Fibromyalgia - How To Deal With It?

Fibromyalgia - How To Deal With It?

Fibromyalgia is a chronic condition marked by constant flu-like symptoms such as fatigue, headaches, insomnia and joint pains. However, the disease has no visible symptoms. There are also no known causes for the disease. While some believe stress to be a trigger for fibromyalgia, other theories suggest that physical and emotional factors such as a prior illness lead to the onset of this disease. This leads to fibromyalgia patients being often misunderstood and misdiagnosed. Almost 80% of fibromyalgia patients are women. While the disease is not lethal, it can make daily life difficult.

The criteria for fibromyalgia were established only in the late 1900s. In addition, the symptoms of this disease present themselves differently in each patient. This makes diagnosis very difficult.

  • Constant pain all over the body is the most common symptom of fibromyalgia.
  • You may also feel constantly tired, and unable to get a restful sleep.
  • Some fibromyalgia patients also suffer from depression or anxiety and an irritable bowel syndrome.
  • Fibromyalgia patients may also feel hyper sensitive to cold or heat, and feel numbness in fingers and toes.

Fibromyalgia has no known cure, and like arthritis, treatment for this disease addresses its symptoms. Most treatment programs include a combination of drugs, physical therapy, and behavioral techniques.

Pain relievers or anti-inflammatory medication and antidepressants are the two types of drugs administered to fibromyalgia patients. However, these should not be counted on as a long-term solution, as the side effects of these medicines can cause additional problems. Therapeutic massages and chiropractic manipulations can also help ease the pain. Along with this, it is also important to rest and relax. This helps reduce stress which often triggers fibromyalgia symptoms.

To get a better night's sleep, try going to bed and getting up at the same time every day. A warm glass of milk or chamomile tea just before bedtime can also help induce sleep. As far as possible, avoid taking sleeping tablets, as the body can soon become dependent on these.

Though you may constantly feel tired, increasing the day to day activity levels can have a positive effect on your health. Consider adding low-intensity activities such as walking, cycling, yoga or dancing to your daily routine. Complement these with a well-balanced, nutritious diet that includes lots of fruits and vegetables. If you or a loved one has fibromyalgia, bad days may not be prevented. But with treatment, they can be controlled. If you wish to discuss about any specific problem, you can consult a Pain Management Specialist.

Quadriceps strengthening exercises- Quad Clenches: Lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, Hold for 3 secs. Repeat 10 times twice a day. Short Arcs: Lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight Leg Raise: Lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Dislocated Kneecap Symptoms and Treatment

Dislocated Kneecap Symptoms and Treatment

Whether you are an athlete or a ballet dancer, you will appreciate the importance of having a stable kneecap. Medically known as the patella, the kneecap is a triangular bone that connects the upper thigh to the lower half of the leg. It sits in a groove in the bottom of the femur (thigh bone). When the leg is bent, it stays within the groove. When the leg is extended, it provides support to the quadriceps muscles.

That being the case, a dislocation of the kneecap is a very common injury. Subluxation is a state where there is partial movement of the kneecap out of its position, thereby making the patient’s kneecap unstable. When it completely moves out of its place, it is known as dislocation. Whether you fall on your knees during a sport or have a fall from a bike or get injured during dance or aerobics, it is common to have a dislocated kneecap. Some people are prone to repeated dislocations.

Symptoms:

The initial injury is very painful and there might also be damage to the surrounding structures. Other symptoms include:

  1. Buckling of the knee, where your legs cannot support your body weight

  2. Sliding of the kneecap to a side

  3. Catching of the knee in the groove when trying to move it

  4. Pain in the front of the kneecap with any activity

  5. Painful while sitting

  6. Swelling and/or stiffness of the knee joint

  7. Crackling/creaking sound when trying to move the knee joint

  8. Inability to straighten the leg

Treatment:

Though these sound scary, the good news is that in 90% of the cases, the knee returns to its position spontaneously. However, putting it back into its place is a simple and safe procedure and can be done by almost any seasoned medical practitioner. The first step is to confirm that the kneecap is indeed dislocated. This can be done by a combination of physical exercise and x-ray. If required, MRI can be used, but it is not required in most cases. Initial treatment would include the following steps in sequence:

  1. Immobilizing the knee with splint by keeping the leg in a straightened position.

  2. Calling for medical assistance immediately. They can replace the knee back in its position carefully (reduction). An injured kneecap can cause what is known as foot drop by putting pressure on the peroneal nerve. The toes drag on the ground, making it difficult for you to walk.

  3. Use ice in the affected area for 15 to 20 minutes, and repeat after three to four hours throughout the day to reduce pain and swelling.

  4. Surgical correction may not be required, if there is a damage to the ligament.

  5. Flat femur and/or tissue laxity can cause repeated dislocations, where physiotherapy and strengthening exercises are useful. If you wish to discuss about any specific problem, you can consult a physiotherapist.

In a bike accident on 8/7/2016 my right lower femur fractured. There was no other injuries. I have been operated on 22/7/2016 with internal fixation plate. I was discharged on 6.8.2016 from hospital after cutting of my stich. In first review x-ray there was remarkable improvement in healing of the bone and callus were noticed. Surgeon advised my for non weight bearing for 02 more months. He also advised me for quadriceps drill and knee bending exercise. But I am facing problem in bending my knee. I feared in case of bending the bone get non union again. Is there any chance of non union during knee bending? Can physiotherapy help be taken for knee bending exercise. As I still in bed rest rest and using bed pan and urine pot for toilet. Can I use walker for going to toilet, bath and slow moving in the home at this stage? How time is require for complete healing of the bone? WHEN I COULD RETURN TO MY JOB AS MY JOB IS CLERICAL NATURE RAILWAY JOB.

MS - Orthopaedics
Orthopedist, Faridabad
As your fracture is already fixed ,you need not worry about displacement of fracture or of non union at all. After any fracture fixation ,It is advised to start active range of motion exercises as early as possible. This prevents the joints from getting stiff, increases blood supply to the fractured bone and thus helps in better healing of bone. This movement exercises do not hamper callus formation in any way but in fact they encourage callus formation and early recovery. If the fixation is good you need not worry about the bending of bone on exercising. Taking physiotherapy helps, but ask you physio to be gentle You should use walker for your daily activities without putting weight on the operated limb. You can take a physio help for that.

The underlying cause should be treated. It could be spondylosis, intervertebral gap reduction causing compression on nerves. In such cases physiotherapy is the solution. Traction may be required if assessed after X-ray reports. Yoga postures are also helpful. For Ayurveda safe treatment, pl send X-ray reports. Till then pl take Nycanthes leaves juice or decoction (Harsingar leaves)
