I'm a teenager troubled by many to almost all the symptoms of ADHD. I've been suffering from low concentration levels, difficulty in sticking to or continuing to do a particular thing, including studies and job. It's been affecting my life adversely. And I'd love to get myself diagnosed to check if I do or I do not suffer from ADHD for my peace of mind. I tried doing this at Deenanath Mangeshkar hospital in Pune but they didn't comply. Can anyone here help me out with this? Thank you.

For a person to receive a diagnosis of ADHD, the symptoms of inattention and/or hyperactivity-impulsivity must be chronic or long-lasting, impair the person’s functioning, and cause the person to fall behind normal development for his or her age. The doctor will also ensure that any ADHD symptoms are not due to another medical or psychiatric condition. Most children with ADHD receive a diagnosis during the elementary school years. For an adolescent or adult to receive a diagnosis of ADHD, the symptoms need to have been present prior to age 12.

My girl child is 6 years and 7 months old. Since yesterday afternoon, she is having fever, which is coming down on giving Calpol (Pediatric syrup), but after few hours it goes upto 102 C. We have been giving her oral fluids and she is resting at home. Is she now an adult and can we give paracetamol in tablet form?

Dear parent If she can take tablet you can give her 1/2 (half) tablet. But with 102 Fever get her blood and urine test done.

Yes its ok. But answer my question that how and why do you know that your breast milk is not enough? If your baby is passing urine adequately i.e if you are changing diapers 6-8 times (8-10 times in case of cotton nappies) then your breast milk is sufficient for your baby. Concentrate on taking high protein diet for good milk output.

Cough which increases on lying is usually due to nasal congestion, postnasal drip and reflux. Try anti allergic syrup and if no relief consult a child specialist.

All About Congenital Hypothyroidism!

What is congenital hypothyroidism?
Newborn babies who are unable to make enough thyroid hormone have congenital hypothyroidism, meaning they are born with an underactive or absent thyroid gland.  If the condition is not found and treated, children can have mental retardation and growth failure. But the good news is that early diagnosis and proper treatment can prevent these problems. In most cases, the condition is permanent and your child will need lifelong treatment. 

What causes congenital hypothyroidism?
 The most common causes are

  1. An underdeveloped thyroid gland
  2. A thyroid gland that’s not located where it should be (in the neck below the voice box or larynx)  
  3. A missing thyroid gland

 Other possible causes include

  1. Defective production of thyroid hormone (an inherited condition)
  2. Problems with the pituitary gland (located at the base of the brain), which tells the thyroid to make thyroid hormone
  3. Less commonly, a mother’s thyroid disease or medicines taken during pregnancy can cause congenital hypothyroidism.

What are the signs and symptoms of congenital hypothyroidism?

Many babies with congenital hypothyroidism appear normal at birth or for several months after birth.  But others may have these signs and symptoms:

  • Jaundice (yellowing of the skin and eyes)
  • Constipation
  • Poor muscle tone
  • Sluggishness
  • A hoarse cry
  • They might also have
  • Feeding problems
  • A thick, large tongue
  • A puffy-looking face
  • A swollen abdomen, sometimes with “outpouching” of the belly button
  • Large soft spots of the skull

Thyroid hormone deficiency can also occur in older babies or young children, even if test results at birth were normal.

Treatment
Early diagnosis is very important. Most of the effects of hypothyroidism are easy to reverse. For this reason, doctors always ask to get screening for hypothyroidism of your newborn.

Thyroxine is usually given to treat hypothyroidism. Once the child starts taking this medicine, blood tests are regularly done to make sure thyroid hormone levels are in a normal range.

When to Contact a Medical Professional
Call your health care provider if:

  • You feel your infant shows signs or symptoms of hypothyroidism
  • You are pregnant and have been exposed to antithyroid drugs or procedures

Prevention
If a pregnant woman takes radioactive iodine for thyroid cancer, the thyroid gland may be destroyed in the developing fetus. Infants whose mothers have taken such medicines should be observed carefully after birth for signs of hypothyroidism. Also, pregnant women should not avoid iodine-supplemented salt. If you wish to discuss about any specific problem, you can consult an endocrinologist.

Hello- Low/absent breast milk supply can be due to certain illnesses, consumption of birth-control pills, certain hormonal changes in the body, nutritional deficiencies, improper latching position of the baby and infrequent breastfeeding due to cracked / inverted nipples. Inadequate breast milk can put your baby at a high risk of malnutrition, a weak immune system, poor memory and many other health problems. Certain galactagogue herbs (in ayurveda) and specific tips can offer significant help in this condition. I recommend you to start herbal supplements to improve your condition.

My son whose Age- 15 month old weight- 8.5 kg Height- 76 cm (30") Weight during birth- 3.6 kg is very weak & not interested in taking food. He is always suffering from constipation, indigestion & bowel movement. We give him cerelac, boiled rice, potatoes ,wheat chuda& soya chhatua. Doctor advice not to give cow milk. His weight is very less with compare to age . He doesn't take any iron &folic acid syrup since last 15 month. So kindly advice me the balance diet & health supplement for healthy weight. Also recommend him to take which iron & folic acid syrup to avoid the deficiency of iron.

Yes your baby weight is less thts almst equivalent to 10 to 11 mnths baby with this good birth weight. You need to first rule out cause for his failure to thrive. For which he may need blood invstgtns step wise. Haemogram (anemia, thyroid profile, if he has recurrent loose stool nd bottle fed stop bottle feed he needs currently solid diet not milk 250 ml a day is more thn sufficient give him prantha, dahi, sbji, salads and fruits with extra ghee and makhan.
