Quadriceps strengthening exercises- Quad Clenches: Lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, Hold for 3 secs. Repeat 10 times twice a day. Short Arcs: Lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight Leg Raise: Lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

Hi. I have been suffering from leg pain and weird feeling in my right leg and now back back for 6 months now and I can not stand on my right leg for more than 5 min. My all blood tests came negative and got my L/S Spine result yesterday only which suggested diffuse disc bulge causing thecal sac compression and bilateral forminal nerve root compression seen at L4-L5 level.

If it is stomach ache which is bothering you, then you actually need to get in touch of a gastroenterologist and maybe get a usg or something done for diagnosis. If it is lower back pain which is bothering you, then you will have to elaborate your problem as to a detailed discussion of your back pain which means duration of pain and mode of onset. I would also like to ask you if it is associated with any leg pain or any other neurological symptoms like numbness, paraesthesias (electrical shock like abnormal sensations) or any weakness in limbs. Please also tell me about any associated symptoms like fever, weight loss etc. What medications you generally take and what has been your treatment history till now for existing problem do not hesitate to contact me if you need any further assistance. Advice. Avoid sitting cross legged. Avoid squatting- quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Sports taping- stretch the tape from both ends and apply on the affected area contrast fomentation (hot and cold).

How Physiotherapy Helps Treat Sciatica?

The sciatic nerve is one of the largest nerves in the body. It goes from the lower back all the way down to the base of the leg. When there is a disorder associated with the sciatic nerve, it is known as Sciatica. 

Sciatica, however, does not necessarily need a surgery or a very elaborate procedure to fix the problem. Most commonly, it is treated simply by practicing a number of simple exercises. Here are some of the most common exercises prescribed to treat Sciatica:

  1. Reclining Pigeon Pose: There are three main forms of the pigeon pose. These are sitting, reclining and then forwarding. It is recommended that if you have very recently started your treatment for Sciatica, then certainly you should go for the reclining pose. In this pose, you are supposed to hold your left leg in the air at a right angle to your back. Then, hold your ankle against the knee. Finally, repeat this stretch with the right leg as well.
  2. Knee to Opposite Shoulder: In this stretch, the starting position is on your back side. You have to have your legs out and feet upwards. From this position, try to bring your right leg towards your left shoulder. Hold this for 30 seconds and then release and relax. Repeat this activity three times and repeat the whole procedure with your other leg. Only take your leg as far as it goes.
  3. Sitting Spinal Stretch: At the start of this exercise, sit on the ground with your legs going straight outwards and your feet flexed in the upward direction. Pull your left knee and put it on the outside of your right knee. Finally, place your right elbow on the left knee, so that your body turns to the left. Hold for 30 seconds and then relax. Repeat the procedure with the other knee.
  4. Standing Hamstring Stretch: Put your foot on an elevated surface such that it is above the floor, but not above hip level. Flex your toes forward such that your feet and legs are nearly in a straight line. Bend as far towards the foot as possible. However, do not stretch so far that you feel pain. Finally, repeat this procedure on the other side after release. If you wish to discuss about any specific problem, you can consult a Physiotherapist.

Do hot fomentation twice daily. Postural correction- always sit with a small pillow at your low back. Sit tall, walk tall. Extension exercises like bridging and bhujang asana x twice daily. Sleep on your back with a pillow under your knees or on your sides with a pillow between your knees. And get vit d levels checked. Go for regular walks follow 30-30 rule when at work. After every 30 min of work. Do 30 sec of stretching or stand and walk a bit.

Hi How soon can we resume weight training. I was diagnosed with 1.diffuse disc bulge at L3-L4. Moderate size broad-based posterocentral disc protrusion causes secondary central spinal canal stenosis, narrowing of bilateral neural foramina, mild compression on L5 traversing and abuts the L4 exiting nerve roots. I will much better and in no pain after 10 days of back exercise suggested by my physiotherapist.

1. Continue with Spinal Exercises for one month minimum 2. Wear lumber spine belt in Gym when you start with Exercises 3. Initially start with cardio exercises only. 4. Take calcium vitamin D 3 and Methylcobalmine supplement for one month minimum.

Hi = relax and stay positive. = increase fluid intake. = eat seasonal fruits for natural vitamins = do streching of hamstring, quadriceps, and calf muscles. = if you are overweight, reduce weight by cycling or swimming as it gives low impact on joints. = do check your calcium and vit. D = apply moist heat pad to affected joint and muscles. If still you dont get relief consult physiotherapist as there are various causes for leg/ joint pain and some test may be required for further investigation. Best wishes.
