My baby's fetal hemoglobin is 7.7 is it normal range of a 8 months old and any risk of thalassemia disease.
Please clarify. Is the hemoglobin 7.7 which may be normal. But if the foetal hemoglobin is 7.7% of total hemoglobin this is too high and further testing for thalassemia is essential.

I am 39 years old Indian, vegetarian man. My son (Born Aug. 2010), for last 2 years, has stopped eating proper meals, only drinks bottled milk (100 ml) 4-5 times a day, or want to eat snacks like chips, biscuits, namkeen, but that too very little quantity. Due to this he remains constipated (pass bowel in 3-4 days) and irritated. This is a vicious kind of cycle where he remains constipated as he not takes proper food in good quantity and on the other hand - don't want to eat because of constant bowel pressure. He doesn't take any Allopathic medicine or tonic. His height is approx 3 ft. and weight 15 Kg. He is otherwise very active child.

Looks like your five year old has never been weaned properly from milk to regular home food T the right time. He is now a big boy and you must set some basic ground rules for feeding. You'll have to stop snacks, set times for 3 major meals and not give more than 500 ml of milk per day. Making meals interesting, offering variations and involving child in food discussion and preparation as a play activity will help to change your faulty feeding patterns.

Hello everybody. I'm Dr Pulkit Nandwani. I'm a consultant gynaecologist and laparoscopic surgeon here at Saroj Medical Institute Rohini. Today, I would like to share some information with you about fibroids.

Fibroids are becoming increasingly common these days. They usually occur in the age group of 30 to 40 years of women but can occur at any age group usually at the age group of 30 to 40years.

Now what are fibroids?

Fibroids are usually noncancerous just an overgrowth of the muscle tissue of the uterus. Fibroids are also called myomas or leiomyomas. Now these fibroids can be located anywhere in the uterus. They can be either in the muscle tissue of the uterus or even in the ovaries or from a stem hanging inside the uterus or out of the wall of the uterus by hanging by a stem. The size and location number of fibroids vary a lot and according to them the symptoms depend.

Now what are the symptoms of fibroids?

The symptoms of fibroids depend on where the situation where the fibroid is located, how big it is and how many numbers of fibroids are located. The symptoms can be they usually cause the change in menstruation. The patient usually experiences heavy bleeding with painful cramping and now instead of monthly bleeds. The bleed may come at irregular intervals and at least twice or thrice in a month. There can be pain during bleeding. Other than the painful bleeding, the fibroid can also result in various symptoms. Because of the size and the weight of the fibroid, it might press on the urinary bladder and the patient might experience frequency of urination even pain during urination. Same if it's at the back of the uterus, it might press on the anus and cause painful deification obtained while passing motions. Because of the heavy bleeding patient might land up with low hemoglobin and having anemia and generalized symptoms of anemia like the fatigue, the weakness, loss of strength, loss of stamina, all these are the  symptoms of fibroids. Just a pain in the lower abdomen can also be the manifestation of fibroid.

Fibroid can also cause infertility. Patient might find difficulty in conceiving or there are chances if the patient conceives if the fibroid is located inside the lining of the uterus it might cause increased chances of miscarriages. There might be abortions then but the good thing is that fibroids rarely cause cancer. It might twist on itself, cause severe pain and but it rarely causes cancer and grows over a period of time. It slowly grows over a period of time, sudden growth is a matter of danger sign.

Now, what are the treatments available for fibroid?

Fibroid if it is asymptomatic the patient is comfortable it was just the routine diagnosis, need not require any treatment at all but usually fibroids cause bleeding problems. So the most common treatment that we employ is starting the patient on medical treatment, a number of hormonal pills come which regulate the hormones inside the body of the uterus and limits the blood flow of the patient and the patient remains comfortable. If medical treatment is either not successful or by the location of the fibroid we already know that medical treatment will not work as in case if the fibroid is coming inside the lining of the uterus. In those cases the fibroid has to be removed surgically for the fibroid is usually done. If it is coming inside the lining of the uterus even if it is very small or a big fibroid is obstructing, the uterus lining causing pressure symptoms. In that case surgery has to be done. Now for surgery also there are two to three types of surgery which can be done.

  • If so patient is young, she has to conceive again and family is not completed, we try to remove only the fibroid. It is known as myomectomy.
  • If the patient is elderly age family is completed, it caues a lot of problproblemshere are multiple fibroids, in that case, the modality of treatment is a one-stop treatment in which we prefer to remove the whole uterus in which case it is known as hysterectomy.
  • Regularly total laparoscopic hysterectomy fully by laparoscopy is done routinely at the hospital at Saroj Medical Institute for fibroids.

So this is much for fibroid for today. If you have any query you can contact me at lybrate. I am Dr. Pulkit Nandwani or you can even take a consultation appointment at Saroj Medical Institute Rohini, Delhi.

Thank you.

You feed nan only if you must and with a cup and spoon or with a sipper and not feed him from a bottle. Also send me his tsh report and keep in touch so that I can guide you regularly as your personal doctor. Frequent motion is due to 1) bottle feeding and 2) possibly due to hypothyroidism. As you are giving formula feed give boiled water with s spoon in between feeds and reply to me.

