I have delivered preterm baby and he is not breast feeding I am feeding him similac 1 baby is 20 days now his weight is low what should I feed him should I change baby powder n as he is not breast feeding I want to stop my milk any medicine please advice as I need to get to work after 15 days.

PGPN, MD
Pediatrician, Vizianagaram
Most of the preterm babies loose 15% body weight and starts regain after 15 day. Expected wt gain is 20- 30 gms on dialy basis. What your feeding now is good. Instead of bottle feed, start spoon feeds. Look for dilution of milk, 1 scoop for every 30 ml of water. Do not stop breast milk.

My baby boy is 2.7 years old but not speaking as other babies in his age group. He talks in his own language and communicate by sign/signals. He also does not chew food and having difficulty to sallow it, I need to give him semi solid food or need to blend it, otherwise he will vomit it. I consulted my baby's pediatrician (and few more doctors) and he said its normal and asked to wait till 5 years. Please suggest me the solution for the both. For speech, should I consult a speech therapist?

Pediatrician, Pune
if he can hear well , speech will develop in due course of time , try to interact with the child as much as possible along with playful activity , can give him regular food made at home for everybody , no need to give semisolid food , though initially you may have some difficulty , let the child feed himself , don't try to spoonfeed yourself , he will enjoy the food as play and finally put it in his mouth by himself

Fever in Children And Home Remedies

MBBS, Diploma in Child Health (DCH)
Pediatrician, Gondiya
Fever in Children And Home Remedies

Fever remains the most common concern prompting parents to present their child to the emergency department. Fever has traditionally been defined as a rectal temperature over 100.4 F or 38 C. Temperatures measured at other body sites are usually lower. The threshold for defining a fever does vary significantly among different individuals, since body temperatures can vary by as much as 1 F. Low-grade fevers are usually considered less than 102.2 F (39 C).

Fever itself is not life-threatening unless it is extremely and persistently high, such as greater than 107 F (41.6 C) when measured rectally. Risk factors for worrisome fevers include age under 2 years (infants and toddlers) or recurrent fevers lasting more than one week. Fever may indicate the presence of a serious illness, but usually a fever is caused by a common infection, most of which are not serious. The part of the brain called the hypothalamus controls body temperature. The hypothalamus increases the body's temperature as a way to fight the infection. However, many conditions other than infections may cause a fever.

Fever in Children - Causes:

Causes of fever include:

When to seek medical care:

  • The child is younger than 6 months of age (regardless of prematurity).
  • One is unable to control the fever.
  • One suspects a child may become dehydrated from vomitingdiarrhea, or not drinking (for example, the child has sunken eyes, dry diapers, tented skin, cannot be roused, etc.).
  • The child has been to a doctor but is now getting worse or new symptoms or signs have developed.

Although you may have done your best to care for your child, sometimes it is smart to take your child to the emergency department. The child's doctor may meet you there, or the child may be evaluated and treated by the emergency doctor.

Take a child to an emergency clinic when any of the following happen:

  • One has serious concerns and is unable to contact the child's doctor.
  • One suspects the child is dehydrated.
  • seizure occurs.
  • The child has a purple or red rash.
  • A change in consciousness occurs.
  • The child's breathing is shallow, rapid, or difficult.
  • The child is younger than 2 months of age.
  • The child has a headache that will not go away.
  • The child continues to vomit.
  • The child has complex medical problems or takes prescription medications on a chronic basis (for example, medications prescribed for more than two weeks' duration)

Home Remedies for Fever in Children:

The three goals of home care for a child with fever are to control the temperature, prevent dehydration, and monitor for serious or life-threatening illness.

  • The first goal is to make the child comfortable by reducing the fever below 102 F (38.9 C) with medications and appropriately dressing the child. A warm water bath can also be helpful .
  • To check a child's temperature, one will need a thermometer. Different types of thermometers are available, including glass, mercury, digital, and tympanic (used in the ear).
    • Glass thermometers work well but may break, and they take several minutes to get a reading.
    • Digital thermometers are inexpensive and obtain a reading in seconds.
  • Oral temperatures may be obtained in older children who are not mouth breathing or have not recently consumed a hot or cold beverage.
  • Monitoring and documenting the fever pattern is achieved using a thermometer and a handmade chart.
  • Acetaminophen  and ibuprofen  are used to reduce fever.
    • Follow the dosage and frequency instructions printed on the label.
    • Remember to continue to give the medication over at least 24 hours or the fever will usually return.
  • Children should not be overdressed indoors, even in the winter.
    • Overdressing keeps the body from cooling by evaporation, radiation, conduction, or convection.
    • The most practical solution is to dress the child in a single layer of clothing, then cover the child with a sheet or light blanket.
  • A sponge bath in warm water will help reduce a fever.
    • Such a bath is usually not needed but may more quickly reduce the fever.
    • Put the child in a few inches of warm water, and use a sponge or washcloth to wet the skin of the body and arms and legs.
    • The water itself does not cool the child. The evaporation of the water off the skin cools the child. So, do not cover the child with wet towels, which would prevent evaporation.
    • Contrary to the popular folk remedy, never apply alcohol in a bath or on the skin to reduce fever. Alcohol is usually dangerous to children.

