Dear doctor. I am working in Gulf. I will mention my case. I am in my early forties and I have a son. My husband died many years ago. My son is 22. We love each other very much. One night while we were both drunk, I allowed him to have sex with me which happened because of my foolishness of not stopping him and it continued 3 times the same night. I was unconcerned about contraception at that time and as a result, I got pregnant. I kept silent and went to a doctor who gave me pills called misoprostol. I had abortion and my son didn't know. After a couple of months my son and me started having sex again and once during an intimate moment, I told my son that I got pregnant once from him and went for abortion. He couldn't bear the sorrow and I too felt guilty and sorrowful. He started seeing baby pictures and started talking about babies to me. Eventually I got attracted and now we both intentionally want a baby and is trying for the same. What are the pre-pregnancy precautions that we have to consider. Also in terms of sex. Another question is, will I be able to satisfy my son as I get older especially after a few years, as menopause will be a reality My 10 year old daughter sleep with me at night in the next bed from mine. My son comes to my bed at night, Will she be able to understand if she see us making love. We always make sure it doesn't happen My son loves me as his mother very much. I feel very bad when he do clitoral licking as I am very much worried about my son's health. I talked with my friend who was in a similar relationship and who had safe deliverySo do I have a similar chance.

DearLybrater I see 2 issues 1. social acceptance both your friends and his friends. 2. age difference can show up later and just prepare for it..You can continue to enjoy sex even after menopause. you can become pregnant..but genitically not advised..

As you mentioned your next date is 28 march, so did you get menses in feb 28? from your previous story we can make out that you had menses in january and you took total 3 pills and still no menses. After taking 3 pills menses can get delayed by a month or so. If you already did urine test then there is no reason to worry. Menses will come. Keep doing urine test every week. All the home remedies your taking to get menses never work.

Benefits of Breastfeeding for Baby & Mother

MBBS, MD - Paediatrics, CRT in Autism

Benefits of Breastfeeding for Baby & Mother

Breastfeeding is best for the baby and its benefits extend well beyond the paradigm of basic nutrition. Breast milk contains all the vital vitamins and nutrients, which a baby needs in the first six months of life. It is fully loaded with disease fighting substances, which protect baby from illness. Many international paediatric organisation recommend exclusive breastfeeding for the first six months and recent research reports have proven that breastfeeding is good for both mother and the baby.

Breastfeeding protects the baby from a long list of illnesses, such as ear infections, lower respiratory illnesses, stomach disorder and meningitis and are less severe when they do happen. Breast milk adapts as the baby grows to meet the changing needs. Breastfeeding builds a strong emotional bond between the mother and the baby and it has long term benefits lasting right into adulthood.

The first milk of the mother contains high concentrations of secretory immunoglobulin A (IgA), which guards against invading germs by forming a protective layer on the mucous membranes in baby's intestines, nose, and throat. Mother's body responds to pathogens (virus and bacteria) that are in her body and makes secretory IgA, which is specific to those pathogens, creating protection for the baby based on to whatever mother is exposed.

The antibodies in breast milk gives a baby's immune system a boost and also helps children avoid type 1 and type 2 diabetes, high cholesterol and inflammatory bowel disease that strike later in life. Babies who were not breastfed run the risk of developing Crohn's disease and ulcerative colitis.

Breastfeeding boosts child's intelligence. Surveys have shown that premature infants with extremely low birth weight who receive breast milk shortly after birth improved their scores of mental development at eighteen months when compared with premature infants who weren't given breast milk. The emotional bonding happening during breastfeeding contributes to some of the brainpower benefits. Breastfeeding lowers the risk of sudden infant death syndrome (SIDS). It reduces a child's risk of becoming obese as a teen or adult as the breast milk contains less insulin, which stimulates creation of fat. The high content of leptin in the milk regulates appetite and fat.

Breastfeeding triggers the release of the hormone oxytocin that promotes relaxation and nurturing. Oxytocin released while nursing also helps your uterus contract after birth, resulting in less postpartum bleeding. It reduces stress level and the risk of postpartum depression. Research studies show that breastfeeding results in changes in breast tissue and lactation reduces the production of oestrogen, which reduces the risk of breast and ovarian cancer. It also reduces the risk of osteoporosis and cardiovascular disease.

The Effects of alcohol on the Body

The Effects of alcohol on the Body

When you drink alcohol, it’s absorbed into your bloodstream and affects every part of your body. In the long term, this can put your health at serious risk.

When you drink alcohol, it’s absorbed into your bloodstream and affects every part of your body. In the long term, this can put your health at serious risk.

Even a small amount of alcohol has an affect on your body. When you drink, alcohol is absorbed into your bloodstream and distributed throughout your body. A tiny amount of alcohol exits your body in your urine and your breath.

