Hello everyone!

I am Dr. Audumbar Borgaonkar, consultant cosmetic and plastic surgeon from Ariva cosmetic surgery centre, Vashi, Navi Mumbai. I will be speaking about hair transplantation procedure. Hair Transplant as the word suggest is a procedure in which hair from one part of the body is extracted and it is implanted in the area where it is needed. There are two main types of hair transplantation procedures. First is Follicular unit method which is also called as trip method. Second is follicular unit extraction method. Follicular unit extraction is the preferred technique. Currently since it is involved with minimum pain and it is sutureless and it has faster healing time.

Follicular unit extraction technique involves removal of singular hair follicles from donor area in random fashion and they are implanted in the recipient area. As regards age, age is not much of a concern for hair transplantation and it can be performed in younger as well as older individuals. Even females with baldness can undergo hair transplantation procedure. The results of hair transplantation are long lasting or permanent. The hair those are extracted are taken from the donor area which is called as permanent zone or DHT resistant area. So these hair are less likely to fall. In cases where there is less donor area we can harvest hair from other body areas like chest or groin region.

The hair transplantation is relatively safe procedure and it is not associated with any major side effects. In the post-operative period there may be swelling which is self limiting and it reduces over a period of 2-3 days. In post-operative period the patient is required to take a course of antibiotic followed by multivitamin tablets. Also, we provide platelet rich plasma or PRP treatment to boost hair growth. If you want to know more information about hair transplantation procedure you can contact me through lybrate or book an appointment there.

Thank you!

Hyperpigmentation - Can It Be Treated With Laser Treatment?

Hyperpigmentation - Can It Be Treated With Laser Treatment?

It is a condition that causes discoloration and discolored patches to appear on the skin. The color of one’s skin is determined by the production of melanin in the body, and hyperpigmentation is caused by a disproportionate production of melanin. People of any race or skin color can be affected by this condition. The most common pigmentation problems are brown sun spots, melasma, age spots, and pigmented birthmarks.  Pigmentation issues very depending on the natural colour of the skin. Treatments include fade creams, IPL, Elos Aurora, laser treatments like Fraxel Dual and in some cases biopsy or skin excision is required to exclude skin cancer such as melanoma.

Three skin irregularities caused by this condition are; Post Inflammatory hyperpigmentation, Lentigines (or sun spots) and Melasma.

  1. Treatment of brown sun spots: Many people have mottled brown spots, freckles and patches on their skin due to excess pigmentation. These are easily treated with medical grade Elos IPL or Fraxel Dual laser. There is a range of different causes of pigmentation. Treatments differ depending on what type of pigmentation is affecting your skin. The brown pigmentation is called melanin and is made by special cells in the skin called melanocytes. The main categories of pigmentation problem are freckles, age spots, (also called liver spots or sun spots) melasma and postinflammatory hyperpigmentation.
  2. Melasma: Women with a genetic predisposition to this condition or those with darker skin complexions and those who have a higher exposure to ultraviolet rays are most commonly affected by melasma. It is a condition that affects the areas of the skin that are most exposed to the sun. It involves female hormones and so women who are pregnant are generally affected by melasma. Melasma is blotchy areas of brown skin pigmentation on the face. Melasma, (also known as chloasma), appears as blotchy pale brownish pigmentation on the face, especially forehead, cheek and upper lip. It is more common on darker skin types. It is especially common in women aged 20-40 and is associated with pregnancy or taking the contraceptive pill or injection. It also happens in healthy, non-pregnant woman taking no medications and men using aftershave. Sun exposure following some cosmetics can cause a phototoxic reaction where the chemical substance absorbs UV light. The resulting pigmentation extends where sun and cosmetic use have coincided, for example down the neck, often more on the right side in countries that drive on the left. Melasma is more stubborn and unpredictable to treat than freckles or age spots because the melanocytes have become sensitized and continue to produce extra pigment with even the slightest hint of sun. Sun protection is an essential part of treatment. Melasma can be treated with Active FX Laser Resurfacing. It destroys up to 80% of melanin, efficiently, which is ideal for cases of melasma that cause excessive pigmentation of skin.
  3. Lentigines: Minute brown spots that appear on your face and body, which is the collective result of continuous exposure to the sun, photodamage or chronic sun damage, are called lentigines. The spots usually have a visibly distinct border and appear on areas most exposed to the sun such as the face and hands.