  • The second goal is to keep the child from becoming dehydrated. Humans lose extra water from the skin and lungs during a fever.
    • Encourage the child to drink clear fluids but without caffeine (and not water). Water does not contain the necessary electrolytes and glucose. Other clear fluids are chicken soup,  other rehydrating drinks available at the grocery or drugstore.
    • A child should urinate light-colored urine at least every four hours if well hydrated.
    • If diarrhea or vomiting prevents one from assessing hydration, seek medical attention.

  • The third goal is to monitor the child for signs of serious or life-threatening illness.
    • A good strategy is to reduce the child's temperature below 102 F (39 C).
    • Also, make sure the child is drinking enough clear fluids .
    • If both these conditions are met and the child still appears ill, a more serious problem may exist.
    • If a child refuses to drink or has a concerning change in appearance or behavior, seek medical attention.

Diet Food For Diabetes

PGP In Diabetologist, Fellowship in non-invasive Cardiology, MD - Medicine, MBBS

Diet Food For Diabetes

Diabetes control is governed by following the right diabetic diet. What to eat and what not to eat is important for diabetes control, cure or diabetes reversal.

These are the top diabetes diet tips from diabetologists and nutritionists. Even if you are pre-diabetic or borderline diabetic, or diabetes is part of your family, it is important to follow a diabetic diet to prevent diabetes.

Here is the Diet Food for Diabetics:

  1. Whole grains, oats, channa atta, millets and other high fibre foods should be included in the meals. If one feels like consuming pasta or noodles, it should always be accompanied with vegetable/sprouts.
  2. Milk is the right combination of carbohydrates and proteins and helps control blood sugar levels. Two servings of milk in a daily diet is a good option.
  3. High fibre vegetables such as peas, beans, broccoli and spinach /leafy vegetables should be included in one's diet. Also, pulses with husk and sprouts are a healthy option and should be part of the diet.
  4. Pulses are important in the diet as their effect on blood glucose is less than that of most other carbohydrate containing foods. Vegetables rich in fibre help lowering down the blood sugar levels and thus are healthy.
  5. Good fats such as Omega-3 and monounsaturated fats (MUFA) should be consumed as they are good for the body. Natural sources for these are canola oil, flax seed oil, fatty fish and nuts. These are also low in cholesterol and are trans fat free.
  6. Fruits high in fibre such as papaya, apple, orange, pear and guava should be consumed. Mangoes, bananas, and grapes contain high sugar; therefore these fruits should be consumed lesser than the others.

Small frequent meals:

A large meal gives rise to higher blood sugar in one's body, therefore it is essential to take small frequent meals to prevent both higher and very low blood sugar values and keep them constant. Small in between snacks can be dhokla, fruit, high fibre cookies, butter milk, yogurt, upma/poha with vegetables etc.

A person with diabetes should follow a diet which is low in carbohydrates, high in fibre and contains adequate amounts of proteins, vitamins and minerals; and avoid fatty foods and sweets. He/she should also take frequent small meals (5 meals pattern).

What not to do:

  • Artificial sweeteners can be used in cakes and sweets for diabetic people (in moderation).
  • Have lots of fluid.
  • Limit intake of alcohol.

Should you have non-veg?

In non-vegetarian diet, seafood and chicken can be taken rather than red meat as red meat contains higher amount of saturated fats. Also, patients with high cholesterol should avoid egg yolk and red meat.

The diabetes diet for Indians includes carbohydrates, proteins and fats. As always, a balanced and planned diet can build and improve personal health. A controlled diabetes diet may seem like a drag and bore, but a good cook can add life to a diet.

Hello, A 3 yr old should eat everything that is made for the entire family. Food ethnicity plays a major role. Teach him to eat food that is made in your household, as he has to follow that all his life. There should not be a separate treatment for him.

I am suffering from dry cold since last month and also from cough. So tell me what should I do to release from cough.
Dear Lybrate User, *body should be kept warm especially feet, chest, throat and head. Sweating is very helpful. The simplest way to do it is drink hot spice tea or some other hot drink and lie down on the bed .Take some warm cover like blanket and let the body sweat for 15 to 20 minutes. *a glass of warm water mixed with one tea spoon of lemon juice and one tea spoon of honey can be taken 2-3 times a day. *case of dry cough especially if it trouble is during the night, take cardamom, dry ginger, black pepper and cinnamon in equal quantity, say 10 gms. each. Take white sugar or rock candy equal to the total weight that is 40 gms. Grind them and make a fine powder. From this take half a tea spoon ( about 3 gms.) twice a day with honey or warm water after meals. *Cold drinks, ice cream, sweets, fried foods, milk products like cheese, creams, yogurt etc. should not be taken. Hot milk without fat can be taken if boiled with a piece of crushed ginger. Half tea spoon of turmeric powder is good to mix with the milk. *you should avoid tomatoes, eggplant and bananas.

In the first 10 days babies will lose upto 10% of birth weight and thereafter they should gain 30- 50 grams per day if growth is fine, This way you can work out and this rate of growth is in first 3 months and then changes.