You absorb alcohol more slowly if you eat, especially if the food is high in fat. However, if you drink more than your body can process, you’ll get drunk. How quickly alcohol is metabolized depends on your size and gender, among other things.

Alcohol consumption causes physical and emotional changes that can do great harm to your body. The long-term effects of alcohol abuse are many, putting your health in serious jeopardy and endangering your life.

Excretory System

The excretory system is responsible for processing and eliminating waste products like alcohol from your body. As part of that process, the pancreas secretes digestive enzymes that combine with bile from the gallbladder to help digest food. The pancreas also helps regulate insulin and glucose.

Excessive alcohol use can cause the pancreas to produce toxic substances that interfere with proper functioning. The resulting inflammation is called pancreatitis, a serious problem that can destroy the pancreas. One of the most frequent causes of chronic pancreatitis is alcohol abuse.

The liver’s job is to break down harmful substances, including alcohol. Excessive drinking can cause alcoholic hepatitis which can lead to the development of jaundice (yellowing of the skin and eyes). Chronic liver inflammation can lead to severe scarring known as cirrhosis. This formation of scar tissue can destroy the liver. When the liver fails to perform, toxic substances remain in your body. Liver disease is life threatening. Women are at higher risk for alcoholic liver disease than men, because women’s bodies tend to absorb more alcohol and take longer to process it.

When the pancreas and liver don’t function properly, the risk of hypoglycemia (low blood sugar) rises. A damaged pancreas can cause the body to be unable to utilize sugar due to a lack of insulin, which can lead to hyperglycemia. Unbalanced blood sugar levels can be a dangerous problem, especially for people with diabetes. Alcohol abuse also raises your risk of liver cancer.

Central Nervous System

One of the first signs of alcohol in your system is a change in behavior. Alcohol travels through the body easily. It can quickly reach many parts of your body, including your brain and other parts of your central nervous system. That can make it harder to talk, causing slurred speech, the telltale sign that someone who has had too much to drink. It can also affect coordination, interfering with balance and the ability to walk.

Drink too much, and your ability to think clearly is in trouble, as are your impulse control and ability to form memories. Over the long term, drinking can actually shrink the frontal lobes of your brain. Acute alcoholic withdrawal can lead to seizures and delirium. And severe alcoholism can progress to permanent brain damage, causing dementia.

Damage to your nervous system can result in pain, numbness, or abnormal sensations in your feet and hands. Alcoholism can cause a thiamine (vitamin B1) deficiency, which can result in involuntary rapid eye movements, weakness, or paralysis of the eye muscles.

Men and women metabolize alcohol differently. It generally takes less alcohol to affect women.

Over time, a heavy drinker can become physically and emotionally dependent on alcohol. It may be very difficult to gain control. Unlike most other common addictions, acute alcohol withdrawal can be life threatening. Cases of severe, chronic alcohol addiction often require medical detoxification.

When an alcoholic stops drinking abruptly, they’re likely to experience symptoms of withdrawal, such as:

nausea
anxiety
nervousness
tremors

In severe cases, it may lead to confusion, hallucinations (delirium tremens), and seizures. Detoxification can take between two and seven days. Medications can help prevent side effects of withdrawal.

Digestive System

Alcohol can wreak havoc on your digestive system, from your mouth all the way to your colon. Even a single incidence of heavy drinking can injure parts of your digestive tract.

Alcohol abuse can damage the salivary glands and irritate the mouth and tongue, leading to gum disease, tooth decay, and even tooth loss. Heavy drinking can cause ulcers in the esophagus, acid reflux, and heartburn. Stomach ulcers and inflammation of the stomach lining (gastritis) can occur.

Inflammation of the pancreas interferes with its ability to aid digestion and regulate metabolism. Damage to the digestive system can cause gassiness, abdominal fullness, and diarrhea. It can also lead to dangerous internal bleeding, which may be due to ulcers, hemorrhoids, or esophageal varices caused by cirrhosis.

Alcohol makes it harder for your digestive tract to absorb nutrients and B vitamins or control bacteria. Alcoholics often suffer from malnutrition. Heavy drinkers face higher risk of mouth, throat, and esophagus cancers. Moderate drinking in the presence of tobacco use can raise the risk of these upper-gastrointestinal cancers. Colon cancer is also a risk. Symptoms of alcohol withdrawal may include nausea and vomiting.

Circulatory System

In some cases, a single episode of heavy drinking can cause trouble for your heart. It’s even more likely your heart will suffer if you’re a chronic drinker. Women who drink are at even higher risk of heart damage than men.

Circulatory system complications include:

poisoning of the heart muscle cells (cardiomyopathy)
irregular heartbeat (arrhythmia)
high blood pressure
stroke
heart attack
heart failure

People with diabetes have an increased risk of low blood sugar levels, especially if they use insulin. Deficiencies in vitamin B6, vitamin B12, thiamine, and folic acid can cause lowered blood counts. A common symptom of anemia is fatigue.

