Sir namasteI am a doctor. Working in a village hospital. My age is 32. I had an xray kub which suggested a calculus in right ureter near ischial spine. For which I took cef-o, flavoxate and potrait mb6 for five days. Again I did an xray kub which suggested a calculi at same place. It hasnt come down. Kindly suggest a remedy.ischial spine.

Dear , if the stone is at the level of ischial spine, it must be small in size, hence passable per urethra. Treatment you took will not help pass the stone out. It was wrong treatment. If you dot have pain now then try oral forced diuresis: 1take anti spasmodic like drotaverine hydrochloride 80 mg with diuretic like lasilactone 50 mg in the morning and drink plenty of water. Do not be impatient, stone will out but not so quickly as you expect. Oral diuresis should continued for 20 days then break for 10 days for three months. Then stop medications, save the stone and get its chemical analysis done. During the treatment if you get the pain take drotaverine every 6 hours till pain settles down. If you have pain at the time of taking medicines in the morning then do not take lasilactone> when pain settles down take drotin and diuretic from next morning. If the stone recurs then try to find out underlying cause.

8 Causes Of Chronic Kidney Diseases!

MBBS, MD (Med), DNB (Nephrology)
Nephrologist, Mumbai
8 Causes Of Chronic Kidney Diseases!

Chronic Kidney Disease (also known as Chronic Renal Failure) is the progressive loss of kidney function occurring over a span of several months to several years and is characterised by the replacement of kidney architecture with nonfunctional fibrotic shrunken tissue. Chronic kidney disease is classified into five stages on the basis or proteinuria (presence of protein in large amounts in the urine) or Glomerular Filtration Rate (GFR) 1 being the mildest with no distinct symptoms and 5 being end stage renal failure.

Causes and Risk Factors of Chronic Kidney Disease

The various reasons (called risk factors) that can increase chances of Chronic Kidney Diseases are:

  • Diabetes mellitus, hyperlipidemia (excess fat proteins in the blood)
  • Hypertension
  • Family history heritable renal disease
  • Older age smoking
  • Autoimmune disease
  • Past episodes of acute renal disease
  • Kidney Stones
  • Excessive self-medication especially pain killers

The direct causes of Chronic Kidney Diseases are:

  • Diabetic nephropathy (leading cause)
  • Hypertensive nephropathy Glomerulonephritis (swelling of the glomerulus in the kidneys)
  • Reno-vascular disease (ischemic nephropathy)
  • Polycystic kidney disease
  • HIV-associated nephropathy
  • Transplant allograft failure
  • Exposure to drugs and toxins

Diet that you must maintain in Chronic Kidney Disease:

It is essential to make dietary as well as lifestyle amends once you are diagnosed with CKD. The main purpose of this diet is to maintain the levels of carbohydrates, fluids and minerals. This is done to prevent the buildup of waste products in the body as the kidneys are unable to fulfil its function properly. It is recommended to refer a dietician who can make you a diet chart that helps you adhere to your diet.

Here is a list of food items that you may avoid or may consume if you are suffering from Chronic Kidney Disease:

  1. Carbohydrates: Carbohydrates are a good source of energy. If the intake of protein has been restricted, then it is recommended to replace that with carbohydrates as a source of energy. Fruits, vegetables, grains and bread can be consumed as they are rich source of fibres, minerals and various types of vitamins. You can indulge in some hard candies or sweets as well.
  2. Fats: Fats can provide a good amount of calories as well. Ensure that you only stick to healthy fats such as the monounsaturated and polyunsaturated fats that are good for your cardiac health.
  3. Reduce your phosphorus intake: A diet with no more than 800 mg of phosphorus can help reduce the risk of too much phosphorus building up in your blood. Limit intake of foods with high levels of phosphate or phosphate additives such as organ meats, whole grain breads, processed foods, cola beverages, cheese, dried beans, liver, peanut butter, dairy products and chocolate. Many beverages and processed foods have these additives. Other high-phosphorus foods to limit include:
    • Ice cream
    • Legumes
    • Milk
    • Nuts
    • Yogurt
  4. Monitor your potassium levels: Usually, potassium is not restricted in stage 3 CKD unless lab tests show potassium is too high. Your doctor may make medication changes or prescribe a low-potassium diet. Reduce an elevated potassium level by limiting some high-potassium foods and potassium chloride (found in salt substitute and many low-sodium processed foods). Some high-potassium foods to limit or avoid include:
    • Avocado
    • Bananas
    • Cantaloupe
    • Honeydew melon
    • Dried fruit
    • Legumes
    • Milk
    • Nuts and seeds
    • Oranges and orange juice
    • Pumpkin and winter squash
    • Tomato products (juices, sauces, paste)
  5. Proteins: Usually, before undergoing dialysis, it is recommended to adhere to a low-protein intake diet. However, when you are undergoing dialysis, it is necessary to consume a certain amount of class one high-quality protein like nonvegetarian, paneer, soyabean, mushroom. This detailed information can be obtained from your dietician.
  6. Fluids: In the early stages of CKD, your fluid intake is not monitored. But as the disease progresses, you need to check your intake of fluids as too much fluids may accumulate in your body and create swelling in your legs and cause pressure on your lungs and heart.
  7. Sodium Or salt intake: Keeping a check on your salt intake reduces the risks of high blood pressure. Also, having food low on salt can reduce your thirst and prevent fluid retention in your body. If you wish to discuss about any specific problem, you can consult a nephrologist.