How is one affected by post inflammatory pigmentation?
If there is an excessive production of melanin in the body, it could then overflow to the surface of the skin; this is caused by an inflammation. Post inflammatory pigmentation is the formation of brown patches on the skin that are the result of infection, injury or other conditions. Patients who already have a skin problem are more likely to be affected by this condition.

Post inflammatory hyperpigmentation (PIH) occurs after injury to the skin from whatever cause and is far more prevalent in darker skin. PIH appears as a dark brown area over the area of the previous injury and is due to excess melanin being produced by melanocytes as a result of the inflammation. Typically PIH resolves over several months but can persist for 1-2 years and in some cases can be everlasting. Treatment is generally patience as it will resolve and treatments risk further inflammation and therefore further PIH.  However hydroquinone can be useful in some patients and others benefit from mild treatments of Elos Aurora, IPL or Fraxel Dual laser.

Treatment of freckles

Freckles are small flat brown marks on sun exposed areas. The medical term is ‘ephilide’ and they are an accumulation of melanin in the surface skin cells (keratinocytes). The tendency to form freckles is inherited and is often seen in red heads and younger people. Freckles usually fade in winter and reappear in summer. They need no treatment except sun protection. They can also be faded by Elos IPL, but will reappear with sun exposure. No treatment can transform a skin which freckles, into one which tans evenly.  Of course, further sun exposure can bring the freckles out again so it is very important to apply a good sunscreen following treatment to maximise results.

How does laser treatment help to remedy hyperpigmentation?

  • Clear and Brilliant, Fraxel and Photofacial are three types of laser treatment that have been known to resolve hyperpigmentation.
  • Short bursts of energy and heat are produced by the laser and aimed at the cells that have a concentration of melanin. The excess melanin from these cells is then broken down and destroyed with the help of the laser.
  • Each case of hyperpigmentation varies, and their treatment varies on the extent of the condition as well. The treatment will be administered over a course of sessions.

Though these treatments are effective, one must always stay protected from damage due to overexposure to the sun, by wearing the sunscreen with the correct SPF and other sun gear.

Know More About Facelift Surgery!

The skin has underlying protein layers, mainly having collagen and keratin. With age, they lose elasticity and the skin begins to sag. This is where facelift gained popularity with various techniques including Botox, chemical fillers, laser beam treatment, and chemical peels. Read on to know how these minimally invasive, economic options work in literally lifting up the face:

  1. Laser treatment: A beam of laser (usually CO2 laser) is used to make small dots on the skin with normal skin left untouched in between. This causes tightening of the tissue underneath, producing a firmer appearance which can take off years from the appearance.
  2. Botox: This accidentally discovered neurotoxin is used to treat wrinkles and sagging skin. The sagging areas of the face (usually forehead and nasal folds) are injected with Botox. The wrinkles are gone and the fine lines are lost, and the result can last for up to 8 months.
  3. Fillers: Chemicals which are naturally present in the skin like hyaluronic acid and collagen extracts are injected into the skin to fill up the underlying skin and produce a fuller, younger appearance. The lower cheeks, corners of the eyes and mouth, and area under the eyes are commonly treated by this method.
  4. Chemical peeling: The top layer is often peeled off using chemicals so that fresh, younger-looking skin is generated. This also helps in reducing pigmentation, reducing forehead lines, de-tanning, and producing a glowing, radiant, younger-appearing skin. It also can be used in other skin conditions including rosacea.

While each of these is widely popular, they come with a set of potential risks and complications.

