What Do You Do About a Severe Toothache?

Those who have experienced it would vouch for the fact that toothache is one of the worst pains. There could be times when the attack happens out of nowhere and you are crying for relief.

The tooth has 2 parts - the visible part called the crown and the invisible part called the root which is embedded in the jaw bone and covered by the gums.  

Both the crown and the root have 3 layers from inside out. The crown has enamel, dentin, and pulp. The enamel is the mineralized part of the tooth, dentin has fine sensory dentinal tubules, and the pulp receives nerve and blood supply to the tooth through a small orifice at the end of the tooth called the apex.  On the root surface, instead of enamel, there is a softer substance called cementum.  The dentin and the pulp continue through the tooth, but are thinner in the root portion of the tooth.

The mouth has the largest amount of bacteria in the body. These act on the food deposits on the tooth and produce acid which leads to breakdown of the enamel. The only symptom when enamel breakdown happens is food lodgment, and it continues till treatment ensues. Once the breakdown reaches the dentin, sensitivity sets in, and most people go for treatment then. If not, the next layer is the pulp, when there is severe pain.  This acute pulpitis causes pain in spurts and can be unbearable.

On the root surface, if there is periodontal disease and the gum line goes down, then cementum gets worn off (far more easily than enamel) and decay reaches the dentin and pulp (again faster than in the crown).

Whatever the case, the treatment would be the same:

  • Dental examination, clinical testing, and x-rays would be diagnostic. Tapping the tooth would reproduce the same pain and that is indicative of acute pulpitis
  • Antibiotics and pain killers would be given to control the pain
  • Once the infection subsides, root canal therapy is initiated.  Using the decayed portion to gain access to the root, thin instruments called reamers and files are used to clean out the pulp space completely. They are then shaped to accommodate an inert substance called gutta percha which ensures infection does not seep into the tooth again.
  • With RCT, the tooth is weakened, and therefore a crown needs to be placed. This could either be a ceramic crown or a full metal crown based on economic and esthetic reasons.

The best way to avoid this is regular visits to a dentist so that decay is identified in the early stages and treated with the minimal cost and maximum natural tooth preservation.

You can start using a sensitive toothpaste. Take a little bit toothpaste on your finger and massage it on the sensitive teeth. Leave for 6-8 minutes and then rinse off.

- use a soft toothbrush -- and brush properly! brush your teeth for at least two minutes twice a day and after meals. Hard brushing can further damage the soft tissues of your mouth. - floss at least once a day and be sure to floss beyond the gum line to remove more plaque. - control bleeding by applying pressure to the area with a cold compress. - rinse with salt water or hydrogen peroxide to keep the area clean. Avoid mouthwashes that contain alcohol, which can dry out your mouth. - stay away from smoking and other tobacco products, which can aggravate bleeding gums. - eat a balanced diet and limit snacking between meals. Carbohydrates and sugars feed dental plaque. - try an oral irrigation device, commonly known as a" water pick, to clear debris from around the gum line. - have your dentures aligned as needed. - you may need deep cleaning along with surgical curettage or gum strengthening procedure. Use pepsodent tooth paste & gum paint for 2 to 3 weeks. Advance procedures can be done with laser.

Often and at regular intervals while eating I cut my inner side of cheeks. From next day it gets reddish and pains too hard. I cannot eat even roti properly. If any thing touches that particular portion it pains a lot. The pain exists for at least 7 to 10 days.

By your symptoms it appears to be cheek biting means your teeth sharp cuspal edges are hurting your cheek. I advice you to visit your dentist so that he can roundoff sharp/pointed edges of your teeth which is creating a problem also he may prescripe you some ointment to apply locally over your cheek, so that wound healing get fast. Mean while avoid consuming spicy and hot food/beverages.

Dear You hv already seen two doctors and they might hv given you some antibiotics and painkillers. But the main thing is you need to do treatment for the ailing tooth. Its either rct or extraction. If its not tooth related you need to go for higher investigations.

The type of treatment you have for toothache /sensitivity/swelling will depend on the cause of the pain, so your dentist will examine your mouth and may carry out an X-ray to try to identify the problem. •If your toothache / sensitivity/swelling is caused by tooth decay, your dentist will remove the decayed area and replace it with a filling. •If your toothache / sensitivity/swelling is caused by a loose or broken filling, the filling will be taken out, any decay will be removed and a new filling put in place. •If the pulp inside your tooth is infected, you may need root canal treatment. This procedure involves removing the infected pulp and then inserting a special type of filling to seal the tooth and prevent re-infection. •If your toothache / sensitivity/swelling can't be treated using these methods, or if your tooth is impacted (wedged between another tooth and your jaw), it may need to be removed.

I got ulcer in my mouth around every month 2-3 times .even I am not able to take liquid or food at that time. What should I do to get rid of this.

Aapke mouth me bar bar ulcer ho jate hai you must ,know the cause of ulcer in mouth main reason is Hurry Worry and curry deficiency of vitamins, especially B12 and C Poor dental hygiene,- Food allergies, Stress, Hormonal imbalance. You need to visit dental surgeon for scaling and polishing of teeth maintain good digestion and oral hygiene .apply boroglycerine and take precautions. Maintain good oral hygiene. Use a soft-bristled toothbrush. Use ice water of and on. Chew a few basil or tulsi leaves.- Apply little honey onto the mouth ulcer or mix in a little turmeric to make a paste that can be applied to the affected area. Honey helps to retain moisture and prevent dehydration. It also helps reduce scarring and hastens the process of new tissue growth. Reduce your stress levels by activity that you find relaxing, such as yoga, meditation or exercise.

Hi, Going by your description it seems you mean that entire teeth are not seen in mouth and only half of the teeth are seen. It may be a condition where your teeth hv fully erupted but are partly covered by excessive gums giving the impression that your teeth are not completely erupted. You may require crown lengthening procedure. It is a cosmetic procedure with excellent result potential. Pardon me if I hv misunderstood your question. Go for filling of teeth which is better because at this age your child have both mily as well as permanent teeth. At the age of 9 permanent teeth are at risk.

I felt in an bike accident on 3 years back. At that time my 3 teeth has broken, on of the tooth has broken half and had root canal treatment to it. Now am planning to replace my broken teeth. Is bridge replacement is better for me or implantation? Amd what will be the cost for each of the treatment. Please reply me. My age is 22. This became a big problem for me :-(.

If you have root canal treatment done, why do you want to extract your tooth. If there is a problem with root canal, then go for re rct or apicectomy whatever your dentist suggests. And take a crown over RC treated tooth for looks. Go for E max if you want excellent esthetics, but try to be with your real tooth, dont get them extracted
