Hos should I decrease my hemoglobin level. Now its level is 20. How should I decrease it at normal level. I stop smoking also.

MBBS, DPM
Psychiatrist, Bangalore
Dear lybrate-user, Congratulations for stopping smoking. When you smoke, your lungs need more oxygen to clean blood properly. So more red blood cells and hemoglobin. Now that you have stopped smoking, your lungs will start functioning well. There is no need for excess oxygen -> more red cells -> more hemoglobin. In due course, hemoglobin will come to normal level, if you continue to breathe well WITHOUT smoking. You can do breathing exercises to speed it up.

Child is having autism. His weight -15 kg, age-5 years old. Please suggest a diet.

MD - Psychiatry
Psychiatrist, Mumbai
Autism spectrum disorders (asd) are developmental disorders that affect children by disrupting their ability to communicate and interact socially. To reduce a child's symptoms of autism, parents often try alternative treatments such as specialized diets. Lately, the gluten-free/casein-free diet has grown in popularity. Some parents report improvements in autism symptoms with this dietary regimen. Little research has been done, though, on the gluten-free/casein-free diet for autism. When following this strict elimination diet, all foods containing gluten (found in wheat, barley and rye) and casein (found in milk and dairy products) are removed from the child's daily food intake.

What is making you feel so weak inside- try to gain strength from the beautiful relationship you had with your loved one.- this loving relationship itself will give you the strength to go on. Be the person your loved one always wanted you to be. Give someone else around you a reason to smile, someone who has not been so lucky as you to have enjoyed a loving companionship. The more love you spread in the memory of your loved one, the more you will be thankful to god for having sent such a nice person in your life, no matter for whatever little time- at least you had someone close. Think of those who are so lonely that never experience what god gifted you! so, bring smiles on people's faces and you will be smiling in return! god bless you,

That must be difficult for you to be spoken to in that way. Try to understand what he wants from you visa versa. Couples do have expectations from each other. Try to do whatever you can yet draw your boundaries over things that are beyond your capacity. Learning to speak assertively will also help you to regain respect and dignity in the relationship.

Smoking or consuming smokeless tobacco is very harmful for your teeth. It can lead to various oral diseases like oral cancer, gingivitis, periodontitis and tooth decay.

I have been addicted to smoking for last 10 years and drinking for last 5 years. Now I want to get rid of my addiction. What should I do?

Reparenting Technique, BA, BEd
Psychologist, Bangalore
Why on earth would you still abuse these substances that can certainly lead you to an early grave? The symptoms are not encouraging either. There is only one reason, and that is, that you have not reached your ‘Rock bottom’ so that you will be stopped right in your tracks! Before that happens you must, on a war footing, exercise restraint and stop the use of all these substances. Since you have probably tried and found it extremely difficult I suggest you do one of the following: It sounds like you are in an advanced stage of alcoholism and tobacco use and need admission to a rehabilitation center to ideally help deal with this problem. There are no guarantees about stopping one’s addiction. But that does not mean we should not keep at it, over and over again until we succeed. Now it is difficult to give up all by yourself because you have become addicted to alcohol and tobacco and there is so much to do. Under such circumstances it is best that you admit yourself into a de-addiction center and stay there for at least 6 months and after that you can attend counseling with a professional. There are medicines that help with the drinking just to ease the initial craving and to act as an antabuse i.e. you will have a serious reaction, if you consume even a small quantity of alcohol or alcohol based substances. It has to be carefully monitored to see that you do not only consume the medication but you are also avoiding alcohol. Ultimately, it is your will power and the support that you receive from the medical fraternity and your close and dear ones. You must also learn to substitute and deal with the oral need, a rigid value system, the script issue, and of course take a look at all the genetic factors to plan a strategy not to get into what is called ‘cross addictions’ i.e. another form of addiction that may appear alright but is in fact as bad as the primary addiction. The center or hospital and the counselor will advise and guide you on several measures and precautions you will need to take to stay with your resolve. Even after the rehabilitation you must attend AA meetings and continue this support for a long time. The family will also need to attend some sessions and go for Al-anon meetings for their co-dependency issues. You cannot be treated in isolation because the family has gotten used to your drinking in particular and have made some unhealthy adaptations to somehow cope. Make a serious plan with whoever else’s support you can get and act on it fast. If you are not cooperative, there are some centers that will come and pick you up, after you are completely drunk, and admit you by force. This will however, require the family’s consent. Do not delay for any reason and stop fooling around with half decisions. This must be your last and final attempt and make it a successful one.

It will require at least 9 month. As your symptoms improves we can taper Lonazep gradually. But prothiaden need more time as it is the primary medicine. Anyway without psychiatrist advice you should not do it.

