Dental Implants or Dentures - What is Best Suitable for You?

Tooth loss occurs due to various reasons decay and infection, fracture, accidents, gum disease, to name a few. Tooth loss has a negative impact on various aspects facial appearance, chewing efficiency, and overall nutrition. The attempts to restore the lost tooth have led to various advancements from removable dentures to cast partial dentures to fixed dentures to implants.

Listed below are features (covering advantages and disadvantages) of both dentures (fixed and removable) and implants. While dentures can be complete or partial, implants are more often partial, of course can be used to replace multiple teeth. A detailed discussion with your dentist along with oral and radiographic examination will help in making a decision.

Removable Dentures:

Advantages:

  1. Replaces tooth but more for cosmetic reasons than function.

  2. Poor chewing efficiency.

  3. Useful when multiple teeth are to be replaced, especially when no adjacent teeth are available for support.

  4. If good bone support is not available, retention is an issue.

  5. Not very healthy for the underlying jaw bone and adjacent gums.

  6. Requires daily and routine maintenance.

Fixed Denture(s):

Also called as bridges, as the artificial replaced tooth is a bridge between two natural teeth.

  1. Requires removal of sound tooth structure as abutment (support). This is the major disadvantage with fixed dentures, which promoted further work from the dentists, ending up with discovery of implants.

  2. Promotes tooth decay in the adjacent teeth.

  3. Good support, so retention is not an issue.

  4. Not as harmful to the underlying jaw bone as removable denture.

  5. Requires maintenance but not like the removable denture.

  6. Expensive compared to removable dentures.

  7. Replacing a bridge also means the adjacent healthy teeth are affected.

Implants:

  1. Very good replacement alternative, replaces not just the crown structure but also the root structure.

  2. An excellent option when the jaw bones and gums are extremely in good condition.

  3. Almost natural restoration of both chewing efficiency and facial appearance, the person can eat almost anything.

  4. Not as harmful on the gums like dentures.

  5. Does not cause bone loss like dentures.

  6. Can be used to replace single or multiple teeth, where multiple implants are placed and a bridge is placed over them.

  7. The titanium screws that are used fuse with the jaw bone over a period of time.

  8. The titanium is also biocompatible, thereby reducing chances of allergy.

  9. Implants are expensive, and many insurance companies may not cover them.

  10. If properly fabricated and maintained well, then it can last for decades.

The decision to go for a denture or for an implant should be decided based on a detailed evaluation of overall health and if required radiographic examination. Both dentures and implants have advantages and disadvantages, and the dentist will help make the right choice. If you wish to discuss about any specific dental problem, you can consult a specilized dentist and ask a free question.

I am 25 years old have a sensitive tooth for the last two months. I have a slightly yellow coloured tooth after an injury in my childhood. In these many years now it is responding to hot or cold items. Please give me suggestions.

BHMS
Homeopath, Faridabad
Hello. Tooth sensitivity — also known as dentin hypersensitivity — affects the tooth or exposed root surfaces. This occurs when the enamel that protects our teeth gets thinner, or when gum recession occurs, exposing the underlying surface, the dentin, thus, reducing the protection the enamel and gums provide to the tooth and root. If hot, cold, sweet or very acidic foods and drinks, or breathing in cold air, makes your teeth or a tooth sensitive or painful then you may have sensitive teeth. Tooth sensitivity can come and go over time. Causes: There are many causes of tooth sensitivity, including: Worn tooth enamel from using a hard toothbrush or brushing too aggressively Tooth erosion due to highly acidic foods and beverages Tooth erosion due to bulimia or gastroesophageal reflux disease (GERD) Gum recession that leaves your root surface exposed There are several conditions which can cause pain, but which are not tooth sensitivity: Dental caries A cracked or chipped tooth Grinding or clenching the teeth Leakage around restorations External tooth bleaching Management: Proper oral hygiene is the key to preventing gums from receding and causing sensitive-tooth pain. If you brush your teeth incorrectly, or over-brush, your gums may become sensitive. Brushing properly twice daily for 2 minutes with toothpaste that does not have high levels of abrasives, and flossing once a day, can help reduce the chance of tooth sensitivity. A diet low in acidic foods (tomatoes, pickles, tea, citrus fruits) and drinks also helps prevent tooth sensitivity. As the blood supply to the injured tooth gets affected which causes change in colour and thickness of the tooth. Treatment: Apply Plantago Q/ few drops on cotton swab, place it on the affected teeth and press it tightly, do it thrice a day. Orally, take Mag. Phos. 6x and Arnica 200 - both 3 times a day for a month. Do visit a dentist for proper diagnosis, share the reports with me. In case possible, will help you in treating your problem.

