Hello! I must advice go for small n frequent healthy meals with some activity. You can take good quality protein in your diet. Like- milk n products, egg, chicken, dal & products. N visit a dietitian. It will help. The 1st thing you need to take care of is good quality protein, complex carbohydrate n good quality fat. N what you are taking before n after work out is also matter. Aim for around 40-50g carbs and 20-30g protein. This stabilises your hormonal system to enable tissue regeneration and keeps blood sugar stable.

I have fever over four days how to solve it? and I have head ache many times please prescribe me some medicine.

AUTLS, CCEDM, MD - Internal Medicine, MBBS
General Physician, Faridabad
I have fever over four days how to solve it? and I have head ache many times please prescribe me some medicine.
Fever is not considered medically significant until body temperature is above 100.4 F (38 C). Fever serves as body's natural defences against bacteria and viruses which cannot live at higher temperatures. Also, the body's defence mechanisms seem to work more efficiently at a higher temperature -Fevers of 104 F (40 C) or higher may be dangerous and demand immediate treatments like: -Acetaminophen (dolo-650 mg) twice or thrice can be used to lower a fever . Aspirin should not be used for fever in children/ adolescents or during a viral illness. -Wear loose fitting clothes and don’t overdress. Overdressing can cause temperature to rise further. -Tepid water (85 F [30 C]) baths may help bring down a fever -The body should be cooled with a wet sponge. -A fever can cause severe dehydration. Drink plenty of fluids.. Have citrus fruits, particularly Vitamin C rich ones like mausami, Amla juice etc. -Take light and easily digestible home cooked meals like sprouts.avoid outside food -Wash hands frequently with soap and water especially after returning from public places or public transport. -Further therapy depends on the cause of fever like urinary infection/typhoid or malaria/dengue infection and the accompanying symptoms that need to be investigated and treated accordingly

I suffer from mouth ulcers from years. They appear at least once in a month, that in Big circles. Do not know what is...
If the mouth ulcer is causing a lot of pain, gently rub a small ice cube over the spot or rinse your mouth with ice-cold water. You could also chew on a clove bud (lavang) and direct the juice generated to the area where the ulcer is located. Clove buds are a great ingredient to cure sore throat too. To prevent the ulcerated area from getting infected, make it a point to rinse your mouth with a little salt water. Remember, this won’t make the ulcer disappear, but it can help reduce the pain. Honey is a natural humectant. It helps reduce scarring and hastens the process of new tissue growth. This, along with its anti-microbial effect ensures faster healing of the mouth ulcer. Chew a few leaves of tulsi along with some water about three to four times every day. This will help in getting rid of ulcers faster and also have a preventive action against their recurrence. Applying a little coconut oil directly onto the mouth ulcer or chewing some coconut (fresh or dry) helps reduce pain and inflammation and hence, ulcers heal faster. Mix some sugar to crushed khus-khus seeds and consume this mixture for instant relief from ulcers. You could apply a little ghee directly to the ulcerated area, or drink a glass of buttermilk two or three times every day. Mouth ulcers left untreated generally go away in a week or so; with these home remedies, you can expect faster relief. However, if you notice an ulcer in the mouth that is persistently present although it does not cause you any pain, do not neglect it or try to treat it with home remedies beyond a few days. Make it a point to see a doctor soon because often, a painless mouth ulcer is one of the earliest signs of oral cancer.

You have an obsessive habit which increases when you are anxious/ nervous/ sitting idle. So, try to divert your mind. For dry lips, keep hydrating yourself by drinking 12-15 glasses of water in a day, take healthy, nutritious food, apply some good herbal moisturising lip balm. You can also have multivitamin - Supradyn/ 1 tab. daily for 3 months (don't take after that).

How should we calculate insulin for type 2 Diabetes mellitus condition ?plz give me with example.

MBBS, Diploma in Diabetology
Endocrinologist, Mumbai
How should we calculate insulin for type 2 Diabetes mellitus condition ?plz give me with example.
Normally starting dose of insulin for type 2 diabetes is roughly 0.4-0.6 units/kg weight. But in type 2 diabetes insulin resistance is variable. In long standing diabetes pt whose beta cells are exhausted and who is obese insulin requirement may go beyond 2-3 units/kg. HbA1c is approx average of blood sugar control in last 3 months. More HbA1c means more uncontrolled diabetes and may require more insulin. Normally diabetologist starts insulin considering age of patient, duration of dm, HbA1c, obesity, current medication details, co morbidities, meal pattern and few other factors. Its individualised treatment and may not work same in other individual. Even insulin has different analogues and have different duration of action. For e.g. Lantus (glargine) has 24 hr action (basal insulin) and humalog (lispro) has 4 hr action. Human actrapid or huminsulin R (regular insulin) has 6-8 hr action. Etc so dosage and timing of insulin is also variable, it depends on which insulin you are taking or which insulin is suitable for you. I hope this answer help you to understand your doubt.

