I m 23 years old Male. My problem is that whenever I go to toilet, I need at least 20-25 minutes. I m totally frustrated with this. I saw other people they need only 5-10 minutes, but in my case I don't feel comfortable in 5-10 minutes, I need almost half an hour. Please suggest me, what I should do now?

You need to change your lifestyle. Engage yourself in some high physical activities. Increase the intake of fibre rich foods, especially soluble fibre like banana, papaya, pineapple, guava, apple, kiwi, mango at least 4-5 servings in a day. Always drink hot water. Also consume sugarcane juice regularly.

Hii doctor my dad aged 59 is having bloating problem on right of the chest from almost 2 years. Last year we went to gastro neurologist he did colonoscopy nd sigmoidoscopy he concluded saying that he had gas trouble everything is fine. After then again he used to get bloating rarely, from past week he is getting bloating along with spasm or cramp (muscle contraction) on right side of chest for sometime and only having relief when having soda I don't know the exact term for that muscle contraction but he don't have any pain but only contraction sensation from last week this is the second time to get these contractions is it life threatening or any indication of cardiac arrest. Or is it because of seasonal change (winter in India) or due to gas please clarify.

Since his problem was confined to upper abdomen and chest, he should have undergone gastroscopy rather than colonoscopy or sigmoidoscopy which deal with the bowels and lower abdomen. He must be suffering from gastritis and esophagitis-inflammation of stomach and lower part of esophagus as a result of acidity (acid peptic disease). It is also advisable to get a sonography of abdomen done to rule out gall stones which can causes similar symptoms. As for heart problem, get his ECG done as it is possible any other problem may be co-existing and has to be ruled out.

There is no evidence to show that sex is unsafe in pregnancy as long as there are no issues with vaginal bleeding, ruptured membranes or low lying placenta. As the pregnancy advances there are mechanical issues that may make it practically difficult.

Hello- Jyada sexual activities mai sammilit hone k karan shukra ka bal (strength) kam ho jati hai jisse genital system weak ho jata hai, isse urine ka infection bar bar ho jata hai isiliye apke penis mai jalan aur urine pila ata hai. Try to avoid close contact with sick people. Cover your nose and mouth with a tissue when you cough or sneeze. Throw the tissue in the trash after you use it. Wash your hands often with soap and water. If soap and water are not available, use an alcohol-based hand rub. Avoid touching your eyes, nose and mouth. Germs spread this way. Clean and disinfect surfaces and objects that may be contaminated with germs like the flu.

Foreskin of penis will never interfere with your being infertile. However if the mouth/opening of the penis is totally block due to the foreskin then get it circumcised. Moreover if you are having Low Sperm count - this will interfere with your being infertile. Get it treated fast.

Keep your Skin Healthy and Glowing this Winter

To overcome the conditions of dryness caused by the solidification of skin's oily content during winter, there are some ways by which you can rejuvenate your skin by adding to its softness and suppleness besides boosting its collagen content. 1) To do away with the conditions of hardness harnessed during winter, consumption of juicy fruits and vegetable with the particular inclusion of olive oil and primrose juice will prove to be a healthy alternative. 2) In order to get rid of external dryness and chaps, application of coconut oil prior to bathing can be another helpful option. As far as application of oil is concerned, it is better to opt for ones which do not clog the dermal pores. Non clogging oils such as avocado oil, primrose oil and almond oil should be preferred over those causing to block skin pores. 3) Application of petroleum jelly on areas particularly affected with chaps and dryness happens to be one of the most effective options. 4) Instead of using soap high in their alkaline content, it is better to have your skin washed with paste of ground gram and yogurt. In case you prefer using soap, make sure to use one with adequate moisturizing content. Soaps based on natural oils and glycerin may turn out to be a healthy alternative. 5) Avoid using hot water for bathing, instead go for lukewarm water enriched with drops of glycerin or herbal oil such as tea tree oil to make up for the loss of moisture. 6) Skin may be similarly hydrated with cold creams and moisturizer oily in content. In place of water based moisturizers opted for during summer, go for the oily ones. 7) Moisturizing lotions based on glycerin and alpha-hydroxyl content can contribute to the smoothening feel of the skin. 8) Skin toner containing essential oils or the rejuvenating elements of vitamin E can prove to be similarly effective. However, if the facial skin is naturally oily, avoid using the same on face. 9) While going for manicure and pedicure, make sure to have your limbs soaked in lukewarm water enriched with drops of natural oil. 10) To prevent brittleness in nails, make sure to pamper your finger and toe nails with sufficient touches of oil, glycerin or moisturizers. 11) In order to overcome flaking, chapping and itchy cracks caused during winter, use cotton socks, gloves to have yourself adequately covered from the pinching chill of winter. 12) Avoid using facial packs and masks which suck out the natural oil content of the skin. Facial pack containing mud should be avoided for this reason. Rather go for cleansing milk and oil based pack. Facial masks containing avocado extracts in addition to olive oil is helpful during winter. Similarly a blend of yogurt, buttermilk and sour cream or the creamy layer of milk with respective ingredients being combined in small measure is helpful for activating the skin's collagen content. 13) To overcome conditions of dryness and chap formation on lips, opt for a lip balm enriched with vitamin E. Likewise, to get rid of ugly cracks on feet, go for a therapeutic ointment or lotion containing vitamin E. In extreme cases of cracks, one has to use ointment containing necessary anti-biotic. A vital skin care tip for winter involves bare minimum use of water. After a wash or shower, make sure to pat yourself completely dry. In case your work demands recurrent exposure to water, make sure to use gloves to have the hands well protected. Use humidifiers to have the indoor environment well hydrated and moisturized. With the help of a humidifier, winter harnessed dryness and conditions of cracks can be overcome. One shouldn't avoid using sun's cream during winter, but it is always preferable using one with an increased moisturizing content.