Kidney Damage - Effective Ways To Reverse It!

Kidney Damage - Effective Ways To Reverse It!

Many of us take the role our kidneys play in keeping us healthy and sturdy for granted. Having your kidneys work well can go a long way keeping you in good shape. You can delay the progression of kidney disease and potentially prevent permanent damage to your kidneys. Here are some things that should be kept in mind in order to protect your kidneys:

  1. Keep your blood sugar and blood pressure in target range: High blood sugar and high blood pressure levels harm the nephrons in your kidneys and prevent them from functioning well. So, get tested periodically to monitor your levels. Improving them and keeping them under the target range will help to prevent further complications.
  2. Get moving: Being active will get your blood pumping and thereby, boost blood flow to your kidneys. It helps your heart to be fit too. So for your body, exercise is a win-win. It can help to slow the rate of chronic kidney disease. Keep a goal to get at least 30 minutes of physical activity each day.
  3. Eat less protein: It is good to avoid protein rich foods as they can stress the kidneys and lead to unnecessary problems. It is important to consume the right amount of protein in your diet and stay away from meat rich in proteins.
  4. Don’t drink soda: Studies have found a positive link between excessive soda drinking and kidney damage. Two or more diet sodas a day can lead to damage and make it progress quicker.
  5. Balance your blood pH levels: When the kidneys don’t function right, they fail to keep the acid balance in your body. A low acid diet with natural veggies will help the kidneys to last longer and restronger. Also, sodium bicarbonate can help protect your kidneys by neutralizing the acid content of your blood. You can mix a teaspoon of baking soda with a glass of water and consume it regularly. Consult with your doctor about it.
  6. Skip canned foods and processed foods: Stick with natural foods and avoid the artificial foods of fast-food joints, ready-made foods and more as they can contain high levels of sodium and other chemicals that deter the kidney function.
  7. Avoid painkillers: Nonsteroidal anti-inflammatory drugs can cause kidney damage. Since kidneys need a strong blood flow to function well, certain painkillers reduce blood flow and make things worse if you have kidney disease.
  8. Antioxidants to flush out toxins: Consider taking antioxidant foods that can help kidney function and protect your cells.

Follow the above tips and prevent your kidneys from damage to lead a happier and healthier life. If you wish to discuss about any specific problem, you can consult a Nephrologist.

Hello doctor.I m worried about a small lump I developed at the opening of my anus which was result of me passing a dry and hard stool with pain.Pls advice how it can b treated. The pain has got lighter in the last one day.I applied an ointment. Thanks

Dear , This is very likely to be perianal haematoma which occurs sometimes after passage of hard faeces. If pain has decreased then it would probably subside. It is better if you take Sitz bath once or twice a day: sitting in a tub full of lukewarm water with a few drops of dettol in it. This will help to subside it quickly. However if pain increase and takes throbbing character then a course of antibiotics should be taken. If pain or throbbing becomes severe enough to disturb your sleep at night then contact a proctologist or a general surgeon. Dr. Kiran Mehta.

Colon Cancer - Fight It With Ayurveda!

According to the American Cancer Society and Colon Cancer Alliance, colon cancer, which is also referred to as colorectal cancer is regarded as the third most commonly diagnosed cancer type as well as the second most important reason of cancer death in the U.S. affecting both men and women. But thankfully, colon cancer can be prevented by changing the food habits.

