At now I am over weight. 94 kgs, 172 cm height at age 24. Give me some solution for reduce weight without any side ef...
1. Aim for long term weight loss @ 2-3 Kg per month = 12-15 kgs in 6 months. So on. 2. Add 2-3 Fruits to your daily diet before meals. Double your water intake. 3. Cut down Sweets/Biscuits/ Colas/Juices from your diet. 4. Completely eliminate Puris/Parathas/Pizzas/Burgers/Fried Rice/Biryani etc 5. Eat slowly and mindfully. Reduce portion size. 6. Exercise daily - 'Intense' Cardio for 40-60 mins. This will keep you toned and trim. 7. Love yourself and respect your body. 8. For a customised plan, you can consult me privately.

Signs and Symptoms You Might be Suffering From Lichen Planus

The skin is the most exposed organ to irritants, and when the body recognizes something as foreign, the immune system kicks into action. It produces what is known as antibodies against the foreign bodies (antigens). However, in many situations, the body produces antibodies against its own cells or tissue or components, causing an autoimmune reaction. One such autoimmune reaction is lichen planus, which is a skin rash caused when the body produces antibodies against its own skin or mucous membranes.

Risk factors: While anyone with a weak immune system can develop lichen planus, the followings increase the chances of that happening:

  1. Presence of other autoimmune disorders
  2. Hyperactive immune system, causing the allergic reaction
  3. Genetic inheritance
  4. Increased stress levels
  5. Prior history of viral infections
  6. Middle aged women
  7. Exposure to allergens like gold, arsenic, iodine, and drugs like diuretics and antibiotics
  8. Prior history of hepatitis C

Symptoms: Diagnosis of lichen planus is quite easy, as it has a characteristic appearance.

  1. Basically, a skin rash that is purple in color with flat tops on the skin
  2. Rashes spread over the body in a matter of weeks
  3. They are itchy, painful, and produce a burning sensation
  4. Could have blisters which burst
  5. Have thin lacy margin
  6. Can be seen in the genital area, scalp, ankle, hands, mucous membranes, and nails

If required, a biopsy can be done in some cases to confirm the diagnosis. Allergy testing can also be done to confirm hyperactive immune system.

Treatments: This can depend on the severity of the condition. In people where it is not progressive or inflamed, it could be observed to run its natural course and subside.

For those requiring symptomatic treatment, the following can be used:

  1. Steroids (topical or oral) to reduce inflammation
  2. Antihistamines to reduce the allergic response
  3. Retinoids (topical or oral) which can help overall skin health
  4. Nonsteroidal creams to clear up the rash
  5. Moisturizer to keep skin healthy and prevent dryness and itching
  6. Cool compresses on the rashes
  7. Loose clothing to prevent irritation and itching
  8. Anti-itch creams and powders and lotions
  9. Oatmeal bath to avoid itching and inflammation
  10. Ultraviolet radiation to the rash to reduce severity
  11. Avoid agents (drugs or chemicals) which can cause lichen planus
  12. If the lesions are in the mouth, mouthwash and/or rinse can be used for topical relief

Lichen planus often does not require any treatment. However, depending on each individual, it may require topical and/or systemic treatment to manage the symptoms. It usually runs its course over 6 to 10 weeks and subsides on its own.

Wrinkles - 5 Options Your Dermatologist May Recommend!

Wrinkles are inevitable with advancing age and most individuals find them annoying. Luckily, there are many available medical options to reduce these pesky lines and look younger. Your dermatologist can help you with a variety of gentle options like:

  1. Retinoid creams: These creams are derivatives of Vitamin A and cause superficial peeling off the skin, reduce rough bumps and lighten superficial pigmentation. These are prescription medicines and have to be used carefully under proper medical supervision to avoid side effects like itching and burning reactions.
  2. Peels: This a procedure in which your physician applies a thin layer of medicine on top of the skin for a few minutes and then washes it off. Over the next few days, a very superficial layer of the topmost skin layer is then gradually shed off. The regenerating skin from below is fresher, smoother and younger.
  3. Lasers and other rejuvenating devices: Different kinds of lasers can be used to target different layers of the skin. Non-ablative lasers gently heat deeper parts of the skin to tighten collagen to give a smoothening effect. Ablative lasers are more aggressive and remove superficial layers of skin with great precision. Radio-frequency and ultrasound machines use other forms of energy to gently heat deeper parts of the skin to achieve a tightening and smoothening effect
  4. Botulinum toxin: This is injected into frown lines and crows lines to relax the muscles causing the wrinkles. The effect typically lasts for about 6 months.
  5. FillersThese are substances which are injected into the deeper wrinkles to pump up the furrows. The commonest substance used is called hyaluronic acid and is the same substance that exists naturally below the skin.

And of course, daily precautions like using sunscreens, moisturizers, maintaining a healthy lifestyle and avoiding smoking goes a long way in preventing wrinkles.

Do's And Don'ts Of A Hair Transplant Surgery!