I am diagnosed as PCOD. I have been getting treatment since 8 months. My periods are regular. But facial hair is problem. PCOD panel test showed all good but still Eggs were not maturing. After 2 months dose of Myocyst M twice daily, The Gyno prescribed for follicular ultrasound on D12 & report is in good limit & provided HUCOG HP 5000 injection on the same day and suggested being in contact with husband. Twice intercourse in D12 & D13, follicular ultrasound showed follicle rupture & eggs release on D14. Still my Gyno prescribed for Duphaston 10 mg tablets along with Myocyst M tabs twice daily. What is chances for my Eggs fertilization. My period date normally falls in 12th of month. May I wait till 12 of this Dec. Month. We have to still intercourse regularly or that was much to get pregnant. Pls suggest.

For pregnancy, we need other tests also, Please read the following Last week I got a call from one of my patients, who conceived after long period of infertility. She called me to remind me that, that very days was the birthday of her son, that is the wealth that she gained after long battle. And even the son tried to talk to me over phone with his soft voice. And this success is really rewarding for any Infertility Specialists. So, infertility treatment is actually based on stepwise manner. We initially advise some tests and then advise a treatment. If that treatment fails, we move to next treatment. So, all the time, you have to keep patience. Eventually, majority of the couples will conceive. Don't give up hope. You have to win the race ultimately Introduction When couples get married, they often view parenthood as the next stage in their family life. They want to have a child, they want to be “mom” and “dad”, they cannot imagine that this may be hard to achieve or may not be a natural process. When several trials to conceive fail, they are shocked. Their basic expectation about family life gets shattered. Most of the couples are desperately looking for medical therapy that will end into a misery. Clearly this is not a struggle to survive; it is a struggle to fulfill a dream, to achieve what they view as a “full life”. What is needed for pregnancy? In the male partner, sperms are normally produced in the testes after puberty (after attainment of characters like growth of beard, moustache etc). From the testes, they are carried through the sperm conducting ducts (epididymis, vas, seminal vesicle and prostate gland). Then during sexual stimulation, after proper erection and ejaculation, they come out through penis. During sexual intercourse, these sperms, present in semen, are deposited inside the vagina. In female partner, the deposited sperms must travel from vagina through the cervix (the mouth of the uterus). The cervix acts as gate-keeper, a it prevents entry of dead and abnormal sperms as well as bacteria present in semen, in the uterus. From uterus, sperms reach the Fallopian tubes (the tubes that are attached to the both sides of the uterus) where the sperms must meet the egg (ovum). The eggs are produced only before birth and so, there are fixed number of eggs inside the ovary. The ovum released from the ovary, into the abdomen at the time of ovulation (rupture of the surface of ovary to release the ovum). That ovum must be taken by the tube and thus inside the tube an embryo (earliest form of the baby) is formed, by meeting of the egg and the sperm. It should be mentioned that out of nearly 200-300 million sperms, in average, deposited in vagina, hardly 500- 800 sperms can reach near the eggs and only one will succeed to form the embryo. The embryo then travels through the tube into the uterus and the uterus attaches the embryo firmly with it and thus the pregnancy starts. So, if there is defect in any one of them there will be difficulty in achieving pregnancy. Thus, to summarise, pregnancy requires 1.Production of healthy (“Normal Morphology”) and movable (“Normal Motility”) sperms in adequate number (“Normal Count”) in the testes 2.Transport of these sperms through the sperm conducting ducts from testes to penis 3.Successful Erection and Ejaculation during Intercourse to deposit adequate number of these sperms in the vagina 4.Transport of these sperms from vagina through cervix to the uterus and the tubes 5.Presence of sufficient number of eggs inside the ovary and ability to release the eggs from the ovaries 6.Pick up of the eggs by the tubes 7.Approximation of eggs and the sperms to form the embryo 8.Transport of embryo from the tubes into the uterus 9.Acceptance of the embryo by the uterus and its growth What is Infertility? Literally, the word “Infertility” means inability to conceive. But in reality, there are very few couples, who have no chance of natural conception and are called “Absolutely Infertile”. In