What Should You Know About Diabetic Macular Oedema

Cornea Cataract & Lasik
Ophthalmologist, Delhi
What Should You Know About Diabetic Macular Oedema

The diabetic eye is considered to be the leading cause of the blindness among the working age. It is usually caused by the change in the blood vessels on the retina of the eye. Retina is that layer of the eye which is light sensitive. In case of diabetic macular oedema, the blood vessels cause the fluid to leak into the retina.

How does diabetic macular oedema cause vision loss:
The loss of vision only occurs when the fluid reaches the macula. Macula is the center of the retina which is responsible for a sharp vision. When the fluid reaches the macula it builds up and causes inflammation. Initially this is not noticed but with time the diabetic macular oedema causes a change in the vision making it more blurred. A healthy macula is very essential for a good vision.

Who is at risk of diabetic macular oedema:
People with type 1 and type 2 diabetes are at a higher risk of getting diabetic macular oedema. Also other risk factors are:

  1. About 0ne in three people with diabetes develop macular oedema
  2. Poor control of blood sugars
  3. High blood pressure
  4. High cholesterol level
  5. Pregnancy
  6. In smokers

How to reduce the risks of diabetic macula oedema:
The risk of diabetic macula oedema can be reduced by quitting smoking, and to make sure that blood sugar and cholesterol levels are under control. This is achieved by regularly measuring the cholesterol and blood sugar levels.

How is diabetic macular oedema detected:
Diabetic macular oedema can be detected during regular visits to the doctor. Patients with diabetes should be offered screening tests. Digital photographs of the patients can be taken as they show the early signs of diabetic macular oedema, though changes in vision might not be noticed at this time.

What happens when you attend the medical retina clinic:
When you go for an eye checkup you will undergo a comprehensive examination which includes:

  1. Visual acuity test: This is a sight test which measures how well you can see the different distances
  2. Eye pressure test: This test is done to measure the pressure of the eyes and usually drops which numb the eyes are used when this test is conducted
  3. Dilated eye examination: In this the drops are placed in the eye to dilate the pupils and then the back of the eye is examined.
  4. Fluorescein angiography: This is a diagnostic test in which an injection of fluorescein dye is given in the hand and then the photographs are taken.
  5. Optical coherence tomography: This is done to measure the retinal swelling

Sir my specs power are -9 and -10 I want to select contact lens what cost it would take?
Depends on company to company and whether you want monthly disposable or daily disposable or yearly disposable. Average monthly disposable would be around 300 per lens per eye. But again highly variable.

I have allergies, and my eyes are affected the most. They, re puffy, red and itchy. Why can do?
Allergy can be treated easily using eye drops. But there are many eye drops and specific eye drops can only be prescribed after seeing the eye clinically as treatment depends on severity. Also dip a handkerchief in little cold water and keep over closed eyes. You will feel better.

Blueberries, grapes as they have antioxidant and anti-inflammatory properties that can help improve your vision. Dha is a fatty acid found in coldwater fish like salmon, sardines and mackerel. Exercises for strengthening eye muscles: -begin by partially closing your eyes; the lids should only be halfway shut. After holding for 10-15 seconds, close your eyes very slowly. Take a few deep breaths to increase oxygen in the blood. -warm your eyes. Rub your palms together to create heat, and then place them against your eyes for five seconds. Repeat this three times. -hold a pen at arm's length. Focus your gaze on the tip of the pen. Bring the pen closer to your nose at a slow and steady pace. Repeat this exercise 5-10 times. -concentrate your vision on the tip of your nose. Then shift your gaze to an object further away, either at arm's length or 20 feet away. Then shift your eyes back to the tip of your nose. Repeat this exercise 10 times. -sit in a relaxed but upright position. Begin by look to the left-most position (without moving your head) and hold to stretch your eye muscles. Hold for 5 counts. Blink and return your gaze back to look straight in front of you. Then look to the right-most position with your gaze and hold for 5 counts. Repeat 3 times, blinking between each hold. Then, repeat this exercise looking up and then down. Do not forget about blinking. -sit in a relaxed but upright position. Look up and to your right for 5 seconds. Return your gaze to center. Look up to and to your left for 5 seconds. Return your gaze to center. Look down and to your right for 5 seconds. Return your gaze to center. Look down and to your left for 5 seconds. Insufficient sleep may weaken your vision, so shoot for 8 hours of sound sleep a night. Rest your eyes 10 minutes for every 50 minutes spent reading or in front of the computer. If your eyes feel overly tired, lie down and place cooling cucumber slices over your eyelids. Medication: use homoeopathic eye drops - euphrasia eye drops twice a day. Thereafter, revert back for further guidance.

