Hello Dr, I am 22 year old male. I do swimming, jogging regularly. Apparently I've stopped all of my physical activities for the past 1 month because I've started getting lower back stiffness, to get rid of that, I need to break knuckles at that part regardless of anything like TIME, place etc. By doing this regularly I've started getting Lower back pain for the past few weeks. My mom is a known patient of osteoporosis, so urging myself I went to take DEXA for determining my bone mineral density, result was AP SPINE L1-L4 MEAN DENSITY OF 0.946gm/cm² corresponding to a t score of - 2.3.Dual femur mean density of 0.916gm/cm²,corresponding to a t score of - 1.3.I Consulted some physicians here, they are carelessly saying that I'm perfectly fine by considering my age, mean while other one is saying that I've not attained my peak bone density growth. I'm a medical student, having few knowledge I don't know what to believe and what not to believe. I'll be so thankful if someone tell me what exactly is happening in my body. And my body fat percentage is 37%that means I need to put myself in obese category, but my hip size is jus 34 inch and I've never felt I'm over weighted person. Please help me doctors.

Hi arun! I could see a healthy champ that you exercise regularly. Your age and healthy lifestyle won't let you down. No worries please. You just think about the activities that you have done before the exact pain started like heavy weight lift, jump from height, sudden fall. If your back pain is the same or severing from the time it started you may be repeatedly injuring the spot which is the source of pain. Please note the time you get severe pain in a day, how it gets reduced with position and time, what movements are possible and not possible in your back because pain, check for change in sleeping surface (like change in bed), sleeping position, amount of workload or work time you spend in a day. After noting all these aspects consult a qualified physiotherapist he/she may help in finding the cause and treatment.

Due to muscle spasm. Lumbar vertebrae appears normal in alignment .No listhesis. Small Schmorl's nodes noted involving the endplates of multiple lumbar vertebra. Degeneration ofL1-L2, L2-L3 and L4-L5 lumbar intervertebral disc Posterior annular tear noted at L1-L2, L2-L3 and L4-LSlevels. At Ll-L2 level there is right paramidline protrusion, No foramina narrowing. At L2-L3, L3-L4 diffuse posterior disc bulge noted ,causing effacement of ventral thecal sac and inferior recess of bilateral neural foramina narrowing at L3-L4 level. At L4-L5 level, posterocentral and left forminal protrusion causing, significant canal stenosis and left neural foramina narrowing ,resulting in compression of exiting L4 and traversing LS nerve roots at this level. At LS-Sllevel, no disc bulge or herniation. Conus medullaris appear unremarkable. Impression: - Lumbar spondylosis degenerative changes as mentioned ,predominant at L4-L5 level. - At L4-LSlevel, posterocentral and left forminal protrusion causing, significant canal stenosis and left neural foramina narrowing ,resulting in compression of exiting left L4 and traversing Left LSnerve roots at this level. TECHNIQUE: The :rv1RsItudy of the lumbosacral spine was obtained by sagittal Tl and T2 weighted images and axial Tl and T2 weighted images through T121L1 to LS/S1 disc spaces. FINDINGS: The MRI study of the lumbosacral spine disclosed evidence of degeneration of lumbar intervertebral disc spaces depicted by low signal intensity on T2 and reduced height at T121L1, Ll/2, L2/3 and L4/S disc spaces. There is mild dorsolumbar kyphosis at L1I2. The spinal canal is of normal satisfactory caliber. The conus is of normal shape and normal signal. The L 112disclosed posterior right paracentral broad based disc protrusion. The L2/3 disclosed posterior left paracentral broad based disc protrusion with focal high signal intensity consistent with focal tear in the annulus fibrosus at posterior left paracentral region. The L4/5 disc space disclosed posterior central broad based disc protrusion with left predominance causing significant compression on the anterior surface of the thecal sac at this level and associated with narrowing of lateral recesses bilaterally more pronounced on the left side with compromising of the exiting left L5 nerve root. CONCLUSION: The MRl examination of the lumbosacral spine disclosed degenerative changes of lumbar intervertebral disc spaces more pronounced on T 121L1, L 112 and L4/5 with mild dorsolumbar kyphosis at L 112 and posterior right paracentral broad based disc protrusion at L 112, posterior left paracentral broad based disc protrusion at L2/3 and posterior central broad based disc protrusion at L4/5. Sir you are having left Lowe limb radiating pain along with tingling and Numbness, you can't walk for long because of leg pain. This is all because of nerve compression by disc material. As per the reports compression is significant but I can comment with confidence after seeing the mri films. Two options are there in your case if pressure over nerve is moderate we can try pain block injection or if pressure over nerve is severe then surgery is the only choice left. But we can decide the treatment modality after seeing the mri films only. Just to inform you now a days technology is so advance that hardly it required more then three stitches after the surgery. No bed rest you can go home next day your own. We are using minimal invasive techniques like endoscopic and microscopic surgery for such type of cases with excellent results.

Sir my name is venkataramana I am suffering from knee pain it is paining very much when I am sitting or standing.
Hi thanks for your query and welcome to lybrate. Your knee pain could be due to certain reasons but common ones are chondromalacia, pain due to knee instability, inflammation etc. If there is no history of any injury, and no s/s of inflammation or infection like swelling, skin redness or local warmth on touching, then I will like to advise you for conservative management initially in form of: - intensive physical therapy with focus on strengthening of quadriceps and stretching of other group of muscles - gentle anti inflammatory medications like tab etoshine or tab paracetamol for 5-7 days usually helps in such cases - adequate calcium and vitamin d supplementation as per requirement - ice packs at home for 3-4 times in a day and see how it goes for around 2 weeks time. If your condition does not improve, then we will have to get certain investigations done. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail

My leg pains every a day sometime feels like put in very uncomfortable when sleep or fold my legs.
: keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

Dear, Frozen shoulder gives acute pain symptoms at night. There's no medicine for improving the shoulder however pain killers may give temporary relief but that will keep the shoulder frozen only. I would recommend her to take physiotherapy especially shoulder mobilisation exercises which a therapist only can perform and it can take from 3 months to 6 months of physiotherapy for the condition to be fully cured (depends on her shoulder condition and medical history) For other joints please get some blood tests done (like rheumatoid factor, Utica acid, ESR and CRP etc) to check out the kind of arthritis and for that you need to consult an orthopaedic.