fact, in many couples who present to infertility clinics, pregnancy may be the matter of time, thus the chance factor. It should be kept in mind that, if there is factors to question fertility of either male or female or the female is of age less than 35 years; after one cycle (one month) of regular frequent intercourse, the chance of conception in human being is only 15%. That means, out of 100 couples trying for conception, only 15 will be able to succeed after one month of trying. The word “Regular” and “Frequent” are important; because to achieve pregnancy, couples are advised to keep intimate relationships for at least 2-3 times a week and this should be increased particularly around the time of ovulation (Middle of the menstrual cycle). Thus chance of pregnancy after 6 months, 12 months and 24 months of regular trying are respectively 60%, 80% and 100%. The word, “Subfertility” seems better and more scientific than “Infertility”, to describe the couples who have reduced chance of conception, due to any cause. However, the word “Infertility”, seems more popular, although it puts pressure on the couples. In most cases, usually we advise to investigate after one year of regular and frequent intercourse, when the couples fail to conceive. However, if there are factors to question fertility; for example female with age more than 35 years, or with previous surgery in tubes/ ovaries/ uterus or known diseases like PCOS or endometriosis; or male partner having surgery in scrotum or groin or any hormonal problems or sexual dysfunctions- the wait period is usually reduced and couples can be investigated, even soon after marriage. What causes Infertility? Please look at the point “Thus, to summarise, pregnancy requires” where 9 points have been mentioned. Thus the common causes may be 1.Problems in male- total absence of production of sperms, less than adequate number of sperms, problems in morphology and motility of sperms (most sperms not healthy or movable), blockage in transport of sperms and inability to deposit sperms in the vagina (sexual dysfunction- Erectile Dysfunction or less commonly, Ejaculatory Dysfunction). Examples include hormonal problems (Testosterone, thyroid, prolactin), diabetes, liver problems, causes present since birth, chromosomal abnormalities, surgery, infection, sexually transmitted diseases, smoking, exposure of scrotum to high temperature, some medicines or psychological causes. 2.Problems in female- total absence of less than adequate number of eggs in the ovaries, problems in ovulation, problems in picking of eggs by the tubes, blockage of tubes, problems in conduction of sperms or embryo by the uterus, problems in accepting the embryos by the uterus. Examles include causes present since birth, chromosomal abnormalities, polycystic ovarian syndrome (PCOS), old age, increased weight, fibroid, endometriosis, pelvic inflammatory diseases (PID), tuberculosis (TB), infections, smoking, surgery, some medicines, hormonal problems (thyroid, prolactin) or excessive stress. 3.Unknown causes- Despite thorough investigations, 25-30% causes of infertility remain unknown. This is called “Unexplained Infertility”. The reason may be mere chance factors or there may be some causes which, still medical science has yet to discover. But this should be kept in mind while treating infertility. That means, even with correction of the possible factors (like improving sperm counts or thyroid problems etc) or with proper treatment (IUI, IVF or ICSI), unfortunately the treatment can fail and the exact reason, why the treatment failed, is sometimes difficult to find out. Treatment of Infertility To start with, please remember there is no hard and fast rules for infertility treatment. Often medical science fails to understand why couples with very severe form of infertility conceive sooner than those who are having all tests normal. That means, whatever treatment is offered, it’s very important to continue regular sexual intercourse, as the chance of natural pregnancy is usually there in almost all couples. Your doctor will present the facts to you, without pressurizing you on a particular option. After coming to know all pros and cons of different treatment options, you can take decision. Do not hurry. It’s quite natural that you might be in stress. In general, after the initial tests, a few periods of natural trying is allowed. After that, ovulation induction (giving medicines to release eggs from the ovaries) is offered, failing which IUI and finally IVF is offered. What will be the preferred treatment for you, will depend on your age, duration of marriage, male and female factors and of course, your age. For example, a woman with both tubes blocked or a male with very low sperm count, IVF would be the first line of treatment.

