My mother 's Thyroid ultra sensitive tsh level is 28.33, she is 58 year old, please suggest me is this very emergency case? And what should I do in such case.

Mr. lybrate-user, TSH level of 28.23 mU/L, shows she has hypothyroidism. This needs to be treated with thyroxine tablets. Because her thyroid gland is not producing the hormone in her body. Unless immediate supplementation started, and adequate hormone is given, she may have serious consequences because of deficiency. From that point of view it is urgent. If not treated there may be weight gain, slow heart rate, intolerance to cold, joint and muscle pains, mood swings, swelling of face and lower limbs, anxiety, depression, dry skin, dry hair and hair fall etc. Hence it is better to start the treatment right away. TSH level should be brought down to 3 to 4 mU/L, to be free from all symptoms. Thanks.

I am a diabetic since last 2 years and dr has prescribed me 1-gluformin xl 500 mg 2-cardace am 5 mg my hba1c was 6. 15 and fasting sugar was 90 p. P was 160. My microalbumin was 9. 26. Mg./dl- creatinine was 69. 9 mg/dl. Acr ratio was also 13. 3 against a limit of <30 mg /g and my doctor told me its ok. Kft-report 1-blood urea 30. 0 mg/ ref range 10-50 mg /dl 2-serum creatinine 1. 15 mg /dl ref range 0. 70 -1. 40 3-serum uric acid- 2. 9. Mg/dl ref -3. 5 - 7. 2 4-sodium -131. 6 mmol/l ref range 135-155 5-potassium-4. 11mmol/l ref -3. 5 - 5. 5 6-chloride-102. 8mmol/l ref -98 - 106. I am also diagnosed with mild prostrate problem. My problem I am suffering from excessive urination during day time from morning 10 am till 8 pm after that its normal during night sometime I go for urination once only that too if I had taken more then 2 glasses of waterbefore going to sleep. Now my question is. Am I suffering from any kind of kidney problem or its because of prostrate problem I have also noticed foam in my urine but not always. Please tell me how to check. Please advice some medicine for controlling my excessive urination which happens after 2 hour. I am also suffering for penis erectile dysfunction. Please help me and advice suitably to over come these problems and also tell me how to check the kidney functions.

Your wt should be 52 kg according to your height. Include less oil less salt less sugar in your diet. Do exercise. Include low calorie foods in your diet. Take 5 meals in a day in frequent intervals. Because of thyroid your weight is too much.

Mr. lybrate-user, If you have acute attack of gout, then you need to control the pain and inflammation with certain antiinflammatory drugs preferably with an injection. Then follow it up with tablets till the severity of acute attack is controlled. This is possible only with visiting your local doctor. Then you may also have to take drugs to reduce uric acid concentration in the blood. Plus avoid high purine (high protein) containing foods like organ meat, red meat, certain sea foods like sardines, salmon, scallops, herrings, haddock, mushrooms and mussels. Also avoid alcohol and aerated drinks (colas). Thanks.

Hello- Diabetic patient ,mai der saber kabhi na kabhi sex sambandhit samasyaein jaise, shighra patan, penis ka theek se uttejit na hona, sex ka man na hona, penis k infection ityadi hone ki sambhavnai rehti hai. Isse bache rehne ka bas ek he marg yah hai ki aap abhi se ayurvedic rasayano ka sewan shuru karein jisse ki diabetes ka asar apke sex power par na pad sake aur apka sex aur anandmay ho sake.

She has very high type -1 diabetes and it is not controlling in any way. She is now taking 2 doses of Insulin of 15 units each. On checkup according to reports her body is not creating insulin so if any physical injury is there it is not getting cured due to high diabetes.

Mr. lybrate-user, thanks for the query. For T1 diabetes, only treatment is insulin as in these patients there is literally no insulin produced. So if 15 units twice a day is not helping control, then dose may have to be increased or a prandial (with lunch) insulin may have to be added. Plus one has to look at the diet pattern and exercise too. A close co-ordination between the patient and care providing doctor has to be there. Thanks.

Hello, My Dad got high sugar level from last year, now it is been quite under controll But from that moment onwards he is suffering from Knee Joint pains regularly, soo many consultations done but not much improvement, one of my friend suggested me to give him omega-3 capsules. Can i go with this ?

You can continue with omega-3 capsules.This problem is not related to diabetes. It could be OA knee which can be confirmed with X-ray of the knee joint.

Your father's condition can be classified as pre diabetes also known as impaired glucose tolerance. He is at increased risk of developing frank diabetes. So for now life style modifications like low sugar and fat diet, daily 30-40 minute brisk exercise etc along with blood sugar check up at least every 6 months is recommended. If sugar level does not come down to normal range, prophylactic metformin regimen would be considered. Thank you.

In high uric acid avoid few diets i.e. high purine-rich foods, milk products, protein foods, meat, dried beans, dried peas, mushrooms, whey proteins which are used in zym, diuretic medicines. Eat balanced diet , eat banana, take amino acid rich diets, L- glutamine like supplements, whole grain, oats, green veg., low cholesterol foods, plenty of water in day time, Do test Kidney Function Test every 6 months to monitor levels of uric acid. Consult doctor always for medicines. Do not take medicines by self. Take tab. Zyloric 100 mg once a day. Tab. Ulracet sos for pain .

