Hi it's normal to have pain after starting gym or any new physical activity due to spasm in muscles. But if pain is not tolerable than consult physician. Best wishes.

You have mentioned the word dislocation which is worrying for me as an orthopaedic surgeon because this is a term we se rarely. Anyway to understand your problem and give an opinion as to why you are not having pain is possible only if I can see your x rays or mri scans of spine. Also I think you are very young that is why you are not experiencing too much pain as of now, but let me reserve my opinion till the time I see your investigations. It requires detailed investigation and examination. Preliminary investigations suggested: hb, tlc, dlc, esr, blood sugar fasting & pp, uric acid vit d urine r/e x-ray of the affected part. Anyway it may be tried sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomantation paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back, knee & general exercises. It may have to be further investigated. Do not ignore it. Let it not become beginning of a bigger problem. Make sure that patient is not allergic to any of the medicines that he/she is going to take. If it does not give relief in 4-5days, inform.

If your pain is only a localized one, and since 2 days, you can start with - ice packs at painful site to reduce muscle spasm - gentle anti inflammatory medication like tab etoshine, tab diclofenac etc which suits your body - a muscle relaxant can be added to provide relaxation to muscle spasm - analgesic spray like dynapar qps can be used for local application after fomentation 3-4 times in a day - rest for 4-5 days, use hard mattress, avoid lifting heavy weight and bending forwards acutely we will initiate your physical therapy programme once your pain subsides. Why do I get Knots in Muscles in Neck and Back?

What are muscle knots?
Muscle knots are points within a muscle where contracted fibers are unable to release 1, or, as the dictionary puts it: highly irritable localized spots of exquisite tenderness in a nodule in palpable taunt bands of muscle tissue.

A common problem for active people, muscle knots technically called myofascial trigger points, or mtps feel like a small knot to your fingertips. These knots can range from the size of a pinhead in smaller muscles to the size of your thumb in larger muscles. 2

Muscle knots can cause pain in two ways: (1) latent trigger points, which are knots that only hurt when you put pressure on them, and (2) active trigger points, which are knots that actively refer pain along your neural pathways, causing it in non-localized areas.

Trigger points in a constant state of contraction have excess metabolic waste and oxygen use because blood flow to this area stops this sends pain signals to the brain. Because your brain wants to stop the pain, it commands the muscle to rest, which leads to under-usage of the muscle. This is what makes the muscle shorten and tighten up.

What causes muscle knots?
The most common causes of muscle knots are:

1) accidents acute trauma, such as bad falls sports injuries that strain you joints and muscles
2) postural stress sitting too long with poor posture, sitting with no support, lifting improperly
3) overstimulation

