I am Dr. Zui Bagul. Today I will talk about the stroke. A stroke is a brain attack in which it causes damage to a part of the brain. This may be either blockage in the blood vessel in the brain or due to bleeding in a blood vessel causing damage. So, why do we need to know about stroke? This is very common and at least 3 people suffer every minute in India. And in your lifetime you may need someone who is suffering from stroke or may even be a family member who needs your help in this. So, your knowledge can help this person that may save his life or even reduce the disability. There are 2 types of stroke: Ischemic stroke & hemorrhagic stroke. Ischemic stroke causes blockage in a blood vessel due to this blood and oxygen is unable to reach that part of the brain. 2nd, hemorrhagic stroke in which due to some weakness in a blood vessel, it burst and blood leaks out and there is damage to that part. How do you recognize a stroke? Strokes are easy to recognize. In the first type, there will be a loss of power or sensation on one side of the body.

Suddenly, if someone is unable to move one hand or leg, that person may be suffering from a stroke. In 2nd type, there will be a difficulty in the speech. Suddenly, the person will be unable to speak or they may have a change in their speech. In 3rd type, a person may experience dizziness and loss of balance. They will be unable to walk. In 4th type, they may suffer from severe headache which they have not experienced before. If you are seeing that someone has suffered from this, then you should bring the person immediately to the hospital. The faster you bring, the better it is for Dr to save one life. What can you expect after bringing them to the hospital? Dr will immediately ask for a CT scan or other brain imaging. We will see whether there is bleeding or blockage. If there is a blockage and you have brought the patient within 4 hours, we have an option of giving an injection which will dissolve the clot and this may make the patient completely OK. If the patient has bleeding then Dr has to give some injections to reduce the blood pressure and also the patient may require surgery.

Other then this, the patient has to be hospitalized for a few days because the risk of complication is very high. And the patient will require at least 6 months to start the recovery. Within the first 3-6 months, recovery is maximum. After that recovery happens more slowly. During this period patient has to do a lot of exercises. At discharge, the patient will be given some medicines in the form of blood thinning agents or some cholesterol drugs. The patient has to take lifelong. Is there any way stroke can be prevented? Yes, there are 3 main reasons for stroke i.e. hypertension, diabetes, cholesterol, there are several other reasons. So, these 3 needs to be maintained by yourself. If you have blood pressure or hypertension, it is essential for you to keep a follow-up with your Dr at least 3 monthly so that your blood pressure can be monitored. You have to take your medications regularly.

Many people stop taking medicines of blood pressure but that is not correct. Blood pressure medications can be stopped only by your Dr. So, you have to continue taking them. You have to reduce the salt in your diet. You can avoid sprinkling salt on top of your food. You can avoid eating papad, pickles or other processed food which have high salt. Daily exercise will also reduce your blood pressure. You need to make some dietary modification. Eat a balanced and healthy diet in the form of small and frequent meals in diabetes. For controlling cholesterol, you need to avoid ghee, oil, fried products, processed food. Eat home-prepared food. I am Dr. Pritish Bagul, Cardiologist. Today I will talk about a heart attack. A heart attack is a condition in which the blood supply to the heart is stopped. The heart is a muscle and it is supplied by 3 different arteries. So, when they supply to these muscles is completely closed down. The muscle suffers from damage. It doesn't get its oxygen requirement and this condition is called a heart attack. It is a life-threatening condition. There are different types of a heart attack. What are the symptoms of a heart attack? The first symptom is chest pain in the majority of cases which is localized to the center of the chest and it can radiate to the left or right arm, towards the jawline. These symptoms are associated with sweating. 30% of the patients can get a heart attack without even chest pain and this is called a silent heart attack which we see commonly in diabetics or especially in females. Some patients can get shortness of breath. So, these are the symptoms of a heart attack. Some patients come with the unusual feeling of acidity. If you are feeling that it is not even stopping with the routine medicines, it could be a heart attack.

So, you have to be cautious and timely contact to your nearby Dr. How do we diagnose heart attack? First is the symptom. We do an investigation like ECG. Through this, we will come to know that which kind of heart attack is that. 2nd investigation can be done with an echocardiogram. We can see that how the heart is pumping it can be estimated. Next, in an emergency, we can do an angiography. We go towards the heart from hand or leg artery and we inject a die in coronary arteries to see through the camera. We see whether the artery is patent or not. in case, the artery is closed, we open the artery in an emergency and that is called angioplasty. So, angiography with angioplasty i.e. a primary angioplasty is the treatment of heart attack. In case, angioplasty is not available, the other option is thrombolysis in which we give blood-thinning medicine which dissolves the clot.

The blood flow is restored and the chest pain is down. So, these are the 2 main modalities of the treatment for the patient in a heart attack. We give medicines to control cholesterol, control sufar, critical care management is required in initial 4-5 days. Blood-thinning medicines are continued. So, this is about the disease and the treatment. How to prevent this problem? First and foremost thing is that you should screen yourself to check if you are at the high risk of developing a heart attack. If there father or mother has suffered from a heart attack then they should be more cautious. They should be aware of their risk profile. Those who have cholesterol abnormality should be more aware. So, getting yourself screened, is one of the most important things. Inculcate healthy eating habits like healthy diet which is less in fats. Include more vegetables, fruits. Junk foods are to be avoided. Control of your diabetes is important. Regular exercise is very important. Everyone should do for at least 45 mins of aerobic exercises. Avoid smoking, alcohol and any other addictions. In case, you are at risk, consult the nearest cardiologist. The patient who are diabetics, we advise them to be on single anti-platelet, cholesterol-lowering medicines. This is all about the term, treatment, and majors to prevent heart attack. I am Dr. Vinay Jaiswal, Cardiologist and today I will talk about coronary angiography and its need. Coronary itself means heart, angio means arteries and graphy means the study of these arteries which means the study of arteries with the supply of blood to the heart. So, how do we do this procedure? We do this with local anesthesia. In coronary angiography, we pass a hollow empty tube through the arteries of the hand and growing into the heart and inject a liquid under x-ray guidance and we estimate the amount of stenosis which we call it as a blockage in common men language. And we estimate how much percent is the blockage. These lacks are lipid-rich, cholesterol gets deposit over a period of time. This process is called atherosclerosis. So we estimate the amount of blockage happening into our arteries. and what kind of treatment will be needed is suggested upon completion of angiography. That means a patient needs angioplasty or a patient needs a coronary bypass grafting or a patient can be managed medically. So, all patients need coronary angiography.

The patients who have symptoms of breathlessness mainly on climbing elevated grounds, climbing stairs or walking on elevated grounds or feeling of chest heaviness while walking, pain or discomfort while walking. Any patient who is having pain in the chest at rest or any patient who is suffering from a heart attack may need an angiography. If a patient is having the symptoms of exertion, a primary evaluation will be done by taking a routine ECG. A 2D echo will be done and if it is normal, a cardiac stress testing is done. And if we get any evidence of possible blockages then coronary angiography is suggested. In case of a heart attack, direct angiography and angioplasty are offered to a patient.

This modality of the treatment is called the primary angioplasty in myocardial infection. Other patients like the patient are having rest angina, that is unstable angina, then we suggest coronary angiography. The patient is having a valvular problem like severely narrowed valves with the age of more than 40-50 years in males, also need to undergo coronary angiography before they undergo valves surgery. So, these are few indications of a coronary angiogram. It is an invasive procedure with very minimal risk of bleeding and very minimal risk of stone.
