Lybrate-user, it is good to know that hypothyroidism is in good control. Usually all symptoms like tiredness and fatigue disappear on good control of hypothyroidism. Since you still feel tired and less energetic, it better to get hb checked. Get a cbc done, which will give full blood cell details. Plus take healthy diet and exercise daily for 1 hour. Also do pranayam. Most often tiredness is more a mental problem. Once you start exercising you will start feeling better. For back pain to there are specific exercises to strengthen those muscles, I am sure once you start doing it you will feel better. Thanks.

It is also essential for men to have normal levels of the FSH hormone. An imbalance of this hormone in men can contribute to infertility. For men, FSH controls sperm production which is of course essential to fertility. Typically, a man’s follicle-stimulating hormone levels should be between 5 and 15 IU/L. High levels of FSH in a male could indicate that the testicles are not functioning properly, or may have been damaged by some condition. They can also indicate the presence of Klinefelter syndrome, a genetic disorder that causes the presence of too many X chromosomes in the male. So get semen analysis done, and consult with some good infertility specialist.

Hello Doctor, We had unprotected sex on 24th July and had ipill on 25th morning. We had unprotected sex again on 25th night. Last cycle my period started on 1st July. It's 2nd July and no bleeding till now. There's a foul smell in urine. Am I pregnant?

It is very less likely to get pregnant with inter courses in the last week of the menstrual cycle, particularly you also happened to pop an I pill. However it is always advisable to be cautious. You will have to wait fir another 7-8 days for periods to begin. If they don't, do a urine pregnancy test to rule out pregnancy.

Hello,

I constantly try to educate patients on proper posture, but because people (myself included) spend so much time driving and sitting for prolonged periods of time. Driving can mean different for different people. It may be a way of leisure for some or as an everyday activity to commute to office. Driving for long time has always accompanied with some or the other musculoskeletal problems. It become a leading cause of back pain to our fast pace life.The problem is treated but the root cause of it is never looked after or we just don't bother about it much..

Why we get the pain or associated problems when we drive for long time?

Poor posture while driving can produce a repetitive load to the tissues that causes sustained stress. Daily prolonged drives apply loads to the spine that can cause viscoelastic tissues to slowly deform and creep. Simply staying in the car seat with poor posture long enough will eventually ensure damage. Sustained load and resultant deformation/creep causes a progressive reduction in tissue strength.With your hands in the proper position on the steering wheel, the thoracic spine, scapula and shoulder joint have a better chance of remaining in neutral. usually, we hold steering wheel with one hand and lean to other side.due to leaning hip bone( iliac crest) and 12th rib approximate; this will contribute to shortness (tightness) of the quadratus lumborum or latissimus dorsi

If the pelvis is rotated, it will contribute to lumbar or SI pathology and overactivity of the TFL(muscle). If the knees are even slightly superior to the hips, then there will be hip flexors muscles over activity or shortness, and hip extensor inhibition or weakness. Prolonged sitting on the glutes can cause poor circulation and lack of oxygen to the tissue, contributing to inhibition.

A person driving with a clutch who has poor or limited ankle dorsiflexion (like after ankle fracture) can perpetuate overly active hip flexors because they will pull the foot off the clutch using the hip flexors, instead of dorsiflexion at the ankle.

Some Do’s:
1) Select a car that matches your height and suits your work.
2) Change posture as much as possible take frequent Microbreaks ,recommended atleast 15 minutes, every 2 hours
3) Place hand on steering wheels so that elbows are close to your side and not so high that you have to reach up for the wheel.
4) Place hands at 3 o’clock or 9 o’clock position this will reduce the amount of strain on shoulders and put hands in safer position.
5) It is advisable to keep two hands in steering wheel except while shifting gears. Change hand posture frequently to reduce fatigue and improve circulation.
6) Learn Microbreaks: An example of a "microbreak" for driving in the car is squeezing the shoulder blades together for 20-30 seconds and repeating 2-3 times. Repeating anterior and posterior pelvic tilts and lateral tilts for a minute or two is sometimes helpful. Other microbreaks for driving include cervical range of motion, and shoulder retraction with elevation and depression.
Some Dont’s:
1) Avoid lifting after driving long hours because your muscles are tired your ligaments are stretched and disc are at risk of injury, give yourself some time to stretch and relax.
2) The death grip posture, the one arm cool dude” the Wrist resting at 12 o’clock position on steering wheel and fingers flop over the top, arm straight out in front and one arm propped on your window.