Pain, paresthesia, numbness etc in thigh especially in outer aspect can be due to compression of a nerve which is called as meralgia, also it could be due to compression and symptoms from the back region. Initially you can start with: - physical therapy like tens - medication like pregabalin or gabapentin in small doses can be started and we can monitor the response - can also take an anti-inflammatory medication for some time especially if there is any pain associated. Let me know in around 1 week -10 days how are you feeling, we can get some tests to establish the diagnosis and also step up the medications as per the response. LOW BACK PAIN EXERCISE; DEAR you FOLLOW THIS EXERCISE LUMBAR EXTENSION STRETCHES Generally these are good for individuals who may have a disc herniation. They should be performed with caution for anyone who has spinal stenosis or any known fractures in their vertebrae. Prone Press Up: Begin by lying on your stomach with your elbows bent underneath you and palms flat on the surface. Keeping your hips and pelvis in contact with the surface, lift your upper torso off the mat with your arms, keeping your back muscles relaxed. Only go as high as you are comfortable. Perform 10 repetitions holding each one for 10 seconds each, working your way to 30 seconds Generally, the flexion based stretches are good for those with spinal stenosis or tightness through the lower lumbar musculature. Single Knee to Chest: Begin by lying on your back with both knees bent. Bring one knee up towards your chest. Perform 2-3 repetitions, holding each one for 15-30 seconds. You may feel a stretch along the lower back or buttocks area. You may also perform this with both legs up towards your chest if it is comfortable Prayer Stretch or Child’s Pose: This stretch is for the lower back muscles along the spine and is a very common yoga pose. On your hands and knees, sit back so your buttocks is resting on your heels. Reach your hands forward to lengthen your spine and feel a stretch in your middle back. You can reach your hands to either side to focus the stretch on the opposite side of your spine. Hold for 10-30 seconds and perform 2-3 repetitions. Angry Cat Stretch: This stretch can incorporate both an extension and a flexion component. You may perform either way or just one way if that is more comfortable for you. On your hands and knees, let your belly sag towards the table to increase extension through your spine (lumbar extension picture). To increase flexion through your spine, arch your back upwards, bringing your spine away from the table (lumbar flexion picture). You may hold each position for 5-20 seconds and repeat several times NERVE STRETCHES Just as you can stretch a muscle, you can stretch a nerve. Nerve stretches are very important to perform if you have any radiating pain from your lower back into your buttocks or legs. Nerve stretches are also very important to perform after lumbar surgery to ensure that there is no scar tissue adhering the nerve to any internal structures. Sciatic Nerve Stretch: This stretch should be performed if the majority of the radiating pain into the legs is felt in the buttocks, back of the leg, and/or through the calf and foot. This follows the pathway of the sciatic nerve. By stretching the nerve, it can help to desensitize it so that it will not cause as much pain. Perform this stretch by lying on your back with your hand behind one knee, preferable the leg with the sciatic pain. Straighten your knee then alternate flexing your ankle back and forth. Only hold your ankle in each position for a few seconds. Perform 10 repetitions on each side. As with beginning any new exercise, start off slowly with a very gentle stretch. Stop if you have any increase in lower back pain or sciatic pain down your leg. If unsure whether you should perform these stretches, consult your doctor or physical therapist.

Mr. Lybrate-user knee joint problem at your age be recovered totally. It may be due to over exertion or injury. Take milk both time morning and evening. First of all check your posture when you are lifting any material like heavy bucket. And try to lift the heavy bucket without bending your back. You may lift the bucket after bending your knees only. This will definitely help you. * gentle massage with lukewarm mustard oil of your waist area will also give you relief. * surya namaskar is only exercise which you need to do. * in diet vegetable protein of cereal, b complex supplements and half teaspoon of haldi in a cup of milk for seven days will give you enough relief.

Apply hot fomentation twice daily. Avoid bending in front. Postural correction- sit tall, walk tall. Extension exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang asana -- lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core strengthening exercises- straight leg raised with toes turned outward, repeat 10 times, twice a day.

Avoid forward bending repeatedly. Do hot water fermentation with added salt in it two times a day. Do back strengthening exercises thrice a day. You may get relief from pain. Consult a physiotherapist for detailed evaluation. Take care. Homeopathy has got wonderful result in backache.

Hi thanks for your query and welcome to lybrate. Actually pain at calf/leg region can be due to many reasons, commonly it is muscular in nature but it can also be due to tendons, nerves, blood vessels etc. My advise to you for initial 2-3 weeks will be: - ice packs application intensively with elevation of your limbs at night - take a short course of anti-inflammatory medication as per your body's suitability - stop your physical activity till pain completely subsides - intensive physical therapy, training and proper muscle conditioning once pain has subsided - slow resumption of physical activities with proper pre activity warm up and stretching of muscles and short increments in duration of physical activity each day as per pain tolerance. - we will get some blood tests for you - serum vitamin d and serum potassium levels, esr and crp once you have all these reports then you can contact me as I can advise you for proper supplementation do not hesitate to contact me if you need any further assistance. Most probably you are suffering from oa of knee. Knee surgery-total knee replacement (tkr) is quite safe & gives uniformly good results, in our hands. We have well equipped center in delhi. Do ask for detailed treatment plan. Surgery should be done when patient is still active & will be able to take benefit of surgery. Do not ignore. It could be beginning of a serious problem. Kindly show me x- rays before a final decision can be taken. You will be thoroughly investigated before surgery is under taken. Risk in surgery is less than the risk in crossing road.
