Diabetes - A Major Risk Factor For Kidney Disease!

My husband is a diabetic from last 8 yrs. We consulted to Dr. and he gave him only one tablet prizzide M. Twice a day before breakfast and dinner. He also takes homeopathy treatment. But in between he went to some. Other Dr. And he changed his tablets. After that my husband is loosing weight. He bcm very thin. I suggested him to stop the new tablets and continue the old one. But still he is loosing weight. Please suggest him a good diabetic dr.in both allopathy and homeopathy.

Hello, Thanks for your query on Lybrate "As" per your clinical history is concerned what medications he is taking and what is his present blood sugar level and if sugar is under control then nothing to be worried at all so please check his fasting blood sugar now and send report. Do follow lifestyle modifications like this- 1) Eat healthy foods. Choose foods lower in fat and calories and higher in fiber. Focus on fruits, vegetables and whole grains. 2) Get more physical activity. Aim for 30 minutes of moderate physical activity a day. Take a brisk daily walk. If you can't fit in a long workout, break it up into smaller sessions spread throughout the day. 3) Lose excess weights. If you're overweight, losing even 7 percent of your body weight can reduce the risk of diabetes. To keep your weight in a healthy range, focus on permanent changes to your eating and exercise habits. MODY is maturity onset diabetes of the young. It a rare type of diabetes different from type 1 & type 2.Its incidence is only 1-2%of all diabetic cases. This is caused by mutation in single gene. Its diagnosis requies clinical suspicion, genetic testing beside other parameters of diabetes.

Endocrinology deals with hormones, hormone systems and hormonal disorders. The organs that come under endocrinology are pituitary gland, adrenal glands, ovaries or testicles, thyroid gland and pancreas. These organs maintain the chemical balance of the body by secreting hormones which regulate all the body systems.

Diabetes is the name for a group of diseases which are characterized by abnormalities in the absorption of glucose and blood sugar levels.

Relation between the endocrine system and diabetes:

The hormone insulin is secreted by the pancreatic beta cells called the islets of Langerhans. It is a peptide hormone that metabolizes carbohydrates, proteins and fats, stimulates the conversion of glycogen or stored protein to usable glucose and also helps the body cells to absorb the glucose. The movement of ionized or free calcium (Ca2+) is also somewhat dependent on insulin.

Insulin is secreted from the pancreas in two phases - the first phase occurs when blood sugar level is high and need to be absorbed into the cells while the second phase is associated with a slower production not influenced by blood sugar.

Types of diabetes and how they are affected by the endocrine system

  1. Type 1 diabetes mellitus: This condition is caused by underproduction of insulin by the pancreatic beta cells. It is called 'insulin dependent diabetes mellitus' (IDDM), since it is directly influenced by the hormone.
  2. Type 2 diabetes mellitus: This type of the disorder is not directly dependent on insulin. It is usually caused by obesity and lack of physical exercise. The cells develop resistance to insulin and because of that blood sugar is not absorbed properly. This eventually leads to under-secretion of insulin.
  3. Gestational diabetes mellitus: Pregnant women might develop high levels of blood sugar even if they did not have diabetes in the past. This occurs due the changes in the regular hormonal cycles and secretion of pregnancy hormones and can result is complications and miscarriage.

Sub-specialties of diabetic endocrinology

  1. Diabetic renal disease: The kidneys filter the blood. If there are chronically high levels of sugar in blood, hormones like erythropoietin (regulates production of red blood cells) and renin (regulates blood plasma and fluid content) are not produced. As a result, the kidneys' capacity is reduced and waste materials build up in blood.
  2. Diabetic vision problems: Insulin problems may cause the blood vessels supplying the retina (light sensitive eye tissue) to expand or contract. This causes blurry and distorted vision and pain in the eyes.

There are well established risk factors for diabetes and if these can be managed, then the chance of delaying onset, controlling progress and containing complications are highly possible. Read on to know how simple things can be effective in preventing and managing diabetes.

  1. Weight control: Obesity is the most important risk factor for developing diabetes. A person with normal BMI is 20 to 40 times less likely to develop diabetes than an overweight person. Losing about 10% of the excess weight can help prevent diabetes by more than 50%. Check with your doctor what should be the ideal recommended weight for you and draw up a routine (diet, exercise, etc.) that will help you achieve and stay around that weight range.

  2. Exercise: All methods of exercise help in moving the muscles, which is drastically reduced given our sedentary lifestyle. Moving the muscle (as much and as often as possible) ensures they absorb more glucose and reduce the stress on insulin production. Something as simple as brisk walking for 30 minutes is good enough to reduce the chances of developing diabetes by 50%.

  6. Good fats vs bad fats: Polyunsaturated fatty acids are found in vegetable oils, nuts, and seeds and help control diabetes. Trans fats present in margarine and baked foods are best avoided.

  7. Non-vegetarian foods: If you have a choice, pick fish and poultry versus red meat (processed or otherwise). The fish is rich in Omega-3 fatty acids and helps control inflammation.

  8. Leave the whites: Get off white rice, white refined sugar and white flours. Include brown rice, brown or demerara sugar and wheat flour instead.

  9. Smoking: Quit smoking and you will note how beneficial it is in preventing diabetes.

  10. Alcohol: Moderate amount helps in effective functioning of the insulin.

Diabetes is a serious disease, but if properly managed is not life threatening. Diabetes can be explained as the body’s inability to regulate insulin production. This leads to fluctuations in blood sugar levels. This disease affects every part of the body including the skin. For some people, a rash is among the first few symptoms of diabetes. Most diabetic patients suffer from a skin condition at some point or the other. Here are a few common skin ailments that affect diabetic patients.

  1. Bacterial infectionsA weakened immune system makes diabetics more vulnerable to bacterial infections than normal. These bacterial infections can be typically seen in the form of styles on the eyelid, boils, carbuncles and nail infections. They usually make the skin around the infected area red, inflamed and painful. Most skin bacterial skin ailments can be treated with antibiotic creams and pills.
  2. Fungal infectionsOne of the most common fungal infections that diabetic patients suffer from is known as Candida albicans. This ailment is characterised by a red, itchy rash that is surrounded by small blisters. It is caused by yeast-like fungus and usually found in moist areas like the armpits or between the toes. Other fungal infections that affect diabetic patients include ringworm, jock itch, athlete’s foot and vaginal yeast infections.

  6. Disseminated granuloma annular: This condition is characterised by a ring or arc shaped areas on the skin. They can usually be seen on the fingers and ears but may also occur in the chest and abdomen. This rash may be reddish or skin coloured. For people without diabetes, the normal range for the hemoglobin A1c level is between 4% and 5.6%. Hemoglobin A1c levels between 5.7% and 6.4% mean you have a higher change of getting of diabetes. Levels of 6.5% or higher mean you have diabetes. Changing the lifestyle together with constitutional treatment will give you best results
