My 5 years old son is having frequent nose bleed for last one month. Though the amount of blood is quite less but the onset can be as frequent as twice a day. We consulted ENT doctor about 15 days back and he suggested its anterior and suggested paraffin drops along with Saline. This reduced the frequency but not entirely cured the problem. For last few days the frequency has again increased. We stay in Pune and the weather has been dry but changing now. He is taking Oleptal DT 150 twice a day for childhood epilepsy. Please suggest if we should revisit a specialist.

Children has tendency of nose picking. So nose bleeding (epistaxis) is very common in this age group. Inn summer there is dry air which causes dryness in nose which also lead to nose bleed. I suggest you that you apply small amount of ointment neomycin in anterior part of nose along with saline nasal drops 3 drops three times a day for minimum 5 days. And also encourage child to avoid nose pricking.

Treatment. Using a nasal decongestant spray, such as phenylephrine or oxymetazoline, can help relieve sinus infection symptoms short-term. But continuous use over 10 to 14 days can cause a rebound effect in nasal congestion.

Do gargle with warm water thrice a day. take sinarest it contains ( Acetaminophen+ Pseudoephedrine+ chlorpheniramine). Ear aches are a very painful condition, which tends to affect mostly children. However, adults can be affected by ear aches as well, which can disrupt their routines and be quite painful. Let's have a look at some of the aspects of ear aches in adults.

Causes of ear aches in adults
There are many causes for ear aches in adults and it may vary from one person to another. However some general causes of ear aches in adults are listed below:

  1. Infections within the body: If you have a throat infection or cold, it could trigger an ear ache as a side effect. Treating the underlying problems will, thus reduce the ear ache or it will go on its own when the symptoms of the cold subside. 
  2. Ear canal injury: Adults are most often guilty of putting in sharp objects to clean their ear canal, which may result in an injury and also lead to infections. These injuries can also be infected and cause fluid discharge and ear aches.
  3. Blockages caused by the regular use of ear buds: Using ear buds or other sharp objects to clean the ear will only result in pushing the ear wax further into the ear. This will result in blockages of the ear canal and cause ear aches.
  4. Dental problems: Certain dental problems, such as wisdom tooth growth can result in problems. Even gum disorders have been known to create ear aches in adults.
  5. Jaw pain and facial pain: Jaw pain and even facial nerve pain has been known to affect ears and cause aches as well.

Symptoms that may accompany ear aches
Depending on what the causes of the ear aches are, the symptoms may vary. Still, let's have a look at some of the known common symptoms of ear aches in adults:

  1. Irritability and lack of focus: Ear aches may initially manifest as a headache and make the person irritable.
  2. Poor sleep: Fluids may build up within the body and thus cause issues while sleeping as the fluids may shift from one area to another.
  3. Ear drainage: If in case fluids build up, they may drain from your ear, which could vary anywhere within yellow to dark brown colour. This may even indicate that your eardrum is ruptured.
  4. Hearing problems: Fluid buildup or other underlying causes may also cause hearing difficulties.
  5. Fever: Sometimes, fever may run as high as 104 degree C due the very infections, which may be causing the ear ache.

Whatever may be the cause, if you have ear aches for sustained periods without underlying reasons, you should consult a doctor immediately to get it checked out, so that appropriate treatment can start quickly.

I have done two brain MRIs in the last 15 months and have been told by the doctors that due to combination of lacunar infarcts observed in the frontal lobe and mucosal thickening in left maxillary sinus, has resulted in low blood flow to the brain. This is caused vertigo and there is no cure.

Vertigo is caused due to disturbances in semicircular canals of vestibular apparatus of the internal ear, blood supply to the posterior part (back) of brain especially pons, medulla and cerebellum. Frontal cortex is not concerned with this type of function. Blood supply to brain is through carotid artery which does not pass through maxillary sinus and its mucosal thickening should not be a reason for decrease in blood supply or lacunar infarctions. While there may not be a reason to deny the facts of the MRI reports but the explanations offered to you sound to be least plausible. Frontal cortex is mostly controlling emotional functions and memory of the person.

Take tab sinarest one tab at night for 3 days. Use otrivin nasal drop 2 drop in each nostril sos. Do saline warm gargle, take steam inhalation 2-3 times daily.

Please meet with an ENT specialist and investigate this throat problem. You have to quit smoking and need not wait for the results of the doctor's visit. You have to quit cold turkey because the body and mind will torture you endlessly while attempting reduction in smoking. The tobacco tar in cigarette smoke lines your lung with huge patches that make that portion dysfunctional. Your oxygen carrying capacity is jeopardized, and breathing is impinged. But don’t be discouraged because it is not too late even now to reverse all the ill-effects of smoking. I don’t know what other organs could be damaged but do go for a thorough medical check up to assess the effects of your abuse. Regarding improving your lungs, do a lot of aerobic exercise breathing through your nostrils only. Then there are a lot of deep breathing exercises in Yoga which you can learn too. Deep breathing cleanses your system and lungs and restores it to good health. Deep breathing uses the 80% of the lungs which are not normally used in normal breathing and even when exerting yourself. The greatest effect is in the lungs because this substance is processed through the respiratory system directly through the lung surface. When quitting, you must take one day at a time; though you must give up all smoking at once, and promptly. If you look too far ahead you will get discouraged and will give up. If you sit and count the number of days you have given up, that will also focus too much on achievement. You must focus on the here and now, on that day to be precise. After all quitting is not that easy due to many factors. So even if you fail one day, you must try and try again. Very few people succeed in their first attempt. Your persistence will eventually pay off. The urge to discontinue is hampered by many factors but the worst of them is the conditioning. In fact these conditions can exist all through the day unlike other addictions. But your desire to want to stop is the most important factor. It is important to find out about the ill-effects of smoking to have a reason that alarms you about continuing in the habit. The information is really scary but true and you must remind yourself about this every day. The nicotine in tobacco smoke stimulates the heart beat to raise the blood pressure in no time at all. So you must avoid it like the plague: it is just very bad medicine for you. In fact it is highly toxic too. You will need to stop smoking or consuming any tobacco related items with immediate effect. You should start vigorous exercises, gradually increasing it over a period of time. Do deep breathing exercises every time you feel like smoking (try Yoga), and replace the oral urge with some healthy food substitute to satiate the need. You may also take hard candy or chewing gum. If the urge is too strong, then use nicotine gums and nicotine patches, for a little while until the smoking drive reduces, substantially. You may also join Smokers Anonymous in your city where the support is really extraordinary for like-minded people who are also in the same situation. Keep yourself occupied or engaged with interesting activities during the times when it is most tempting. Keep the company of non-smokers for some time i. E. At least for the first 21 days. Above all announce it to everyone that you have quit smoking. Here’s a piece of very good advice: even if you accidentally/willfully take a cigarette, it is not the end of the world – you can start the cessation all over again. You must persevere with the best support until you defeat this addiction. Counseling is very useful in the initial stages and when temptations are at their highest.
