I am a type 2 diabetic. For the last 10 yrs. Was on glycomet gp1 forte + valibo 0.3 till now. Another specialist has advised to use trajenta duo 2.5/500 + diamicron xr 60 mg. Is the to new medicine safer to use?

General Physician, Nellore
It is safe. The new medicine does not cause low sugar levels but it potentiates the effect of the other medicine. Hence, you should be careful to avoid low sugar levels.

My daughter 16 Yrs age has Diabetic type-1 she is taking insulin 4 times a day but still her sugar level is very high. She is taking Human Actrapid 11 units before breakfast, 8 units before lunch, 6 units before dinner and at 10 o clock she take Lantus 16 units. Can we change the insulin from Human Actrapid to Human Mixtard 50/50 to 3 times before meal. Please advise.

lybrate-user, Treatment of T1 diabetes should always be done with close supervision from your local diabetologist, as increase in Insulin dosage or alteration of dosage can lead to hypoglycemic situation. Hence, it is essential to consult the local expert as he or she will be easily available in the event of certain problems. Insulin Actrapid helps to control post prandial glucose rise and since it is a short acting compound control is better. Using a Mixtard 50/50, may be at times disadvantageous. Best is to consult your physician or diabetologist. Thanks.

I am having type 2 diabetes since last 14 years. My latest FBS is 124 and PP2 reading is 165. HBA1C is 7.4. The medication taken by me is Gemer 2 before breakfast and dinner, Tenglyn 20 before breakfast and Pioglit 15 before lunch. I also do 45 minutes of brisk walking and exercise daily. I also have a diet as recommended by a dietician. Please advise as to whether any change in the medication is required to get the target HBA1C of less than 7.0.

Mr. Lybrate-user, looking at the details provided, it is possible to reduce your hba1c% <7, by just increasing your daily exercise by a few minutes and also by adujusting daily intake marginally. There is no need to add any additional drug. You are really maintaining blood sugar levels quite well. Keep it up. With regards.

Is thyroid test done in an empty stomach or whether it can be done when you take your meals of afternoon. Because my doctor done this test with other tests when I already took my lunch .and the result was 4.61 % lil bit beyond normal. Whether I need to do this test again?

Most guidelines will suggest that fasting is not necessary when performing a thyroid blood test. This can be a confusing issue since studies have regularly shown that early morning fasting translates to higher TSH levels—as much as 26 percent higher—compared to those taken later in the day with no fasting. A variation of T4 is seen more as compared to T3 and TSH. What to do- 1. Always get tested at the same time of day, in the same manner (fasting/non-fasting), and at the same lab to ensure consistency and a more accurate reflection of your condition. This is especially true if you are managing well on your current medications. 2. If possible, take your hormone replacement medications at least six hours before a test or wait until after you've had the test to take them. 3. If you are testing later in the day with normal values but aren't experiencing the benefits of replacement therapy, speak with your doctor about testing at different times of the day to see if and how values fluctuate for more queries, contact us. We will be waiting for your feedback.

Is vitiligo (skin problem, white spots) linked to diabetes. If no then what is the cause behind it. How it can be cured.

Is vitiligo (skin problem, white spots) linked to diabetes. If no then what is the cause behind it. How it can be cured.
Hii lybrate-user, vitiligo is a genetic disorder trigger by indigestion and stressful conditions. There was always confusion in patients mind regarding choosing right treatment approach for vitiligo. Allopathy can give quick results but is not very safe for long term uses if steroids are used. Homeopathic is safe but very slow and in most of cases it can not stop the spread of disease i. E old patches shows some recovery but new white patches continue to come even when we are using homeopathy. Ayurvedic herbal medicines are used to purify blood and stop the spread of disease and external application are capable of quickly start regimentation. For detail replies regarding 1. Causes of vitiligo 2. Is it genetic or can it spread to next generation 3. Causes of spread of disease after it remains stable for some years 4. Should we use homeopathy, allopathy or ayurveda. Extra use of haldi and vit. C should be avoided in vitiligo. You can send pictures of vitiligo effected parts. We will review your case and will guide you best possible treatment and time frame required.

Mr. lybrate-user, Thanks for the query. At 40 years, HbA1c% should be closer to 6.8%, to say one has good control. From the glucose levels mentioned, it is clear that present treatment is not helping in controlling blood glucose level. Ideally FBG should be < 100 mg, PP 150 to 160 mg & HbA1c% <6.8. Therefore, we need to have a re-look at the present treatment, dietary intake and extent of daily exercise. We may have to change the present drugs or add one or more compounds to the current treatment. It is better if you come in a private conversation (paid consultation) with all the details, then I will be able to guide and even prescribe the medication required. Thanks.