  1. Anaesthetic reaction: Most of the above procedures require local anaesthesia and there could be a reaction to this chemical.
  2. Hematoma: There could be small hematomas in the underneath layers of the skin, which will take some time but dissolve on its own
  3. Numbness/tingling: The face has a rich nerve supply, and if any of these are touched, there could be numbness or tingling or even loss of sensation in some cases. While some may improve with time, there also could be permanent damage
  4. Scarring: In chemical peels, there could be minor incisions and therefore scarring can happen.
  5. Delayed healing: Depending on the person’s immunity, healing could be delayed, and as it is the face, social and/or professional life can be affected.
  6. Infection: As with any surgery, chances of infection arise and can be managed with antibiotics.
  7. Bleeding: As most of these are non- or minimally-invasive, chances of severe bleeding are less but still persist.
  8. Bruising: The face, in the immediate postop period, can have a bruised appearance, which will improve over 4 to 5 days.

What Is Rhinoplasty?

MBBS, MS - General Surgery, MCh - Plastic Surgery
Cosmetic/Plastic Surgeon, Navi Mumbai
What Is Rhinoplasty?

Rhinoplasty, more commonly known as Nose-Job, is a cosmetic procedure designed to change the shape and size of the nose.

A Rhinoplasty can effectively change the size and shape of the nose to complement the facial balance. It also helps improve nasal symmetry efficiently enhancing your facial features.

How is the surgery performed?

We prefer General Anaesthesia for the surgery to make patient pain free and most comfortable. It’s a Day-care procedure or may need overnight stay.

The operation involves an incision which is made for easy access to the cartilage and bones of the nose. These incisions are generally placed inside the nose, so that they do not appear as scar marks after the surgery is done. Moulding, removal or replacement of the bone or cartilage depends on the demands of the restructuring of the nose. Once the reshaping of the nose is done, the nose tissue is put back into place and a splint is temporarily placed to support the new structure of the nose. The splint is removed at 5-7 days.

Precautions to take after a nose surgery

After a nose surgery, the area around your nose will stay swollen and bruised for around 10-15 days. Thus you have to ensure that-

  1. You get adequate period of rest.

  2. Use cold compresses over the swollen face, this will reduce the swelling and ease the pain.

  3. Do not indulge in relatively vigorous physical activity for 15 days.

  4. Take you medicines regularly.

Risks of nose surgery

Every surgery or medical procedure involves certain risks, and so does Rhinoplasty. Along with some puffiness and bruising around the eye and nose region, you may also experience-

  1. Some amount of bleeding

  2. Injury to the septum area

  3. Irritation of the skin and skin problems

  4. Infections

  5. Nasal blockage due to swelling in the inner nasal area

  6. Complications resulting from anaesthesia

Although there are certain risks to Rhinoplasty, the surgery is considerably safe if done by a qualified expert plastic surgeon and the results are quite permanent.

Wrinkles - How To Get Rid Of It?

Wrinkles - How To Get Rid Of It?

There is a wide range of options to get rid of wrinkles. Laser treatments are considered to be one of the best ways to eradicate wrinkles for a long time. There are other treatments such as microdermabrasion, photo facials, and chemical peels as well. Dermal fillers are used to treat deep wrinkles. Wrinkles of the forehead and the eye are best treated with Dysport and botox.

There are other treatments such as microdermabrasion, photo facials, and chemical peels as well. Dermal fillers are used to treat deep wrinkles. Wrinkles of the forehead and the eye are best treated with Dysport and botox.

How Does Laser Work?

Laser treatments such as the Fraxel and the fractional CO2 make tiny pores in the skin. These pores or incisions have two purposes. The first is to make sure that the skin produces collagen and the second is to ensure that the skin heals quickly. The laser can address wrinkles in areas such as eye, face, hands and legs. Out of all the treatment plans, laser treatment is by far the costliest one. They are typically used by maxillofacial surgeons having in-depth knowledge of physiology. Treatment of this kind might require more than one session depending on the extent of wrinkles.

How Does Dermal Filler Work?