Sexual and Reproductive Health

Erectile dysfunction is a common side effect of alcohol abuse in men. It can also inhibit hormone production, affect testicular function, and cause infertility.

Excessive drinking can cause a woman to stop menstruating and become infertile. It also can increase her risk of miscarriage, premature delivery, and stillbirth. Alcohol has a huge effect on fetal development. A range of problems, called fetal alcohol spectrum disorders (FASD), can occur. FASD symptoms, which include physical abnormalities, learning difficulties, and emotional problems, can last a lifetime.

For women, the risk of breast cancer rises with alcohol use.

Skeletal and Muscle Systems

Long-term alcohol use makes it harder for your body to produce new bone. Drinking puts you at increased risk of osteoporosis (thinning bones) and bone fractures. Muscles become prone to weakness, cramps, and even atrophy.

Immune System

An immune system weakened by alcohol abuse has a hard time fighting off viruses, germs, and all types of illness. Heavy drinkers are more likely to get pneumonia or tuberculosis than the general population. Chronic alcohol use increases your risk of many forms of cancer.

Is 1.8 amh level low. Please guide. Trying to conceive since almost 6 months and 32 yrs.
Hello lybrate-user, * AMH 1.8 is considered relatively low. Though women with low AMH, lower than 1.8 do conceive. * As you r 32, get other Hormonal profile done like FSH, LH and TSH. Also husband's semen analysis. * If all reports are within normal range then you can try planned relation every alternate day between 9th to 20th day of period counting from 1st day of period. This is the most fertile period of a menstrual cycle. * if you do not succeed in 3 to 4 months, please do consult infertility specialist to avoid delays. As the age advances above 30 years AMH goes on decreasing. All the best.

Are Those Jerks At Work Dragging You Down?

Masters in Clinical Psychology
Psychologist, Navi Mumbai
Are Those Jerks At Work Dragging You Down?

We spend a majority of our time in office. Naturally, the atmosphere of the office affects the way we perceive things around us - our moods, passions and personality. How do you like your office atmosphere? Do you enjoy working with your colleagues? Do you feel respected? It is essential for you to have an affirmative answer to each of these questions, for your own good. But if it is not so, what can you do?

  • Shift: Staying in a negative space will pull you down and harm you. If possible, ask to be transferred to another department. Few of us have the luxury of quitting on a whim. So, update your resume and spread the word that you're looking for a change. Don't quit your current job without having a back-up plan in place. However, when looking for a new job, keep the things you don't like about your current job in mind, so that you don't find yourself in a similar situation again.
  • Find like-minded people: No matter how many colleagues you don't get along with, there must be a few you like. Watch their back and ask them to watch yours. Whenever you hear of anything that might affect the people around you, share the news with them but with discretion. Limit your interaction with people who pull you down and drain your energy.
  • Document everything: Don't let things come to a my word against your's situation. This can be very stressful and harm your health. As far as possible, ensure all your communication is recorded in the form of emails, messages etc. Do not rely on someone else to take notes at meetings, rather make your own. Keeping a work diary is a good idea.
  • Lead by example: Negative people feed off negativity around them. If you are a positive person, they will gradually begin avoiding you themselves or you might even make a positive impact on them. Respect people you work with and you will get their respect in return. Before condemning people, take a look at yourself and ensure that you aren't doing anything to attract that kind of behavior.
  • Do something about it: Complaining can get things off your chest but often do nothing to change the situation. Be a part of the solution to the negativity in your office. Be regular with your own work and stick to your deadlines. If you need something, follow up with people instead of expecting them to follow up with you.

Lastly, don't try too hard to change the system. You can only control your own life. If you see your efforts going to waste, get back to that resume and start sending it out. If you wish to discuss about any specific problem, you can consult a Psychologist.

I am 23 years old female I am having lump in right side of my breast and it has increase in size! I don't have any pain in it the lump can move every where in breast. It is movable. Is it cancerous or non cancerous?

MD - Obstetrtics & Gynaecology
Gynaecologist, Mumbai
It looks like it is fibroadenoma, it should be removed only if painfull and increasing rapidly in size after initial investigation like mammography and sonography of the breast and/or FNAC. Fibroadenomas are not cancer and don’t increase the risk of developing breast cancer. They occur because of an increased sensitivity to the female hormone estrogen. It is very common can occur in numbers. Most are about 1 to 3cm in size but occasionally, it can grow to more than 5cm. Most fibroadenomas stay the same size. Some get smaller and some eventually disappear over time.

Condoms have more failure rate. Also you will have to stay dependent on your partner's decision. If you want a method which you yourself can handle, and with much more efficacy then try oral pills, the specific one that suit you.