Are you planning on undergoing hair transplantation because of extensive hair loss? A hair transplantation surgery is effective in making up your lost hair, but there are several facts you should know before undergoing the procedure. There are certain strict do’s and don’ts associated with hair transplantation, both before and after the process.

Before hair transplantation
Do's

  • Wash your hair well before the surgery day on the morning of the surgery.
  • Wear clothes which do not need to be pulled off over your head after the surgery.
  • Arrange for transportation for going back home.

Don’ts

  • Do not take aspirin or other anti-inflammatory medicines for seven days before the procedure.
  • Do not consume alcohol, take vitamin B, E supplements for seven days before the surgery.
  • Do not smoke for three days before, and on the day of the surgery.

After hair transplantation

Do’s

  • Remember all instructions given by the surgeon for care after the procedure. Although there are general guides for care, you should listen to your surgeon and his guidelines.
  • Try keeping your scalp dry on the first day after transplantation, which enables your skin to heal.
  • Keep your scalp clean during the recovery process. Soreness and numbness may be felt, and you should not put anything on your scalp that may cause side effects.
  • Take proper care of your scar. Scarring and itching are likely and you should deal with these using topical creams prescribed by your surgeon.
  • You may take painkiller medications to ease discomfort and pain, three to four hours after the procedure.

Don’ts

  • Avoid exposure to direct sunlight immediately after the hair transplantation, and also for the next few days.
  • Avoid touching or fidgeting with your grafted scalp. Picking and rubbing your sore scalp may cause the new grafts to fall out before getting the chance to grow.
  • Do not take alcohol for the first five days after the hair transplantation procedure.
  • Abstain from smoking for one month after the procedure as it disrupts the flow of blood to your hair follicles.
  • Do not involve yourself in any form of sports activity or exercise right after the procedure. You may resume working out after a period of ten days, and you must wait for one month to get back to other physical activities and sports.
  • Avoid engaging in activities which result in intense sweating. Sweating enhances the risk of infections to your grafted scalp. You should also avoid steam baths or sauna treatments for one month after the procedure.

In case you are a diabetic, you need to talk to your surgeon about ways of handling your diet and insulin management, before and on the day of the hair transplantation procedure.

Genital Herpes - Know About This Sexually Transmitted Disease!

Genital Herpes - Know About This Sexually Transmitted Disease!

Genital herpes is a sexually transmitted disease. One out of every six people in the United States suffers from this disease. It is more common than you think. Like any other STD, it is extremely important to take proper protection (use a condom) while engaging in sexual activities.

Genital Herpes is caused by the herpes simplex virus (HSV). There are two types of herpes simplex virus.

  1. HSV-1 is most commonly associated with blisters and ulcers around the mouth known as cold sores.
  2. HSV-2 is associated with blistering lesions in genital areas that are exposed during sexual contact.

However, both types of herpes simplex virus can infect the mouth or the genital areas, meaning that genital contact with a cold sore on the mouth can lead to genital herpes. Likewise, kissing someone with a cold sore can spread the herpes simplex virus infection.

After the initial outbreak of herpes, the virus travels through the nerves and resides in nerve tissue within the body.

Most people with genital herpes don’t know they have it. That’s because in most people it produces either no symptoms or very mild ones.

Symptoms of Genital Herpes:

  1. The area around your genitalia becomes red and cracked without any pain or itching.
  2. Itching around your anal region
  3. Blisters around your genitalia can cause immense pains when they break open.
  4. Pain in the open sores while urination.
  5. Headaches and backaches.
  6. Flu and swollen lymph nodes are very common.
  7. Fatigue is also felt if you suffer from genital herpes.

Symptoms like these can be mistaken for other infections such as vaginal yeast infections, bacterial infections and bladder infections. So, if you are suffering from such symptoms it is advisable to go in for a health check up followed by a physical exam and a blood test to confirm whether you have genital herpes or not.

How can it get spread?

You can only get Genital Herpes from vaginal intercourse and not anal or oral intercourse. The fluid present in the sore of a genital herpes patient has infected fluid. If the fluid comes in contact with their partner’s sexual organs while having intercourse they can be easily infected.

Can Symptoms Be Treated?

There is no cure for genital herpes. But the symptoms can be lessened and prevented with treatment. Treatment can also reduce the risk of infecting others.

Your healthcare provider may prescribe antiviral medications to help prevent or reduce the pain and discomfort from an outbreak of symptoms. Medication taken on a daily basis to suppress the virus can reduce the number of outbreaks and reduce the risk of infecting others

How to Avoid Getting Genital Herpes:

  1. Try to have sex with people who have been tested negative for herpes or any other STDs.
  2. Use a latex condom while having sex. It prevents any sort of liquid to pass inside your partner’s genitalia.

Intracytoplasmic Morphologically Selected Sperm Injection (IMSI) - Live Your Dream!

Intracytoplasmic Morphologically Selected Sperm Injection (IMSI) - Live Your Dream!

Of a million sperms released in one ejaculation, a single sperm manages to fertilize the egg, which grows into an embryo and then a baby. Due to various reasons, when this does not happen, it is termed as infertility and is currently on the rise.