The longest journey begins with a right sitting posture, not with a turn of the ignition key !!!! dive safe

Amenorrhoea is a symptom with many potential causes. Primary amenorrhoea (menstruation cycles never starting) may be caused by developmental problems such as the congenital absence of the uterus, failure of the ovary to receive or maintain egg cells. Also, delay in pubertal development will lead to primary amenorrhoea. It is defined as an absence of secondary sexual characteristics by age 14 with no menarche or normal secondary sexual characteristics but no menarche by 16 years of age. Secondary amenorrhoea (menstruation cycles ceasing) is often caused by hormonal disturbances from the hypothalamus and the pituitary gland, from prematuremenopause or intrauterine scar formation. It is defined as the absence of menses for three months in a woman with previously normal menstruation or nine months for women with a history of oligomenorrhoea.

Dear, the ovulation day for a lady with regular 28 day cycle is the 14 day but to be sure since you are a pcos patient would suggest to get a usg follicular monitoring.

1.Eat a lot – This sounds much easier than it really is. Whatever you’re eating now, you should probably double it. If you eat three meals a day, instead eat six. You need to be eating every 2-3 hours, and each meal needs to be the size of a normal meal. This is going to be difficult for a few weeks, because you’ll often have to force yourself to eat even when you’re not hungry. 500 extra calories a day = 1 extra pound gained per week. Whatever you’re eating now, add an additional 1000 calories (spread throughout the day), and you’ll put on 2 lbs a week. Not just any calories though… 2.Eat a lot of good things – You need to eat a ridiculous amount of calories (probably 3500+ per day) if you want to gain weight, but you want to make sure most of those calories are GOOD calories. You could easily get 3500 calories eating Taco Bell and Twinkies, and drinking Mountain Dew, but that will just make you fat. If you want to build muscle, you want to eat healthy calories that are loaded with good protein, good carbs, and healthy fats…which brings me to my next point: 3.Protein = building block for your muscles. Chicken, fish, meat, eggs, milk, almonds, peanuts. Eat lots of this stuff, all the time. 4.Carbs will help you put on weight, but it won’t be muscle – pasta, brown rice, wheat bread, oatmeal, etc. Will help you put on weight, but a lot of that weight will be fat. Every meal should have vegetables and fruit. If you just eat protein, your body will resort to using it for energy rather than building muscle. I learned this the hard way in college; four years of exercising and three protein shakes a day got me NOTHING. Unless you’re eating tons of good fats (almonds FTW), good carbs (fruits and veggies), and protein, you won’t be gaining weight. 5.Compound exercises are your friend– Concentrate on complex, compound exercises that recruit as many muscles as possible: bench presses, dumbbell presses, squats, deadlifts, pull ups, chin ups, and dips. Do these exercises, and concentrate on lifting as much weight as possible. Don’t worry about triceps extensions, shoulder shrugs, bicep curls or crunches. All of the compound exercises listed here use every muscle in your body, and when you overload your body with calories and protein, those muscles will grow.. 6.Appearance is a consequence of fitness – This is the mantra of the actors of who trained for the movie 300 – would you be okay looking like a Spartan? Concentrate on being really strong and lifting heavy weights, and your body will follow suit. It doesn’t matter if you can only bench press 10 lb dumbbells right now. Wherever you’re starting out, concentrate on being stronger each and every time you exercise. Push yourself, get stronger, lift more, and before you know it you’ll be ripped. 7.When exercising, keep your rest between sets to a minute or less, and don’t do more than 12 reps in a set – Keep your range of reps between 6 and 12, and try to keep the time you rest between sets to a minute or less. Example: incline dumbbell chest press – 12 reps of 50 lbs, wait a minute, 10 reps of 55lbs, wait a minute, 8 reps of 60 lbs. 8.Let your muscles rest – never exercise the same muscle two days in a row. Your muscles get rebuilt (larger) during your days off, so never exercise the same muscle before it’s ready. I usually wait at least 48 hours before I hit the same muscle again. 9.Sleep – you need to be getting 8-9 hours of sleep every night for maximum gains. Your body is doing nothing but lying there and building muscle while you’re sleeping. If you are only getting 6 hours or less, you’re not going to get all the benefits of your exercising and diet. I know it’s tough, but those 3AM raids on Runnyeye need to be put on hold for a few months. SLEEP. 10.Cardio is your enemy – Running long distances isn’t going to help you. If you’re going to run, do sprints or run up a hill. Think about it: would you rather look like a sprinter or a marathon runner? Keep your distance cardio to a minimum if you want to put on some pounds. 11.Make it part of your routine – It’s okay to skip a workout here and there, but it is NOT okay for you to skip a meal if you’re serious about weight gain. You need to be always eating. It sucks, it’s practically a full time job, but it’s what you have to do. Eat. 12.Realize you will put on some fat – with all of this eating, you are going to be putting on some fat along with your muscle. That’s okay! Figure out what your goal weight is, and then add another 5-10 lbs on top of it. Once you get to that weight, cut back on the carbs in your diet, do more sprints, and keep exercising: you’ll shed the fat quickly and be left with a killer figure. Eat all the vegetables you want, but cut back on breads, pasta, rice, and oatmeal. U need to take a high protein high calorie diet.

