Japanese encephalitis is a type of viral brain infection that's spread through mosquito bites. It's most common in rural areas throughout South East Asia, the Pacific islands and the Far East, but is very rare in travellers?

Japanese encephalitis (JE) is a disease spread through mosquito bites. Symptoms usually take 5-15 days to develop and include fever, headache, vomiting, confusion, and difficulty moving. Symptoms that develop later include swelling around the brain and coma. JE is a serious disease that may cause death. Transmission of Japanese Encephalitis Virus. Japanese encephalitis (JE) virus, a flavivirus, is closely related to West Nile and St. Louis encephalitis viruses..In the subtropics and tropics, transmission can occur year-round, often with a peak during the rainy season. The virus is maintained in a cycle between mosquitoes and vertebrate hosts, primarily pigs and wading birds. Humans are incidental or dead-end hosts, because they usually do not develop high enough concentrations of JE virus in their bloodstreams to infect feeding mosquitoes. JE virus transmission occurs primarily in rural agricultural areas, often associated with rice production and flooding irrigation. In some areas of Asia, these conditions can occur near urban centers. In temperate areas of Asia, JE virus transmission is seasonal. Human disease usually peaks in the summer and fall. In the subtropics and tropics, transmission can occur year-round, often with a peak during the rainy season. JE virus is transmitted to humans through the bite of infected Culex species mosquitoes, particularly Culex tritaeniorhynchus. The prevention of Japanese encephalitis in travelers presents the juxtaposition of 4 factors: a disease that is widespread throughout Asia, a disease with a low incidence in travelers, a vaccine about which there are safety concerns, and a clinical course that can result in death or permanent disability in two-thirds of symptomatic cases. Travel medicine practitioners often seem to be polarized into 2 groups: a group that gives more weight to the severity of the disease (and therefore often recommend vaccination) and another group that is more persuaded by the low occurrence of cases in travelers (and therefore rarely recommend vaccination). This review assesses the known risks of contracting Japanese encephalitis and the risks associated with the vaccine and tries to develop an appropriate way to recommend this vaccine to travelers who may be at significant risk.

Lupus Flare-up - Ways You Can Manage It!

Dermatologist, Bangalore
Lupus Flare-up - Ways You Can Manage It!

Lupus is an autoimmune system disease in which the body's immune system gets hyperactive and attacks the normal and healthy tissue. It is a chronic illness. Individuals with lupus may encounter episodes of a flare up when symptoms show up, and episodes of remission when the side effects are under control. During a lupus flare up one may experience side effects such as weariness, exhaustion, weight reduction, fever, and iron deficiency. Flares and remissions can happen at any time, all of a sudden.

Some of the cautioning signs of a flare are as follows:

  1. Weakness and fatigue
  2. High fever
  3. Increased pain
  4. Development of a rash or worsening of one
  5. Disturbed stomach
  6. A migraine or wooziness

Some of the ways through which one can reduce lupus flare-ups are as follows

  1. Figure out how to perceive the signs of a flare up and talk to your specialist regarding them.
  2. Keep up your physical well-being. Make sure to visit your specialist frequently, regardless of the fact that you are feeling fine. Plan customary dental, eye, and gynecological exams.
  3. Get enough rest. Be adaptable with your timetable of day-by-day exercises.
  4. Attempt to limit your anxiety and stress. Since this might be difficult to do now and again, think about creating a method of managing stress. Build up an emotionally supportive network that includes your family, companions, medicinal or nursing experts, group associations, and care groups.
  5. Participate in an activity or a project to help you keep up physical wellness and decrease stress.
  6. Have a solid and healthy eating routine.
  7. Limit your exposure to the sun and different sources of bright light, for example, fluorescent or incandescent lamps.
  8. Notify your specialist immediately regarding any damage, ailment, or infection in case you do not feel well.
  9. Delay elective surgery (dental surgery and teeth pulling) until your lupus is under control or going away.
  10. Lupus may bring about issues for a pregnant woman and her child. Thus, women with lupus need to be careful while planning any pregnancy. Try not to quit using conception prevention until you have talked about the likelihood of pregnancy with your specialist and he or she has verified that you are perfectly healthy to get pregnant.
  11. Talk to your specialist before you quit taking any recommended medicines.
  12. Check with your specialist or attendant before taking any over-the-counter prescriptions.
  13. Be cautious while attempting any over-the-counter medications for your skin or scalp. In case there is any redness, rash, raised areas, itching or irritation, do not use the medicine.
  14. Know that specific physician recommended medications might trigger a flare. up Likewise, tell your lupus specialist or doctor if any new prescriptions have been recommended for you.
  15. Make sure to check with your lupus specialist before accepting any vaccination. Routine injections, including those for season's flu virus and pneumonia, are an essential piece of keeping up your well-being, and you need to get them if your specialist confirms. If you wish to discuss about any specific problem,

In case you have a concern or query you can always consult an expert & get answers to your questions!

I am not eating too much but I become fatty person day by day, its obesity. so, what should I do.?

Dietitian/Nutritionist, Mumbai
I am not eating too much but I become fatty person day by day, its obesity. so, what should I do.?
Have you followed a personalized diet plan prescribed by a registered dietitian? I am both a registered dietitian and a doctor who will help you in losing your weight as well as solve your other health problems through diet. Let me know your food intake details. The best diet is one which is healthy and well balanced with foods from all the food groups, and one which is varied and that you can stick to weight loss on some diets can be dramatic in the short term, but there's more of a chance of rebound weight gain. Set yourself a realistic target and aim to lose between (0.5-1.0kg) a week I being a registered dietitian strongly accommodate the patients own preferences without the patient feeling that they are on a diet not compromising the weight management goals. Don't fall for self dieting or fad diets availiable online they have harmful effects in the long run, if you lose a lot of weight very quickly, you may not lose as much fat as you would with a more modest rate of weight loss. Instead, you might lose water weight or even lean muscle tissue, since it's hard to burn that many fat calories in a short period. I will need to carry out detailed assessment in your case, after assessing your personal challenges to weight loss, I will recommend a strategy to gradually change habits and attitudes that have sabotaged your past efforts. Also the overall objective should not be to drastically reduce weight but doing it gradually and help rebuild body tissues and to maintain ideal body weight. I being a both a registered dietitian (rd) and doctor have been successfully helping patients with their problems through a holistic approach using customized therapeutic diet and medications. I will also suggest home remedies. Do reply back for private consultation for rx.

Get your thyroid and vit-d tests done. If these are imbalaced, then balancing them is necessary. Ayurvedic management of premature greying of hair: -grind the curry leaves and squeeze out their juice. Mix curry leaves juice and buttermilk. Boil the mixture on low flame for 5 minutes. Let it cool to become lukewarm. Apply this lukewarm buttermilk-curry leaves juice mixture on your scalp. Leave for half an hour. Wash hair with lukewarm water. -apply onion juice onto your scalp, massage, and leave it on for about 30 minutes to an hour. Repeat the process daily for a few weeks. Oil- boil a handful of fresh curry leaves in 6-8 tablespoons of coconut oil until the leaves become black. Strain the oil. Apply this oil to your hair daily at night and massage your scalp only with fingers gently. Massage for about 15-20 minutes. Leave it overnight and wash off your hair the next morning. Use this oil for at least 3 months to get results. Diet: -simply eat a teaspoon of black sesame seeds daily for at least three months. -boil the curry leaves in water and drink the hot beverage. -protein, vit a, vit e rich diet. -avoid too much fried, oily, greasy, spicy, and acidic foods. -avoid excessive intake of tea, coffee. -keep off stress by practicing regular meditation.
