I have shortness of breath and extreme fatigue. I suspected thyroid and I went for thyro profile check. I opted complete check up including liver profile, pancreas, renal, toxic elements, lipid, cbp. All tests are normal except lipid: Total cholesterol: 163 Triglycerides: 132 LDC: 95 HDC: 32 Ratio: 5.1 homocysteine: 49 Last three were highlighted. I am overweight .Does high homocysteine indicate that I suffered heart attack before? I quickly started a new diet plan to avoid carbs. Any need to hit panic button and visit a cardiologist right away? Kindly advise.

1) Stop panicking none of these tests indicate that you have had heart attack. They just indicate whether you have any risk of it or not. 2) Cholesterol profile is within normal limits and you just need to have a healthy lifestyle with some weight loss. 3) Regarding high homocysteine is just a risk marker and not a very definitive one, so no need tp panic. 4) Begin with weight reduiction by exercising and diet change. 5) Daily 40 mins of exercise is recommended begin gradually and then increase to the recommended level. 6) Take a detailed diet advice from a dietitian for a low fat diet with emphasis for weight reduction. 7) Quit smoking and drinking. 8) Regarding your breathlessness and fatigue issue - please get your hemoglobin checked along with calcium and vitamin D3 levels as well if thyroid is normal and it could be secondary to lack of physical activity as well, so build your stamina gradually.

Dear lybrateuser, -The solution to your problem is that you stop drinking altogether, stop drinking at the point where your problem starts as this is a adverse reaction to the alcohol intake, it can become serious like dizziness, headache & collapse, so better would be to avoid intake.

Role of Neurosurgery in Management of Stroke

A stroke may be called a brain attack. It may happen to any person at any time. A stroke occurs when blood supply to the brain is cut off. The brain cells become oxygen deprived due to the stroke and eventually die. Dead brain cells result in brain dysfunction due to which patients lose control over muscle and memory.

Strokes can be mild to life threatening and requires immediate medical attention. Stroke requires advanced care and should be treated by extensively qualified and experienced medical experts. 

Neurosurgery satisfies all the required clauses for treatment of stroke and accounts for being one of the best ways for management of stroke. Renowned hospitals have a dedicated neurosurgical wing and a team of neurological specialists who know how to deal with stroke the best.

Vascular Neurology
This form of neurology focuses on patients who have undergone a stroke or have experienced symptoms of stroke. A multidisciplinary approach is taken for the prevention of recurrent cerebrovascular conditions.

Types of stroke

  1. Ischemic stroke: This kind of stroke occurs when there is a blockage in a blood vessel, which supplies blood to the brain. This form of stroke is the more common. The underlying cause of this stroke is known as atherosclerosis, which is a condition where fatty deposits occur in the walls of the blood vessels and cause blockage in them.
  2. Hemorrhagic stroke: This type of stroke occurs when a blood vessel gets weak and ruptures into the brain. This is caused by two types of weak blood vessels known as aneurysm and arteriovenous malformation.
  3. Transient ischemic attack: This type of stroke is also known as a mini stroke. These attacks occur because of blood clots. The blockage caused is temporary or transient in nature.

Treatment of Ischemic stroke

  1. Tissue Plasminogen Activator: This is one of the best ways to treat ischemic strokes. This medicine is given to the patient intravenously. It dissolves the blood clot and improves blood flow to the area of the brain which is affected. The medicine should be given within three to four hours after stroke symptoms appear.
  2. Endovascular procedure: This is a process by which the blood clot is removed using a catheter, which gets inserted into the area of the blocked blood vessel. It helps in restoring blood flow to that area. 

Treatment of Hemorrhagic stroke

  1. Endovascular procedure: Several endovascular techniques and methods are used so that the weakening of blood vessels is reduced and less bleeding is caused.
  2. Surgical treatment: Different modes of neurosurgery may be undertaken to stop bleeding and for relieving pressure within the skull.

I am 61 years and am obese weigh 100kg. And 5.7" Is it safe for me to have this surgery or is there some other way I can loose weight. I have suffered a minor stroke last year and a minor heart attack about 13 years back. I have 7 dislocations in my back! rest, I am fine!

Try loosing weight by diet, otherwise go for surgery, remember every surgery carries some risks. Doctors cannot recommend many exercises, you need to eat healthy with lots of seasonal fruits and vegetables, little fats, less carbs but more proteins like toned milk.

For pain take tablet paracetamol 650 mg and get your vital parameters of the body checked from a nearby doctor and revert back with findings.

Hi Doctor, I want to consult with you my mother's problem. My mom suffering from headache problem in which her head got very severe pain, face got swelled, she vomited, blood pressure became low and sometimes the pain is unbearable and she started crying. Hi lybrate-user thank for choosing lybrate as long as the headache is concern of your mother and ct imaging study, if it is unremarkable and nothing bad happened within this last 10 to 12 years then headache is not due to any pathology but this could be tension headache and it need to be treated in that way only" charity begins at home" so better spend some more time with her, she might be in need to some moral support and it is advisable to see the psychiatrist too, hope this would helps.

More likely to be cervical neuralgia due to pinching of nerves in cervical vertebrae. Get one x ray cervical spine to confirm. Syop using pillowsbelow the head, dont work with forward bending of neck for long. Do back bending exercises of neck. You should be relieved of the pain. You may not need Stress ECG unless your pain starts only after some exertion like walking, running etc.

Doctor my husband is diabetic and I ensure his regular blood tests also. He too has high cholesterol for which he has been prescribed Fenolio 145 and Atorva tablets once in a day .Now he is feeling a lot of weakness and weight loss .And now most of the times he feels indigestion also. Pls suggest.

Please ensure that his blood sugar levels are under control as high blood sugar levels can cause weight loss. Also see to it that he takes a simple diet which is easy to digest. Also he should have his dinner by 7 - 7.30 pm. He can take Triphala powder 1 tsp at bedtime with a glass of Luke warm water.