Hi. Pinkeye (also called conjunctivitis) is redness and swelling of the conjunctiva, the mucous membrane that lines the eyelid and eye surface. The lining of the eye is usually clear. If irritation or infection occurs, the lining becomes red and swollen. Pinkeye is very common. It usually is not serious and goes away in 7 to 10 days without medical treatment. Most cases of pinkeye are caused by: Infections caused by viruses or bacteria. Dry eyes from lack of tears or exposure to wind and sun. Chemicals, fumes, or smoke (chemical conjunctivitis). Allergies. Viral and bacterial pinkeye are contagious and spread very easily. Poor hand-washing is the main cause of the spread of pinkeye. Sharing an object, such as a washcloth or towel, with a person who has pinkeye can spread the infection. Symptoms of viral pinkeye include: Redness in the white of the eye. Swelling of the eyelids. Itching or burning feeling of the eyelids. Swollen and tender areas in front of the ears. A lot of tearing. Clear or slightly thick, whitish drainage. Management: - Try to rest your eyes if redness is due to fatigue or eye strain. No other treatment is needed. - Wash your hands, hold clean water in your hand, keep your eyes closer to the water and dip the eyes by blinking the eyelids frequently in the held water in hand. Repeat it for 3-4 times again. - Avoid watching T. V. Working on computer or mobile, reading or frequently keep blinking your eyes to avoid dryness. Take frequent breaks in between. - Maintain hygiene, use a clean napkin or a tissue to wipe the tears and cleaning the eyelids gently. -Wear an UV-rays protected sunglasses while you are out in sun. Medication: Take homoeopathic medicines - Schwabe’s Belladonna 30 and Euphrasia 30 - both thrice daily for 4 days. Use Euphrasia Eye drops twice daily. Revert back thereafter.

Hye. Thankyou for the query. It's possible you are not eating as per the body's needs. Skipping meals, eating more of junk foods, not drinking enough water- coupled with stress and work overload can lead to acidity and headaches. You must first work on the reason for headache. Eat regular meals, stay hydrated, avoid excessive tea or coffee and avoid smoking. If all this still does not help check your eyesight and check for migraine.

I have red eye for 3 days what should I do. Please help me doctor. Also I am not able to sleep.
Eye redness is most often due to swollen or dilated blood vessels. This makes the surface of the eye look red. There are many causes of a red eye or eyes like infection, allergy. Foreign body in eyes. Some are medical emergencies and some are a cause for concern, but not an emergency. Others are nothing to worry about. Consult your local eye specilist to see what is the cause.

Having cyl no. +sph no. Of +3+1.25 on both eyes giving 6/6( R) & 6/9 (L) vision I face difficulty even with glasses, should I go for Lasik at this age of 44 yrs so that at least my dist vision is corrected properly ?

Phaco Development Program, Fellowship in Pediatric Ophthalmology, DNB Ophtalmology, MBBS
Ophthalmologist, Mysore
I would not recommend LASIK for the following reasons: 1. LASIK for plus powers is not very predictable. 2. Age: there is more chances of residual power or power changing even after LASIK. It is better that once you start developing cataract go in for early cataract surgery rather than go for LASIK now.
