is characterised by a hypomanic instance that lasts for at least four days and one depressive instance lasting for a minimum of two weeks. Bipolar 2 doesn’t involve a manic episode. Frequent mood swings and violent behaviour can be major challenge in a patient’s life.

Warning signs of manic and hypomanic behaviour:
a. Quick passing thoughts
b. Unusual talkativeness
c. Distraction
d. Increased social activity
e. Inflated sense of self-esteem

Symptoms of bipolar disorder

  1. Anxious distress: Apprehensions about not finishing a task on time, feeling of restlessness, feeling of losing control, difficulty in concentrating due to distress, fearing an awful event will occur in the near future, etc., are key symptoms of anxiety distress.
  2. Melancholic features: Patients suffering from bipolar disorder like to be left alone. They may not seek pleasure in any activity and even good things may not have much effect on them.
  3. Catatonia: This is a condition wherein, a person stops reacting to the environment. Not speaking and holding unusual body positions are some signs of these symptoms. There might be a tendency of mimicking too.
  4. Psychosis: This is one of the classical symptoms of bipolar disease. It is characterised by hallucination, constant depression and delusions.
  5. Seasonal pattern: There is a pattern of manic, hypomanic and depressive episodes throughout the year. The pattern might change within a month or season.
  6. Rapid cycling: The number of mood swings can exceed four or more times in a single year.
  7. Peripartum onset: This is a condition where bipolar disorder symptoms are witnessed in a pregnant woman during pregnancy or within the four weeks following delivery.

Symptoms in children: It is difficult to find symptoms of bipolar disorder in children and teens. Along with bipolar disorder, kids are most often diagnosed with other mental health problems as well. The problems can result from stress, trauma or chemical imbalance of the brain. Perhaps the biggest symptoms of bipolar disorder in children and teens are their frequent mood swings, chronic depression and feeling of despair. There might be a tendency of oversleeping as well. Reaching out to a doctor or professional help is very necessary for cases related to children and teens. If you wish to discuss about any specific problem, you can consult a psychiatrist and ask a free question.

Hi, my 5 years son is hyperactive and he got angry many times he make loud noise every time, from 2 months he started to speak single word like water, chabhi, toy but cannot make sentence he is not able to make friends in class he is not seating in class or home for more than 5 min. Pls advice how to calm him & make matchure like other kids of same age.

MS - Counselling and Psychotherapy, Master of Arts in Clinical Psychology, Bachelor of Arts - Psychology and English Literature

Hi lybrate-user First of all do not pressurize your son by asking or demanding him to be quiet or demanding him to behave like other kids. Try to control him only during emergency situation for his safety as well as for others. Otherwise let him be his oewn self nothing wrong. Kids will change in the due course of time. First of all adults or family members stop arguing or fighting in front of him, try to avoid as possible as you can, try to control your emotions as well, and try to be as social, loving, polite and kind to others in front of him, bcoz the first role model for kids are their parents, then their siblings, uncle, aunt, and other relatives. Try to maintain, peace and harmony at home. Try to bring in a warm atmosphere at home. You can definitely advise your son regarding the same when he comes to you and sits next to you more affectionately, that is the perfect time to teach him how to behave, good and how to avoid bad qualities. They will definitely register the same even though they appear to have not listened to you in due course they will change. Do not worry. Be polite and loving to kids. If you need further assistance you can contact me either through text chat or online consultation yes you are most welcome. With regards

3 Ways To Manage Chronic Depression!

Are you suffering from extreme depression? Does your depression episode continues for a long time and makes you feel too tired to get out of bed? Chronic depression is capable of turning your life upside down, causing turmoil with all your activities, thereby, spoiling your regular life totally. At times, a person can get so drained by depression that getting out of bed seems impossible because of fatigue. It is quite difficult to deal with such situations.

Here are some important tips on how to overcome your chronic depression and get out of bed.

  1. Be depressed upright: It is likely to be driven by the voices in your head during your depression. You should respond by replying to the voices that you are depressed, but you are going to be depressed outside. This is important, as activity and depression do not go well together. You should try to stop your thinking totally and put yourself in an automatic mode. Preparing to deal with thoughts from before is beneficial. When your body is in motion, it is easier to deal with the thoughts.
  2. Try to make it to the shower: When you do not feel like leaving your bed at all, you should try making it to the shower somehow. Taking a shower makes you a bit relaxed and diverted and you can expect relief from your depressive episode. This is an effective way to get yourself up and running from bed, instead of isolating yourself under the blanket. Taking a shower might bring you some hope and encourage you to step out of bed and do something productive. Even if you decided to be in bed all day strictly, a refreshing shower is likely to change your mind.
  3. Bribe yourself: You can try bribing yourself as a reason for getting out of bed. You can remind yourself how good you will feel after having a cup of hot coffee, or how nice it would feel to listen to a certain song while going to work.

It is important to give yourself a purpose or reason to get out of bed. Many people believe that they might feel like leaving the bed in spite of being heavily depressed, if they had a worthy purpose. It is very important for you to get involved in something. You might hate a job, or hate working, but it will give you a reason to divert from your depressive thoughts. If you wish to discuss any specific problem, you can consult a psychologist.

I am having lots of desire for sex. When I look at a women's boobs I go mad. What should I do to control myself.
There is no need to be alarmed to this extent. Between the ages of 16 to 18 years, for a duration of almost exactly two years, all boys and girls will have a spurt in the production of the male sex hormone, testosterone. For some people, this is delayed by a couple years and more too. Due to this hormone there are three identified effects, all because of this chemical: you will become sexually active; you will become aggressive and rebellious; and you will all suffer acne and pimple problems. You have no choice because this is a body clock mechanism that gets activated and over the two years that it prevails, it will gradually subside and return to normal. But during this time you could (driven by the hormones) do something inadvertent that there may be no turning the clock back. So you need to exercise control and channelize this energy through games and sports and hobbies. Also, you would have developed secondary sexual characteristics and they seek to be used and seek fulfillment. Again this is not a problem but the drive can be very bad for those who do not have sufficiently well developed value systems or a good conscience. These kids can lose control and become reckless. Most kids have a problem, like you do, around their sexual urges and may resort to acting out in the form of masturbation, pornography, and other sexual exploits. If you indulge in any of these you can get into a guilt trip or develop an addiction that may plague you for a long time. There is no threat to your mental well being. Your character will certainly be tested, and it will reflect your development in this regard. The best thing to do is to get a sex education from a counselor and ask all the questions you wish answered. Otherwise you will want to experiment or seek information from your peers who are not adequately qualified to really do justice to your queries.