My father have mouth ulcer from one year the size of mouth ulcer increasing slowly Now the size is 5.5 ×4.4 cm please give suggestion how to cure?

MDS - Oral & Maxillofacial Surgery
Dentist, Chennai
If the mouth ulcer is causing a lot of pain, gently rub a small ice cube over the spot or rinse your mouth with ice-cold water. You could also chew on a clove bud (lavang) and direct the juice generated to the area where the ulcer is located. Clove buds are a great ingredient to cure sore throat too. To prevent the ulcerated area from getting infected, make it a point to rinse your mouth with a little salt water. Remember, this won’t make the ulcer disappear, but it can help reduce the pain. Honey is a natural humectant. It helps reduce scarring and hastens the process of new tissue growth. This, along with its anti-microbial effect ensures faster healing of the mouth ulcer. Chew a few leaves of tulsi along with some water about three to four times every day. This will help in getting rid of ulcers faster and also have a preventive action against their recurrence. Applying a little coconut oil directly onto the mouth ulcer or chewing some coconut (fresh or dry) helps reduce pain and inflammation and hence, ulcers heal faster. Mix some sugar to crushed khus-khus seeds and consume this mixture for instant relief from ulcers. You could apply a little ghee directly to the ulcerated area, or drink a glass of buttermilk two or three times every day. Mouth ulcers left untreated generally go away in a week or so; with these home remedies, you can expect faster relief. However, if you notice an ulcer in the mouth that is persistently present although it does not cause you any pain, do not neglect it or try to treat it with home remedies beyond a few days. Make it a point to see a doctor soon because often, a painless mouth ulcer is one of the earliest signs of oral cancer.

I have mouth ulcers and it happened very frequently. I have taken folicacid becasole but no work. Advise what I do?

MDS - Oral Medicine and Radiology, BDS
Dentist, Ahmedabad
Hi, folic acid and b complex will help only if vitamin deficiency is the cause. There are other causes of mouth ulcers like stress, constipation, trauma, food allergies, etc. You should visit an oral medicine specialist for complete management. Meanwhile, apply kenacort 0.1% oral gel on the ulcers for relief in burning and pain.

I am a 22 years male. I have problem of producing excess saliva. during night I also suffer from blockage in my throat. I am having medicine for stomach problem. No smoking, no drinking. I have used pan masala rarely.

MDS - Oral & Maxillofacial Surgery
Dentist, Chennai
Although excessive saliva is not too common, there are a number of conditions that can cause it - some harmless, others more serious. It is possible that your saliva problems are just a result of the way you speak. Some people speak faster or slower than others, just as some people blink more often, breathe faster, or have a more rapid heartbeat. This may just be what's normal for you. Since it is negatively affecting your life, I suggest your problem can be ignored. However there are medical conditions that causes excessive salivation injection glycopyrrolate reduces secretions in the mouth, throat, airway, and stomach.

Eating Habits
Proper nutrition is necessary for healthy teeth and gums. Eating a well-balanced diet gives gum tissues and teeth the important nutrients and minerals needed to stay strong and resist infections. Soft, sticky foods tend to remain on the grooves and between teeth. Hence thorough rinsing after every meal is necessary.