Study with a calm & relaxed mind. Try to enjoy your studies & stay positive. Do not take your studies as a stress. Take the following measures to improve your concentration & prepare well: •Make a study routine giving maximum time for studies. Also give adequate time for sleep, rest & relaxation. Between studies you have to relax also. This will help in sustaining your interest & concentration. Try to stick to your routine. At night look back how much you could follow the routine & reward yourself if the day was productive & to your satisfaction. In the form of reward you can give little extra time for relaxation next day, watch TV or spend some with internet, eat something you like, meet a friend etc. Encourage yourself daily. •Revise your lessons regularly. •Study relaxly with a calm mind & take small breaks during studies. •Group studies & group discussions are also helpful. •Do not try to memorize blindly. Try to understand your lessons. •Write down the main points & form mnemonics from them. Mnemonic devices are techniques which help to remember something. It’s a simple shortcut that helps us associate the information we want to remember with an image, a sentence, or a word. •Have a plan to manage your time of study. Make a routine for studying. Study for 30-45 minutes & then take 5-10 mins break. •Be confident of yourself & do not compare yourself with others. •If you feel stressed while studying, talk to someone, listen to some music or do something that relax you. •Create the environment in your study room similar to your exam hall. Then you will find your exam hall comfortable & familiar. •Expect exam outcomes rationally & what is within your capability. Impractical & over expectation that is beyond one’s capability brings in fear & frustration. •During these days of preparation for exam eat healthy, stay hydrated & avoid substances of abuse, as a healthy body prepares a healthy mind. •Every morning relax yourself adequately before you sit for studies; go for a walk, spend some time with nature, go for a walk, do yoga, breathing exercise or relaxation techniques. •On the day prior to exam sleep adequately. •Reach the examination hall before time so that you are relaxed & mentally prepared for the exam. Do not guess the exam outcomes just target to give your best. Think positive. Start writing with a calm mind. Be aware of the anxiety symptoms, & if you feel anxiety is building up, just concentrate on breathing in & out slowly or tighten & relax your shoulders or stretch your legs. You can also take a small break & drink some water •Do not look around; just concentrate on your paper. •Revise quickly once you finish answering. All the best for your preparation & for your exam. Give your best.

Even though going to gym is a good habit, whether it will get you out of your depression, is highly doubtful. The word depressed is a common everyday word. People might say" i'm depressed" when in fact they mean" i'm fed up because i've had a row, or failed an exam, or lost my job" etc. These ups and downs of life are common and normal. Most people recover quite quickly. With true depression, you have a low mood and other symptoms each day for at least two weeks The common symptoms are (few might be present in a person with depression) Feelings of sadness, tearfulness, emptiness or hopelessness Angry outbursts, irritability or frustration, even over small matters Loss of interest or pleasure in most or all normal activities, such as sex, hobbies or sports Sleep disturbances, including insomnia or sleeping too much Tiredness and lack of energy, so even small tasks take extra effort Changes in appetite — often reduced appetite and weight loss, but increased cravings for food and weight gain in some people Anxiety, agitation or restlessness Slowed thinking, speaking or body movements Feelings of worthlessness or guilt, fixating on past failures or blaming yourself for things that aren't your responsibility Trouble thinking, concentrating, making decisions and remembering things Frequent or recurrent thoughts of death, suicidal thoughts, suicide attempts or suicide Unexplained physical problems, such as back pain or headaches General medical conditions and substances causing depression also needs to be ruled out. Consult a psychiatrist and get evaluated.

Dear, Tobacco is addictive. Cigarette also contains 4000+ carcinogenic chemicals other than nicotine. The best method is to leave it and continue with your will power. There are other methods like Nicotine Replacement therapy and pharmacotherapy combined. Nicotine chewing gums too help as an alternative for cigarettes. But still you will need will power. You may consult a physician or deaddiction therapist or a rehabilitation facility for further advice. You can also opt for online deaddiction therapy. Take care.

Dear Mayank, Overthinking is not necessariliy a sign of OCD. However, assuming that your condition is indeed OCD, and it completely settles with fluvoxamine, the key thing to look for here is the duration of treatment. very often, the symptoms re-emerge because of premature stoppage of medicines. So first thing is to do a detailed interview and rectify these things. Psychotherapy (in the form of BT or CBT or CT) ca be done to make you self reliant and wean you off medications. But this will need considerable effort and time from your side as well as from the therapist. But that is really the way to go if you want to stop medicines. Take care!

I am addicted to chewing tobacco I want to know how I can release my addiction give me few tips.

If it is Hans and gutkha products, they are both very dangerous. I am so glad that your intention is to quit these habits. There are no guarantees about stopping one’s addiction. But that does not mean we should not keep at it, over and over again until we succeed. Both are tobacco products and will therefore have addictive components to them. It may feel like you can’t stop but that it really not true. If you do not try, then it will never happen. You must try and try again until you succeed. The problem with addiction is that you have two products which are both conditioned to various events in your daily life and as such it will make it difficult but not impossible. You could work with an addiction counselor to help you process the rehabilitation. It is important to first find out about the ill-effects of tobacco use to have a reason that alarms you about continuing in the habit. The information is really scary and true. The nicotine in tobacco stimulates the heart beat to raise the blood pressure in no time at all. So you must avoid it like the plague: it is just very bad medicine for you. In fact it is highly toxic too. You will need to stop consuming any tobacco related items with immediate effect. Nicotine also reduces the erectile capacity of your penis to hamper satisfying sex. You should start vigorous exercises, gradually increasing it over a period of time. You need to replace the oral urge with some healthy food substitute to satiate the need. You may also take hard candy or chewing gum. If the urge is too strong, then use nicotine chewing gum or nicotine patches, for a little while until the tobacco drive reduces, substantially. Both will help but I am inclined to favor the gum because it is an oral habit and it will occupy the mouth just as though you were still chewing tobacco. When you want it will be easier to give up the gum. Keep yourself occupied or engaged with interesting activities during the times when it is most tempting. Keep the company of non-users for some time i.e. at least for the first 21 days. Above all announce it to everyone that you have quit consuming Hans. You must persevere with the best support until you defeat this addiction. Counseling is very useful in the initial stages and when temptations are at their highest.