Dermal fillers are a popular method to address wrinkles and are used to destroy wrinkles around the mouth and the eye. Lasers can also diminish hollowness and dark circles near the eye. Moreover, they are very effective in making up the lost volumes of the jawline, temples, and cheeks. Dermal fillers, however, last for about a year and needs to be redone to hold on to the facial changes. They are typically safe and contain limited side effects. A consulting session with the doctor will help to know more about the potential risk of this treatment.

Removing wrinkles from under the eye-

Botox and Dysport are two of the most popular options to get rid of wrinkles under the eye region. While lasers can diminish the appearance of wrinkles, botox and Dysport can permanently get rid of it. They also ensure that the wrinkles do not come back for quite some time. Some other wrinkles that botox and Dysport can fix include wrinkles of the forehead, eye, and the brows.

How Effective Are Wrinkle Creams?

In comparison to botox and lasers, wrinkle creams are far less effective. This being said, they are extremely popular for the fact that they are readily available over the counter and are very cheap. Most wrinkle creams contain retinol that helps to increase collagen production and stimulate turnover of the skin. Wrinkle creams that are designed for the eye do not contain retinol since they are an eye irritant and can cause damage to the eye. These creams also contain anti-oxidants that help the collagen to hold their own and stop the interference of free radicals.

How To Take Care Of Flaky Skin?

MBBS, MS - General Surgery, MCh - Plastic Surgery
Cosmetic/Plastic Surgeon, Navi Mumbai
How To Take Care Of Flaky Skin?

When the natural moisture content of your skin begins to dry up, you experience dry skin. Although it is a temporary skin condition which occurs mainly during the winter or in a generally dry climate, for some people, it could be a problem for a lifetime.

Naturally dry skin tendencies v/s drying of other skin types
Naturally, the water content of a healthy skin is around 10 to 20 percent. This natural moisture level can dwindle due to many factors and the skin may show signs of drying. Thus you may end up getting dry, flaky, itchy and lifeless skin. People with naturally dry skin experience 75 percent more water loss than normal skin. Since the skin loses water and sebum (the oil secretion from the sebaceous glands within the skin), the topmost layer of the skin begins to dry. As a result, the top layer starts peeling away. These skin peels are called flakes.

Causes

  1. You can experience dry skin due to excessively cold weather, which in most cases is accompanied by a lack of humidity.
  2. Again, if your skin is exposed to too much heat or skin drying conditions like sauna baths or prolonged hot showers, you experience dry skin.
  3. Applying skin cleansing materials or chemicals can also make the skin dry.
  4. Some internal disorders like illness or infection can make your skin dry.

Treatment
When you have a perennially dry skin, you should moisturize it daily to keep it hydrated. You should consult a dermatologist for picking out the right skin cream for your skin. The appropriate moisturizer, if you have very dry skin should be oil or petroleum based. When you purchase the prescribed moisturizer or lotion, make sure you buy something which contains vitamin E, Shea Butter and Avocado Oil. Since these moisturizing agents aren’t water based , they can lock in the skin moisturizer effectively.

Advantages of moisturising regularly

  1. Dry skin is prone to wrinkle formation – When you moisturize daily, the collagen levels of your skin are restored, which can then effectively fight wrinkling and aging of your skin.
  2. Prevents flakiness – Since the upper layer of your skin starts peeling off, your skin may appear dull, lifeless and flaky. The condition worsens when there is a cold weather outside. Moisturizing your skin daily will make your skin get a natural glow and stop flaking form occurring in the first place.
  3. Reduces itchiness: Moreover, daily moisturizing will make your skin less itchy.
  4. Moisturising reduces the negative effects of makeup: Not moisturizing dry skin will make it look less hydrated. Applying makeup on your dry skin directly is the worst thing you can do. This may spoil your makeup and make it look creasy and broken. Thus, you should keep your skin hydrated by applying moisturiser regularly.

Things To Know About Breast Augmentation - Before Going For it!

Things To Know About Breast Augmentation - Before Going For it!

In today’s world, females have advance techniques available for shaping up their breasts. It is a part of their overall persona and beauty. Breast augmentation is one of the greatest plastic surgery operations that is done in hospitals across the globe.