Symptoms And Treatment of Post Traumatic Stress Disorder!

MBBS, MD Psychiatry
Psychiatrist, Kottayam
Symptoms And Treatment of Post Traumatic Stress Disorder!

A particularly stressful situation or event that has created terror or been overly dramatic or dangerous can lead to stress and anxiety long after the individual has physically come out of the situation. This kind of a traumatic condition is usually what characterises Post Traumatic Stress Disorder (PTSD).

In simple words, PTSD is a neurological disorder which affects individuals after experiencing severe traumatic situations.

Symptoms of PTSD:

  1. Mood symptoms: Mood swings are the most common outcome of these symptoms. The patient may experience a complete lack of enthusiasm and drive to indulge in activities like hobbies and socialising. Also, the patient may end up harbouring feelings of negativity towards family, friends and strangers, besides going through phases of guilt and self blame for the occurrence of the traumatic event.
  2. Re-experiencing symptoms: These symptoms include flashbacks and life like re-imagination in the form of dreams and nightmares. In many cases, the patient may remember and re-experience the entire scene or series of events that have led to the trauma in the first place. This can be triggered by a memory or association with words, events and other such things that have to do with the memory of the traumatic episode.
  3. Arousal symptoms: Arousal or reactivity symptoms may trigger the same reactions that the person showed during the traumatic experience. These may include being on edge constantly in anticipation of a reoccurrence, or even getting startled by the slightest thing since the mind is already occupied with thoughts of the event, constantly. Anger and emotional outbursts may also be caused due to these symptoms.
  4. Avoidance or rumination: Those suffering from PTSD avoid being reminded of the trauma, such as people, situations or circumstances associated with the event. They try to suppress memories associated with the event.- Many others ruminate excessively and prevent themselves from coming to terms with it.

Treatment for PTSD:

Mindfulness meditation (not a treatment option) helps in recognizing cognitive dissonances and affected thought patterns and aids in recognizing and overcoming their influence. Other therapies, including Cognitive behavioral therapy (TF-CBT) and eye movement desensitization and reprocessing (EMDR) are also done depending on the patient's condition.

How do medications help regulate these responses?

The medications prescribed for treating PTSD symptoms act upon neurotransmitters related to the fear and anxiety circuitry of the brain including serotonin, norepinephrine, gamma-aminobutyric acid (GABA), excitatory amino acids. There is great interest in developing agents with novel and more specific mechanisms of action than are currently available to target the PTSD symptoms described earlier while also minimizing potential side effects.

Studies show that a number of medications are helpful in minimizing PTSD symptoms. Most of the time, medications do not entirely eliminate symptoms but provide symptom reduction and could be more effective when used in conjunction with an ongoing program of trauma specific psychotherapy for patients.

Medications

Several types of medications can help improve symptoms of PTSD:

  1. Antidepressants. These medications can help symptoms of depression and anxiety. They can also help improve sleep problems and concentration. The selective serotonin reuptake inhibitor (SSRI) medications sertraline (Zoloft) and paroxetine (Paxil) are approved by the Food and Drug Administration (FDA) for PTSD treatment.
  2. Anti-anxiety medications. These drugs can relieve severe anxiety and related problems. Some anti-anxiety medications have the potential for abuse, so they are generally used only for a short time.
  3. Prazosin. If symptoms include insomnia with recurrent nightmares, a drug called prazosin (Minipress) may help. Although not specifically FDA approved for PTSD treatment, prazosin may reduce or suppress nightmares in many people with PTSD.

You and your doctor can work together to figure out the best medication, with the fewest side effects, for your symptoms and situation. You may see an improvement in your mood and other symptoms within a few weeks. If you wish to discuss about any specific problem, you can consult a psychiatrist.

I am 21 years old I work a lot and nowadays I am suffering from severe headache and back pain too. I am not ablevto ...
Few things related to headache so that we can plan the treatment accordingly - if pain is associated with blurred or decreased vision than check your eye sight for refractive error - if the pain is associated with cold and fever than we have to rule out sinusitis as the cause and if the pain is pulsatile in nature and with a throbbing pain usually with an aura than we have to rule out migrane as the cause - if the pain is associated with neck problems than we have to rule out cervical spondylosis as the cause - if the pain is associated with either low or high bp or due to hyperacidity than we have to treat the cause after examination.