You can report to a govt facility to take emergency dose of anti HIV meds. Also, get yourself checked for HIV, HBsAg, anti HCV. Repeat these tests after 3 months. If you develop symptoms like ulcer, rash, swelling in groin, fever, see your doctor immediately. It's the best not to indulge in high risk sexual behaviour.

If she has a normal menstrual cycle the the chances of ovulation is around 12th to 14th day. 19th to 28th will have more chances for pregnancy. More precise from 21 to 23rd.

Polycystic Ovarian Disease

Polycystic Ovarian Disease results from the disorder more commonly known as PCOS or Polycystic Ovary Syndrome. A syndrome is starkly different from a disease, for the former entails a set of symptoms usually occurring in concurrence, while the latter means a particular biological condition with an identifiable reason behind it. 

A syndrome solicits your concern and forethought in order to eliminate or at least control the chances of an underlying disease. Trigger to a syndrome is undetectable and hence fighting it efficiently is fairly problematic. It is rare to find adolescent girls without hormonal problems.
 
Hormonal imbalances are just too common to be a novel topic any further. Polycystic Ovary Syndrome is faced due to imbalances in hormone secretions. It refers to the development of multiple cysts in your ovary. This condition usually affects women falling within the age group of 15 to 50. Eight among ten women, at a given point of time, are reported with PCOS. Symptoms of PCOS, when neglected, can lead to Polycystic Ovarian Disease. If you have been facing similar health issues, it is advisable to undergo a check- up and get diagnosed at the earliest possible. 

Symptoms of Polycystic Ovarian Disease

  1. A receding hairline or increased hair fall can lead to thinning of scalp hair. Such a sign should not be taken lightly as it could be indicative of Polycystic Ovarian Disease.
  2. Extremely painful menstruation, irregular periods or prolonged periods with heavy bleeding might be reason enough for worrying.  
  3. Development of acne could be another sign. Acne is common to girls attaining puberty, but an unprecedented growth should be a matter of concern. 
  4. Patchy skin, pigmentation and discoloration of the skin are the other symptoms.
  5. PCOS may also result in continual depression and anxiety.
  6. Obesity is the most commonly noticed symptom. People suffering from Polycystic Ovarian Disease have lower metabolism and thus, reducing weight is an ordeal. 
  7. Many women tend to grow thick facial hair or chest hair. Such a condition discourages the person to go out in public. It leads to social ostracizing which further bears severe effects on one's mental health.
  8. PCOS mostly affects women belonging to the childbearing age. As a result of this syndrome, females stop ovulating. Conception becomes extremely difficult as the syndrome may lead to infertility.

Polycystic Ovarian Disease can result in Type 2 diabetes or even cardiovascular problems. If not diagnosed and treated at an early stage, it might lead to ovarian cancer. There is hardly any preventive measure to counter the chances of such a syndrome, but certain alterations in your diet under the guidance of a dietician and regular exercising can improve your health.
