Glaucoma -- The Silent Thief of Eyesight

Glaucoma or "Kala Moti" is often an ignored disease as in many cases it does not cause redness ,pain or watering of eyes but the vision decreases gradually and, most importantly , irreversibly !!

Due to its irreversible nature , early detection is vital . All those above fifty and especially those with a blood relative having Glaucoma , should go for regular eye checkups with an eye doctor. A check up in an eye clinic includes an "eye pressure" measurement. Often this pressure is raised in Glaucoma. Also the eye doctor will take a look at your optic nerve to see if it is showing any signs of glaucoma damage.

All suspicious cases are subjected to two important tests. First is Perimetry where we asses the side vision or field of vision which is first to be affected in early Glaucoma.Second is an OCT which scans the retina of the eye and measures the actual loss of nerve fibre layer.

If detected early Glaucoma can be well controlled with regular use of eye drops. If the reduction in eye pressure is inadequate with eye drops then we have the options of Laser or finally Anti Glaucoma Surgery.

Bone Tumor - Do You Know Limping Can Be A Sign?

MBBS, D.O. Orthopedic Surgery
Orthopedist, Chandigarh
Bone Tumor - Do You Know Limping Can Be A Sign?

Uncontrollable division of cells present in the tissue leads to the formation of a lump or mass of tissue. This mass of tissue is known as a tumor and when this condition is diagnosed in the bones of a human body, it is known as bone tumor. Most bone tumors are non-cancerous (benign) and cannot grow. However, it is still possible that the bones could be weakened and the condition could result in fractures and other such problems. The tumor replaces the normal and healthy tissue.

It is observed that cases of malignant bone tumors are much less as compared to benign tumors. Some examples of benign bone tumours include:
1. Giant Cell tumor: In this condition, the tumor affects the legs
2. Osteochondroma: This form is most common among teenagers
3. Osteoblastoma: A tumor which develops in young adults and affects the spine
4. Osteoid osteoma: A tumor that affects long bones
5. Enchondroma: Usually found in hand and feet.

As stated earlier, metastatic cancer is a form of cancer that spreads to the bone from other spots in the body. Cancers from other parts that commonly spread to the bones include breast cancer, prostate cancer and lung cancer.

It is common for bone tumors to bear no symptoms and they can only be diagnosed using an x-ray. However, a typical symptom of bone tumor is pain which:
1. Starts in the region of the bone tumor
2. Often feels achy
3. Worsens with activity

Other symptoms that might be related to bone tumors are:
1. Abnormal swelling around the bone
2. Fever
3. Limping, in rare cases

Treatment
1. Benign bone tumors can be treated by just using medications. However, the tumours might relapse even after treatment.
2. Treatment of malignant tumor might require a number of specialists. The stage of cancer plays a vital role in the treatment.

Common treatments for bone tumor are:
1. Amputation: It is the surgical removal of all or a part of the limb and is conducted only if the tumor is large.
2. Radiation therapy: High intensity X-rays are used to kill cancer cells and treat the tumors.
3. Chemotherapy: Tumor cells which have infiltrated the bloodstream are eliminated using cancer drugs. If you wish to discuss about any specific problem, you can consult an Orthopedist.

I had sex with my wife on 4th june and on 5th june we took the unwanted 72. Firstly she did get any withdrawal bleeding and her due date was around 20 june Its 8th july she still haven't got her periods started. 1) is there any chances of her being pregnant? 2) when should we expect cycle to start? 3) what should be our next step? 4) is it normal to see this much delay in the cycle to start which is around 18 days from the normal cycle date?

MBBS, DNB - Obstetrics & Gynecology
Gynaecologist, Mumbai
1. Yes there is a chance of her getting pregnant. There may be failure of unwanted 72. However chances are less. 2. Cycles may be irregular after emergency contraceptive. Exact date is difficult to predict. Depends on factors like what is patients menstrual cycle length. How stressed pt is. Any co morbidities etc 3. Pls get a urine pregnancy test. And if positive go to a nearby gynaecologist. Or consult me online. 4. If upt is negative and her periods are delayed for 18 days then it is not normal. She will require further evaluation.

Massage Therapy - How It Can Help In Reducing Pain?

MBBS, MD - Anaesthesiology, FIPM, Fellowship in palliative medicine, certificate in interventional pain management, Multidisciplinary pain management course
Pain Management Specialist, Pune
Massage Therapy - How It Can Help In Reducing Pain?

When a child falls down and starts crying, the first thing a mother does is to pick up her child and console him/her and rub where you are hurt. Even when we grow older, our natural instinct when we bang into something is to rub the affected part. Even with modern medicines, the healing power of touch has made massages a popular type of alternative medication for aches and pains.

A massage helps relieve pain in a number of ways.

  1. First, it relaxes the body and mind and keeps the mind from thinking about the pain.
  2. Second, a massage warms the body and increases blood flow. This is very beneficial in cases of pain caused by sore muscles and stiff joints.
  3. Third, studies have also suggested that a massage can promote the release of opioids or natural painkillers in the brain. This hormone relaxes the body and encourages feelings of contentment.

A massage also reduces cellular stress and boosts mitochondrial biogenesis. This helps speed up the healing process of cellular injuries and gives energy to the cells. Many pregnant women also find a massage very helpful in dealing with labour pains. Other benefits of a massage include reducing anxiety, exercising muscles, enhancing immunity and easing a person’s dependence on medications.

There are many types of massages, but there is no clear evidence that makes one type of massage more beneficial than the others. While some people prefer a Swedish massage that involves kneading the skin, others prefer a deep massage or pressure point massages. Some of the common types of body pain that can benefit from a massage include headaches, lower back pain, arthritic knee pain, neck pain and fibromyalgia. A massage is also said to be helpful in reducing pain triggered by cancer and cancer treatment.

As compared to painkillers, a massage is typically considered safe as long as you are being massaged by a trained professional. In the wrong hands, a massage can, in fact, worsen pain. For slight, occasional pains, a massage is great, but when it comes to more chronic pains, it is best to consult a doctor before getting a massage. This is because the pain may be a symptom of a larger problem. Other instances when you should not get a massage include in cases of a fracture, deep vein thrombosis, burns or open wounds, severe osteoporosis or thrombocytopenia or if you suffer from a bleeding disorder.

In case you have a concern or query you can always consult an expert & get answers to your questions!

My grandmother is now 67. And she got menopause at 45. 2 days back she got some bleeding .And today the bleeding was heavy. What could be the underlying cause. please help me with treatment.

MBBS, DGO, MRSH, DGO,
Gynaecologist, Gurgaon
Get a pap smear done for her to rule out cervical cancer. India is having highest population of cervical cancer, after China, she may not have had cervarix vaccines in her premenopausal days, so don't delay and immediately get a pap smear and sonography done for her, it's better to be safe than never. And prevention is better than cure, as even if there is cancer and it is in early stage, uterus can be removed, followed by chemotherapy and radiotherapy to prevent it spreading further.