Cyclic Vomiting Syndrome in Children - Causes, Symptoms and Treatment

MD-DNB (Pediatrics), FCPS (Paediatrics), Diploma in Child Health (DCH), MBBS
Pediatrician, Gurgaon
Cyclic Vomiting Syndrome in Children - Causes, Symptoms and Treatment

Cyclic vomiting syndrome (CVS) is characterized by periodic bouts of nausea and vomiting that happens at cyclical intervals. It affects all ages, but is more common in children. The condition is quite stereotypical in that there are paroxysms or bouts of vomiting that is recurrent and follows days of normal health.

Causes:

There is no definite reason identified, but it is said to have a strong hereditary correlation. Studies have shown mitochondrial heteroplasmies (abnormal growth of mitochondria, which is a cellular component) to be one of the factors that can lead to CVS. The genetic correlation, however, is very difficult to establish, specifically because vomiting and nausea are common symptoms that occur with most conditions in children. And CVS is most commonly noted with conditions like infections and emotional excitement. Infection could be either tooth decay or sinusitis or anything else. Lack of sleep, anxiety, holidays, allergies, overeating, certain foods, menstruation – a host of factors have been shown to induce CVS. There is also a strong association with migraine and conditions that lead to excessive production of stress hormones.

Symptoms:

The syndrome (a group of symptoms) usually has 4 phases:

  1. Symptom-free interval phase: The child is completely normal in this phase, which happens in between bouts.

  2. Prodromal phase: Prodrome is an indication that a disease or a condition is about to happen. In CVS, this is usually nausea and abdominal pain that can last from a few minutes to a few hours. Treatment in this phase can curb the disease. However, there could be some children in whom this may not manifest and the child may directly start with vomiting.

  3. Vomiting phase: Repeated bouts of paroxysmal vomiting happen associated with nausea, exertion, fatigue, and drowsiness.

  4. Recovery phase: As the nausea and vomiting begin to subside, which may take a couple of days, the child returns back to normal slowly. However, the lethargy and energy levels will take a couple of days to return to normal.

Treatment:

Treatment again depends on the severity and the phase at which it is being recognized. If a child has repetitive bouts, then the parent and the doctor would have identified a pattern to it.

  1. If the causative agent has been identified, for instance, infection or migraines, then managing that takes care of the CVS also.

  2. If identified during the prodromal phase, again it can be managed with suitable anti-emetic medications.

  3. If identified after full onset, rest and sleep and medications to control nausea and vomiting are required.

Adequate hydration with electrolyte replenishment and sedatives can provide additional support. However, in most cases of childhood CSV, the pattern will be identified and that helps in better management, both the child/parent and the podiatrist.

If your baby is exclusively breastfed, it doesn't matter if he does not pass stool everyday. Some breastfed babies go without passing stools for a week, but it is perfectly normal provided they are exclusively breastfed.

Stress and Trauma Related to Teenage Pregnancy

MBBS, MD - Psychiatry
Sexologist, Mumbai
Stress and Trauma Related to Teenage Pregnancy

Pregnancy is a journey in itself where you tend to go through physical changes, psychological changes and social changes as well. While the body struggles to cope with the hormonal and other major changes, the mind also tries continuously to adapt and cope with emotional as well as mental stress. Teenagers are considered to still be in a maturing stage when they have difficulty dealing with factors such as peer stress, emotional ups and downs, identity issues, personality building challenges and psychological barriers.

So, getting pregnant at this stage can be very traumatizing. Issues such as postpartum depression have become very common among teenagers.

Psychological Barrier in Teenage Pregnancy: As per the Pediatrics Journal, teenage mothers tend to have higher rate of depression compared with adult women who experience pregnancy. This may be due to various reasons like difficulty to face the society, unplanned pregnancy, immaturity to deal with pregnancy and related symptoms, own social involvements, which come between pregnancy and a whole lot of reasons.

However, the psychological effects and stress, which account for these barriers are sometimes quite severe and according to researchers, teen mothers often show symptoms of Post Traumatic Stress Disorder, which may sometimes lead to suicidal tendencies.

Signs and Symptoms of Stress and Trauma:

  1. Some of the major stress signs in teenage pregnancy either in the prepartum or postpartum period may include anxiety, depression, concentration difficulties, eating disorders, insufficient sleep, sadness or mood swings.
  2. Already teenage pregnancy leads to excessive mental stress and if there is a complication in delivery or premature birth before 37 weeks, the level of depression tends to get higher.
  3. There is a distinct difference between postpartum depression and baby blues. While baby blues are similar to stress and sleep problems, eating troubles, mood fluctuations seem the same as postpartum depression, they tend to go off within two to three weeks, while depression lasts.
  4. Excessive fatigue resulting into a brittle mood.
  5. Sudden attacks of panic are also quite common.
  6. Difficulties in bonding with baby or having patience with baby's demands.

This type of stress and trauma may lead to child abuse, neglecting children or depression. Lowered self-confidence and irritated state of mind are also some of the common complaints. Therefore, to avoid the postpartum stress and major difficulties, teenage pregnant moms can consult psychiatrists for help and advice.
