I am diabetic. I use to take 10iu daily before break fast. My fasting sugar level is some what normal. But pp level is beyond my control even after taking mixtard insulin. How can I control that.

MD - Pulmonary Medicine, MBBS
Pulmonologist, Pune
Hello, taking right dose of insulin is the best way to control fasting and pp sugar. Even if after taking insulin, pp sugar goes high, you can try tab. Vobit 0.2mg one after food twice daily to control pp sugar. Check fasting sugar after seven days to find control.

I am 70 years of age working male residing in mumbai. Right now I have no problems. But because of pollution, we all are getting less oxygen. At 70 our lungs are not working 100 %. Can we use oxygen cylinders at night say for 2 hours everyday to compensate oxygen deficiency? does this use of oxygen cylinder has any side effects in long run? please advice.

MD - Pulmonary Medicine, MBBS
Pulmonologist, Pune
Hello, your concern about oxygen is right but this is not a solution for your problem. Oxygen is a medicine and you should use it very cautiously. Unnecessary oxygen supplementation will not help and may cause side effects like fatigue (tiredness), and morning headaches. It dries up your mucosa and results in infection. Most importantly it can suppress the drive to breathe as body gets readymade oxygen from outside. So it is not advisable to use oxygen until your blood oxygen level is below subnormal. It also poses a fire risk if not handled correctly at home.

I am a copd patiant - age 75- saturation arround 90 - need oxygen constantly- also creatine level is 2.1 for last 6 yrs. Need to be little more mobile- get breathlessness very quickly.

MD - Pulmonary Medicine, MBBS
Pulmonologist, Pune
Hello, copd is a chronic progressive disease. You should take all your inhaler medicines regularly. As you said, in spite of taking oxygen you get breathless, it means you are having very severe copd. You can enroll yourself for pulmonary rehabilitation programme to increase your exercise tolerance. It will help you to manage your daily activities with minimal breathlessness and minimizes the need for oxygen supplementation.

I am 47 yrs old female. 9months ago my sugar level was 465-160 and wrigt was 68kg. Now my sugar level is in normal range and weight 51kg. Now I consume 1pcs metadose 850 ipr and amway daily. Now my problem is body weakness. Energyloss. I have no power to cook or home work.

MD - Pulmonary Medicine, MBBS
Pulmonologist, Pune
Hello, as you said your sugar level and weight both are under control, there is no worry about that. But worry is about side effects of metadose tablets. It can cause sugars to go down or lactic acidosis. So be sure about these side effects and check your sugar regularly. If sugar is low, then dose of metadose tablet must be reduced. Also check your abg to find out acidosis. Second thing is check your daily calorie intake. If you are having less calories, you may feel weak and dizzy. One of the above tests will find out your problem.

My mom is suffering from tb from past 5 years. Even though they complete one year treatment of t. B, its not cured. Now its getting increasing. Any other treatment to cure this completely? please suggest.

MD - Pulmonary Medicine, MBBS
Pulmonologist, Pune
Hello , first of all it is very important to rule out drug resistant tb by doing sputum bactec afb culture and sensitivity test. Once confirmed, we can start her on definitive anti tubercular treatment based on reports. So do the test as early as possible and consult your doctor for further treatment. Thank you.