With IVF, couples sometimes go through multiple sessions to ensure fertilization. ICSI or intracytoplasmic sperm injection improved this chance of fertilization. It is where a single sperm is used to fertilize an egg in an artificially controlled environment and then injected into the uterus where it grows further.

The next step in this technique is IMSI, where the chances of successful pregnancy are further enhanced and also promises improved quality of the embryo. IMSI, as it stands for, has “morphologically selected” which means under a highly powerful microscope, the best sperms are selected and then used to fertilize the egg. Morphology stands for shape, and the shape of the sperm is a direct indicator of the sperm quality. This not just ensures good success rate but also ensures the sperm which is used produces a high-quality embryo.

Features

  1. High success rate of fertility
  2. Reliable and efficient method of ART (assisted reproductive technology)
  3. More expensive than traditional IVF methods or ICSI
  4. Requires complex equipment, training, and set-up

Indications:

  1. Useful in couples who have failed IVF previously
  2. Male infertility with unidentifiable cause
  3. Poor sperm quality

What to expect?

Both partners are prepared both physically and mentally prior to IMSI.

Before the procedure - men:

  1. A screening is first done to decide if the sperms can be used directly
  2. In case of hereditary diseases, a donor is preferred
  3. The first technical step involves sperm collection, either directly or through a donor
  4. Sperms are collected either through masturbation or through a small incision in the testicles
  5. Sperms used could be fresh or frozen; fresh ones may be stored for later use also

Before the procedure – women:

  1. The woman is put on some ovulation therapy to ensure the release of multiple eggs, so a good one is picked. These high doses of hormones ensure multiple eggs are released.
  2. Egg release is monitored through blood and urine tests
  3. They are collected within after 36 hours of release

During the IMSI procedure:

  1. A healthy egg is chosen and placed in a glass container containing hyaluronidase
  2. Sperms are placed in a medium which will slow their movement, which enables picking a good sperm for fertilization
  3. The dual advantage of this is that in addition to a high success rate of fertilization, the quality of the embryo is also assured

Thinning Of Hair: Should You Get Your Hormones Checked?

MBBS, MD - General Medicine, Certification In Diabetes
Endocrinologist, Delhi
Thinning Of Hair: Should You Get Your Hormones Checked?

Thinning hair, noticeable bald spots, large clumps of hair in the hairbrush or shower drain are indications of hair loss. This loss of hair is an effect of hormonal imbalance.

Have You Seen these Signs of Hair Loss?

  • A widening part of the scalp
  • A ponytail becomes noticeably thinner
  • Large clumps of hair in the hairbrush or shower drain and on your pillow after a night’s sleep

Why does it happen?

  1. Play of Hormones: Hormonal imbalance is seen in women during menopause, perimenopause, pregnancy, and in endocrine disorders, such as polycystic ovarian syndrome (PCOS). Progesterone and estrogen are two important female hormones. Progesterone keeps a check on estrogen levels. When progesterone levels are too low, it leads to a state of hyperestrogenism which triggers excessive hair shedding and hair loss. Dihydrotestosterone (DHT) is a potent form of testosterone that normally leads to hair loss. Women, who produce high levels of male hormones (androgens), have increased testosterone levels which can convert to DHT, leading to hair loss.
  2. Stress: When a woman experiences any stressful condition, her adrenal glands increase the release of cortisol (stress hormone). This leads to secretion of adrenaline which leads to increased levels of testosterone and DHT.
  3. Under or over performing thyroid: An overactive thyroid and lack of sufficient parathyroid hormone can also result in thinning of hair. Accompanying symptoms are fatigue, unexplained weight gain, aching joints, constipation, dry skin, feeling cold all the time, and poor sleep.
  4. Pregnancy and childbirth: Pregnancy and childbirth change a woman’s hormonal balance and can result in hair loss. Therefore, it is normal for postpartum mothers to experience thinning of the hair and even bald patches. Such hair loss is temporary and heals itself within several months.
  5. Menopause: Menopause also leads to hair loss. During menopause, a woman’s estrogen levels fall and are no longer in balance with DHT levels. This low estrogen level and high DHT lead to hair loss.
  6. Nutrient deficiencies: Low levels of certain nutrients within the body, for example, vitamins A, E, and iron may result in hair loss.

How Can You Help Yourself?

  1. Consume omega 3-fatty acids: Omega-3 and omega-6 fatty acids are kinds of essential fatty acids (EFAs). They increase hair growth. Sources are walnuts, flax seeds, cold-water fish, such as salmon, sardines, herring, and mackerel.
  2. Get plenty of sleep: Lack of sleep causes increased stress levels, higher production of cortisol, and ultimately hair loss. Good amount of sleep will lead to harmonious functioning of the body.
  3. Take dietary and hair supplements to prevent hair loss: Certain supplements can help minimize the production of DHT, prevent DHT from binding to hair follicles, or stimulate inactive follicles to encourage hair growth.