Gone are the days, when you would sit back at home and blame nature for your saggy breasts. You have the gift of plastic surgery at your rescue. Breast augmentation can enhance the beauty of your breast and uplift its features. In this process, the implant is put under the pectoral muscle so that you can still breastfeed your infant if needed.
The process is executed with the help of a small 4 cm incision. The recovery process takes a week’s time. Breast augmentation can be carried out in a surgical centre or in any hospital’s outpatient facility. It doesn’t require a hospital stay in most cases. In some selected cases only, the procedure is done using local anaesthesia. In those cases, the patient stays awake. The breast region is made numb.

What to expect during the procedure?

The woman opting this must know what all are expected to happen during the procedure. The surgeon will insert the breast implant by making a single cut in one of the following three areas:

1. In the crease under breast
2. Underarm (axillary)
3. Around the nipple (periareolar)

After the incision is done, the surgeon will try and separate the breast tissue from the patient’s muscles and connective tissue. This will lead to the creation of a pocket either behind or in front of the pectoral muscle. After this, the surgeon will put the implant into the newly-created pocket. It will be centred behind the nipple.

Saline implants are put in an empty form and then filled with sterile salt water. Silicone implants are, however, pre-filled with silicone gel. After the implant is over, the cut is stitched with sutures and the skin is bandaged with adhesive and tape.

What to expect after the procedure?
For a few weeks, the patient might experience Soreness and swelling. It is also possible to develop bruises. Your scars will fade away with time. It is quite possible that the scars will not vanish permanently ever in your lifetime. You must wear a compression or sports bra at the time of healing. You might be required to take painkillers, as advised by the surgeon.  In case you have a concern or query you can always consult an expert & get answers to your questions!

Know More About Acne Scar Removal!

With the current self-obsession of the need to look good all the time, acne is seen as a major problem by the teenagers and adolescents. It is related to the hormonal changes but also aggravated by food habits. During their active phase, acne causes a lot of pain and burning. There is a constant tendency to pick at them to get rid of the pain and the itching.

They dry over a period of time and leave scars which are not as painful but are esthetically a major concern. For someone with oily skin who are more prone for developing acne, these scar marks could be very traumatic emotionally.

There is good news for these people as cosmetic advancements give multiple options for getting rid of these scars. Read on to know some of the most effective ones.

  1. Laser: A beam of light is aimed at the scar tissue, which does not affect the surrounding tissue but gets rid of the scar tissue. In addition to acne, laser is used in multiple skin ailments (whiteheads, cysts, blackheads, etc.) There is an intense bacterial presence in the acne, and when the laser beam is aimed at the acne, the bacteria are killed. This also reduces further incidence of acne and amount of sebum produced. The number of sittings required would depend on the severity of acne, and most people would require 2 to 3 sittings for complete removal of the scars.
  2. Photofacials: The outer layer of the skin (epidermis) is cooled and the inner layer (dermis) is heated. This stimulates the production of collagen and fibrous tissue, which replaces the acne tissue, giving a fresher, youthful look.

In both cases, remember to:

  1. Avoid use of irritants like perfumes and deodorants in the affected area
  2. Avoid waxing or shaving for at least 10 days before the procedure
  3. Avoid exposure to harsh sun for about a week prior to the procedure
  4. Avoid medicines which increase the risk of bleeding, aspirin for instance

Post treatment: In both treatments, the skin may appear red, swollen, and inflamed for the first day. There also may be a prickly sensation, similar to a sunburn. Cool packs or moist cloths and a good moisturizer can be used to soothe the skin. Avoid makeup until the skin is back to normal appearance. Depending on the type of skin and acne, the appearance will take about weeks to months after the procedure.

Benefits:

  1. Minimally invasive
  2. Reduced pain
  3. Completed in one appointment
  4. Reduced recovery time
  5. Can be clubbed with other acne therapy modes
  6. Good results with reduced recurrence and sebum production

With so many advancements, acne marks are sure to go away!!