It is also important to get regular screenings through colonoscopy, which will help you to determine the early or precancerous stage that can be treated with the removal of cancerous cells. Colorectal cancer is deemed to be cancer-related to the food habits and it can be prevented by correcting the same. Here are the top ways through which you can help your condition naturally:

  1. Reduce the amount of red meat consumption: Health studies reveal that people who eat red meat on a regular basis are more prone to developing colon cancer. This is because the chemicals required to digest this food damage the DNA material which in turn is the main reason behind the cause of cancer.
  2. Increase the amount of garlic intake: By consuming more garlic, you can reduce the risk of developing different types of cancer, particularly those of the gastrointestinal tract. Garlic is rich in anti-bacterial properties that can block the pathways of cancer causing elements and boost the repair as well as reduce the proliferation of cells. It is also high in selenium and sodium content which are beneficial for the overall health.
  3. Consume all types of plant antioxidants: The bright coloured fruits and vegetables, as well as herbs and spices, can greatly contribute to adding a wide array of antioxidants in your regular diet. Foods that are rich in anti-oxidants include carrots, pomegranates, cranberries, kale, broccoli, tomatoes, apples, cabbage, turmeric, sage, rosemary, saffron, purple and red grapes.
  4. Cook foods in olive oil: Olive oil is rich in anti-cancer properties and it helps in reducing bile acid and increasing the enzymes that can regulate the growth of the cells on the intestine lining so that tissues can be renewed. Antioxidant compounds called phenolics are present in olive oil and have also proven to be a cancer protective anti-oxidant.
  5. Increase the consumption of Omega-3 fatty acids: It has been reported by several types of research that a diet rich in Omega-3 fatty acids can reduce the occurrence of colon cancer and therefore you must eat a lot of salmon, cod, mackerel and sardines for decreasing the pro-inflammatory levels in your body.

These few alterations to your food habits can greatly help you in preventing colon cancer and you can consult your doctor for a detailed analysis and recommendation based on your individual health condition.

In case you have a concern or query you can always consult an expert & get answers to your questions!

Did You Know - Constipation Can Also be a Sign of Urinary Incontinence

Did You Know - Constipation Can Also be a Sign of Urinary Incontinence

Urinary Incontinence is the sudden urine loss that occurs involuntarily in women. Some of the factors which cause urinary incontinence are pregnancy, menopause (know more why Women Are More Vulnerable to Heart Diseases Post Menopause) and childbirth. It should be noted that urinary incontinence by itself is not a disease, but is a symptom of other underlying disorders such as diabetes, infections and other conditions.

Causes of temporary cases of urinary incontinence include:
1. Constipation (learn more that Work for Constipation Relief)
2. Urinary tract infections
3. Excess consumption of alcohol and caffeine
4. Consuming carbonated drinks
5. Use of artificial sweeteners
6. High doses of vitamin B and vitamin C
7. Being on sedatives, muscle relaxants, blood pressure and heart medications
8. Eatables which are too spicy, acidic or sugary

However, persistence of incontinence might be due to a more serious condition. Some of the causes are stated below:

  1. Age: The bladder muscles tend to weaken with age. This affects the bladder’s urine holding capacity.
  2. Pregnancy: Hormonal changes and increase in weight during pregnancy can cause incontinence.
  3. Menopause: Oestrogen is responsible for the healthy maintenance of the lining of the urethra and bladder. The onset of menopause causes the oestrogen hormone levels to drop. The gradual damage of the bladder tissues causes incontinence.
  4. Childbirth: The bladder control muscles are weakened during normal vaginal delivery, thus leading to incontinence.
  5. Obstruction: Occurrence of tumours in the urinary tract can block the normal flow of urine which can cause incontinence.
  6. Hysterectomy: The same ligaments and muscles support both the bladder as well as the urethra. With removal of the uterus by hysterectomy, the pelvic floor muscles are deteriorated. This leads to urinary incontinence.
  7. Neurological disorders: Conditions such as multiple sclerosis (autoimmune disorder in which the immune system destroys the protective sheath of the nerves), Parkinson’s disease (disorder affecting motor functioning of the body), brain tumours, spinal injury or strokes are capable of causing incontinence.

Different forms of urinary incontinence are classified on the basis of their symptoms:

  1. Urge incontinence: Urge or urgency incontinence occurs when there is a leak before the person reaches the toilet.
  2. Stress incontinence: Small amounts of urine leak out during normal physical movements such as sneezing, coughing and exercising. This happens because of the minimum stress exerted on the body, and hence, the name.
  3. Functional incontinence: Sudden leakage of urine occurs due to external deterrents or physical disabilities such as not being able to find a toilet.
  4. Overactive Bladder: This is characterized by frequent and urgent urination.
  5. Overflow incontinence: A full bladder, at times, leads to unexpected leakage.
  6. Transient incontinence: If urine leakage occurs due to temporary situations such as infections, or new medications.

Too much cholesterol in the blood can lead to cardiovascular disease. What you eat has a direct impact on your cholesterol level. In fact, a diet rich in fiber and other cholesterol-lowering foods may help lower cholesterol as much as statin medication for some people. Working with a registered dietitian and doctor is the key to develop and maintain a successful health program to manage it. I will inform you on how to make diet, lifestyle changes, make you understand instructions for taking medication because it won't work if you don't take it as directed.