Jesseyji, Lethargic feeling could be due to many reasons including diabetes. Other conditions which can cause such a feeling are hypothyroidism, iron deficiency anemia and certain other vitamin & calcium deficiency. It is better to get blood glucose level as well as get a thyroid assay and CBC done. The results will give a clear idea. Infertility is a medical condition which is characterised by problems in successful conception, even after prolonged and frequent unprotected sex amongst a couple. Infertility may occur due to problems with either the male, the female partner and in many cases both of them. This condition can cause much strife in a relationship not just within the individuals but also be a cause for social embarrassment. The causes of infertility could be-

  1. Testicular injury or deformed testis,

  2. Irregular menstrual periods,

  3. Problems with erections,

  4. Uterus is ill-shaped,

  5. Premature ejaculation,

  6. Polycystic Ovarian Syndrome,

  7. Pelvic inflammatory diseases caused by Sexually transmitted infections such as gonorrhea and Chlamydia,

  8. Scanty sperm count,

  9. Thyroid disorders,

  10. Blockage of the fallopian tube.

Other lifestyle related reasons could be excessive alcohol consumption, aging, diabetes, obesity, high pressure and as a side effect of radiation therapy or other medications.

Ayurveda and Infertility-

According to Ayurvedic scriptures, infertility occurs when there is a deficiency of nutrients in the Shukra Dhatu of an individual. This might occur as a result of indigestion and excessive toxic content in the human body. This could be caused due to excessive consumption of spicy foods or if they have chronic stress or anxiety. The Ayurvedic remedies boost the nutrition of the Shukra Dhatu in order to boost fertility. The most potent Ayurvedic herbs which can cure infertility are as follows:

  1. A prescribed consumption of the powder of dried banyan bark along with sugar might help to cure infertility.

  2. The inclusion of cumin and turmeric powder in regulated dosages in your daily food might also help to facilitate fertility.

  3. A consumption of the root of the herb Ashwagandha for 3 times a day has also been shown to be beneficial.

  4. A daily consumption of the powder of Vanari Kalpa in warm milk might help in curing any problems related to the ejaculation of semen.

My wife is 57 years old her clinical symptoms were fatigue, feeling sad, tiredness, morning headache. I am retired male nurse come medical lab tech (naval hosp. Retired) got her thyroid and lipid profile. All were normal, but tsh was 13.02 hence she was advise thyronorm 25 mcg daily. After 3 months repeated test report is 0.14 where as t3 and t4 remains unchaged (t3 104.6, t4 10.00 should we reduce the dose or we can continue with same dose. Said doctor is not available. Please advise thanks & regards.

The tsh values are low because she is taking thyronorm 25 mcg, daily, and this is quite a low dose, when tsh is low t3 & t4 will be normal. Thyronorm is also t4 only. So please do not think of reducing the dosage. This has to continue. Only thing is keep checking tsh levels once 3 - 4 months.

Mr. Lybrate-user, the problem could be damge to blood vessels supplying to lower limb. To confirm how serious is the problem it is essential to get a colour doppler test as well as angeiography of lower limb vessels. With that it is possible to asses the extent of blood supply. Plus it is vital to have excellent blood sugar control with fasting near 100 mg and pp 150 to 160 mg and hba1c% <7. You are very young, so get these investigations and consult a vascular surgeon along with your diabetologist.

Caffeine - Can It Cause Infertility?

Many studies have shown that excessive caffeine intake affects your chances of conceiving by influencing the ovulation, hormone, and the fallopian tubes but the opinions on this issue are divided. However, research on this topic mostly indicates that, though caffeine can give you a kickstart in the morning, it can have quite a reverse effect on your efforts to conceive.

How intake of excessive caffeine reduces your chances of conceiving?
The fallopian tubes are responsible for carrying the eggs from ovaries to the uterus by rhythmic movements and too much caffeine can affect this movement and cause disruption in the journey of the eggs. Moreover, consuming more caffeine has also been associated with baby's growth restriction, miscarriages, and weight issues. It is preferable to get rid of caffeine altogether from your diet when you are trying to conceive but if you are an avid coffee drinker then you might want to cut back or replace your caffeine intake with milk and syrup which is far more healthy and beneficial.

Which foods and beverages do you need to avoid?
There are so many caffeinated foods and beverages, that it is almost impossible to avoid them. But it is better to take note of them so that you can regulate your caffeine intake. Coffee, tea, soft drinks, energy drinks are obvious sources of caffeine as they are supposed to give you energy. Some herbal teas also contain caffeine. Chocolate, ice creams and candies with coffee flavors, over the counter drugs for allergies, cold and headache also contain caffeine so it is important to read the ingredients list before consuming any of them.

To higher your chances of getting pregnant, it is important to cut back on caffeine. Most doctors are of the opinion that exceeding 50 mg of caffeine intake daily can seriously hamper your chances of conceiving. If you wish to discuss about any specific problem, you can consult a Gynaecologist.

Mr. Lybrate user, yes, that advise is good to fllow, you need to practce pranayama daily twice[morning night]. There are types of pranayamas, like anuloma, & viloma. Its better to take any yoga therapist/instructor/s help,& also, you better to do few asanas, atleast practicing suryanamaskaras daily minimum 4-5 times also will give good results. Take care. Try to take water atleast 3-4 litres. Irrespective of thirst, daily. Taking 1/2litres [or 1 litre}in morning itself is good cleanser for body.