1. Take bland diet, avoid spicy, oily, fried and junk food2 eat and drink slowly. 3. Avoid fatty food, carbonated drink like soda/ soft drink/beer, chewing gum, sucking hard candy etc, 5. Avoid gas forming food like onion, bean, peas, potato, lentil, lettuce, cabbage, cauliflower, broccoli, mushroom, whole wheat bread. Fruits like apple/peach/pear 6. Avoid stress, anxiety 7. Stop alcohol/ smoking 8. Go for regular exercise and avoid sedentary life style 9. Take one tablet of pan40 in the morning 10. Take unienzyme 2 tablet twice daily after food for few days.

She should tk plenty of water to hydrate her body Should go for meditation to reduce her stress to calm her nerve. Her diet be regular and easily digestible to avoid gastritis. Dip cotton swabs in cold rose water and wipe your face and ears too for immediate relief. Also splash mild hot and cold waters simultaneously on your face which acts as a astringent and relaxes facial skin and stabilises the temperature.

Your sleep is disturbed, stop worrying, take bath two times, have regular light exercise, avoid spicy and pungent diet, take fruits and fresh juices. Practice meditation, yoga and pranayam, cultivate positive thoughts, this will help in getting good sleep.

Urinary Tract Infection in Childhood - What Parents Need To Know?

Many children all over the world are affected by urinary tract infections or UTIs that can be resolved with simple antibiotics, but might also lead to complications at times. Most often, kids under 2 years of age are affected by it, and either the bladder or the kidney might be infected leading to cystitis or pyelonephritis , in smaller children kidneys are presumed to be infected unless proved otherwise.

Causes of UTIs

Though bacterial infections are the most common cause, viral or fungal infections might occur in some cases as well. Most UTI in infants are caused by structural abnormalities of urinary tract. Uncircumcised male infants or children with poor toilet habits, or female children with poor toilet hygiene are susceptible to this disease. Children suffering from a weak immune system might also be at a risk.

Symptoms and signs

Symptoms of UTI are very difficult to differentiate from any other illness in small kids under 2 years of age, symptoms include high fever, diarrhea vomiting, and dehydration. Basically any fever for which no cause is apparent must be evaluated for UTI. UTIs in older children beyond 5 years of age come with different signs including pain during urinating (dysuria), frequent urination, abnormal urge for urination, or bedwetting. Sometimes, fever, abdominal pain, blood in urine or vomiting might be signs as well.

Which doctor to consult?

In most cases, UTIs in children are treated by pediatricians, but if kidney function is troubled then a pediatric nephrologist needs to be contacted immediately. Pediatric Nephrologist to be consulted once the fever is over to look for cause of UTI.

  1. Tests carried out: To understand the underlying cause of the infection and any anatomical or functional risk factors, several examinations or tests are carried out. Vital signs like blood pressure, body temperature, and breathing rate are checked. The abdomen is palpated to find tenderness near the kidneys. Genital areas are also examined for signs of trauma, redness, discharge and such. Urine cultures are essential for diagnosing UTIs finally and this helps in assessing the antibiotic sensitivity profile too.
  2. Good to know: Right after an antibiotic is administered, UTI in children starts getting resolved. But recurrent UTIs might lead to urinary tract abnormalities like kidney malformation. Also note that UTI is not contagious, and cannot be passed on if children share a bath or if you sit on an infected toilet seat.

Treatment

In most cases, UTIs respond well to oral antibiotics, though Pyelonephritis may require hospitalization and intravenous drip. Some studies are also carried out to check if the child is susceptible to renal scarring or kidney failure. These are as below:

  1. Renal ultrasound: Defines the location of the kidneys and their size and shape clearly.
  2. Voiding cysto-urethrogram: The bladder is filled with a dye through a catheter in this method. Then the catheter is removed to study if the bladder is getting emptied without any reflux or obstruction.
  3. Renal scan: To test the functioning of the kidneys and the risk of renal scarring, a bit of radioactive material is used.
  4. Intravenous pyelogram: Though rarely used, in this method, a dye is injected into the bloodstream and X-ray images are obtained.

Prevention

  1. Maintaining proper hygiene: Girls should wipe from front to back and uncircumcised boys should be able to gently retract the foreskin to reveal the urethral opening.
  2. Complete voiding of bladder: Encourage kids to urinate every two to three hours, since they often ignore a full bladder to carry on playing.
  3. Consumption of fluids: Drink plenty of fluids and avoid constipation. If you wish to discuss about any specific problem, you can consult a Pediatrician.
