Mr. lybrate-user, Thanks for the query. To diagnose Gestational diabetes following criteria are considered: Fasting >92 mg, on 75 g glucose 1 hour level should be >180 mg & 2 hour glucose > 153 mg. The level mentioned 133 mg is at what time after taking 75 g of glucose? Plus the tablet mentioned is an ayurvedic preparation without any clinical studies. I would never give it in pregnancy. First of all whether she has GDM is itself a question mark. I hope you have followed all the details mentioned. Please take care. Thanks.

Why and When Should I Go for HPV Test?

Persistent infection of the uterine cervix with high risk human papilloma virus (HPV) leads to development of cervical cancer. Infection with HPV is common especially in sexually active young women, most infections are transient and spontaneously clear within 1 to 2 years without causing cancer. These transient infections may cause temporary changes in cervical cells. If a cervical infection with high risk HPV type persists the cellular changes can eventually develop into more severe precancerous lesions. If pre cancerous lesions are not treated, they can progress to cancer. Thus, an HPV DNA test is recommended to identify the high risk types of HPV.

Why is Pap Smear done?

An HPV test is done to:

Check for high-risk types of human papillomavirus (HPV) in women who had a Pap test that showed abnormal cervical cells called atypical squamous cells (ASCUS). An HPV test can help look for one or more high-risk types of HPV.
Check for HPV in women older than age 30 as part of screening for abnormal cervical cells.
Help check for abnormal cervical cells after treatment of a high-risk HPV infection.

When should one get screened for cervical cancer?

American Cancer Society does not recommend using HPV DNA test to screen for cervical cancer in females under 30. This is because females in their twenties who are sexually active are much more likely (than older females) to have a HPV infection that goes away on its own. For these young females, results of this test are not as significant and may be confusing.

For females age 30 and older, Pap & HPV co testing is less likely to miss an abnormality (i.e. lower false negative rate than pap testing alone. It also lengthens the screening interval to 5 years and still allows abnormalities to be detected in time to treat them.

Women of age >65 yrs who have had adequate screening history or those who have had total hysterectomy do not need any further screening.

HPV TEST

Is HPV test as safe and effective as Pap Smear?

The advantage of the HPV test is that the Pap test misses a fair number of carcinomas as it has a lower sensitivity. Studies have also shown that a negative HPV test is better at predicting that a woman will be free of lesions for the next three years than a negative Pap test.

And while the experience for the patient is the same for both tests, the HPV test requires less material from the cervix to detect an infection so there's less concern that a sample will be inadequate.

How is the HPV test performed?
Before an HPV test, do not douche, use tampons, or use vaginal medicines for at least 48 hours.
In the HPV test, a doctor takes a swab of cells from the cervix, just as for the Pap test. The cells are then analyzed in a laboratory. The genetic material (DNA) of the HPV virus is studied. This test can identify 13 or 14 of the high-risk HPV types associated with cervical cancer. HPV test results are generally available in 1 to 2 weeks.

You may have a small amount of vaginal bleeding or gray-green discharge after this test.

Chance of pregnancy is not there since you had taken emergency contraceptive pill. However Do a pregnancy test 10 days after missed period and consult doctor for advice and treatment.

For first 13 weeks of pregnancy fetus's thyroid is dependent on mother's thyroid function. It is good that you have started medication today and have also taken Endocrinologist's appointment. TSH will have to be monitored at short intervals to ensure that it is below 2.5 micro IU/ml. Continue the medication and follow Endocrinologist's instructions.

Breast Reduction
Breast reduction, or reduction mammoplasty, is for a woman experiencing health problems and/or extreme self-consciousness associated with very large, heavy breasts. The main complains are backache, neck pain and skin rashes or itching along with difficulty in carrying large breasts. The goal is to give the woman a more attractive contour with smaller, better-shaped breasts in proportion with the rest of her body. She will then benefit from freedom of health problems associated with large breasts and an improved self-image.
Medical problems associated with very large breasts include back and neck pain caused by the excessive weight, skin irritation, skeletal deformities and breathing problems. Bra straps may leave shoulder indentations. Large, heavy breasts also contribute to poor posture, and can interfere with normal daily activities such as exercise. Excessive breast size may also lead to a decreased sense of attractiveness and self-confidence.

THE PROCEDURE
Breast reduction is done under general anesthesia on an outpatient basis or in the hospital. The surgery removes fat, glandular tissue, and skin from the breasts, making them smaller, lighter, and firmer. It can also reduce the size of the areola, the darker skin surrounding the nipple.
Incisions are made around the pigmented nipple-areolar complex and extend vertically below the nipple and in the fold under the breast. The nipple-areolar complex is moved upward to the desired location. The incisions are covered with a light dressing.
Afterward, the breasts are placed in a surgical bra that will hold them symmetrically during initial healing. Initial discomfort subsides daily and can be controlled with oral medications. Scars will fade in 6-18 months. Surgery will likely reduce, but not eliminate the ability to breast-feed.

RECOVERY: WHAT TO EXPECT
When performed by a qualified plastic surgeon, breast reduction is a safe procedure. Nevertheless, as with any surgery, there is always a possibility of complications, including bleeding, infection, or reaction to the anesthesia. Some patients develop small sores around their nipples after surgery; these can be treated with antibiotic creams. You can reduce your risks by closely following your physician’s advice both before and after surgery.
Much of the swelling and bruising disappears in the first few weeks following the surgery. Breasts may appear slightly mismatched, or have unevenly positioned nipples. Their new shape will be apparent within 6 months to a year, and will depend on hormonal fluctuations, weight changes and pregnancy.
Although your surgeon will make your scars as inconspicuous as possible, some permanent scarring is inevitable. Smokers are more likely to experience poor healing and wider scars. The scars will be red and lumpy in the months following the surgery, but the redness will fade and in time the scars will be less obvious.

ABILITY TO BREASTFEED
Because the surgery removes many of the milk ducts leading to the nipples, breast-feeding may no longer be an option.
Some patients may experience a permanent loss of feeling in their nipples or breasts. Rarely, the nipple and areola may lose their blood supply and the tissue will die. (The nipple and areola can usually be rebuilt, however, using skin grafts from elsewhere on the body.)

THE END RESULT
Breast reduction produces the most dramatic results of all plastic surgeries. It ends the physical discomfort or large breasts and makes your body appear more evenly proportioned, and clothes fit better. Your new image will take some getting used to, as much as you desired the change. Give yourself—and your family and friends—time to adjust to the new you and, like most women, you will enjoy the benefits.

It may be due to hormonal imbalance. If this problem is continueously going on, get ultrasound of lower abdomen and thyroid profile test done to ruleout the main cause.

Benefits of Red Wine:

1. The antioxidants: 

 The red wine contains antioxidants, especially resveratrol, which is found in grape skins and seeds. The importance of antioxidans is long known, they fight free radicals and this way protects the healthy cells against their action.

2. Prevents heart attack:

 The resveratrol has an important role in blood thining. It works quickly, preventing the platelets (cells involved in blood clotting) to stick together. Through this mechanism, the resveratrol can prevent heart attack.
 Consumed with moderation, as medicine, red wine decreases the risk of heart attacks in eldry people, with aproximately 30-50%. For people who have already suffered a heart attack, the red wine has the power to prevent another attack. Red wine can increase the amount of hdl (the good cholesterol) and prevents ldl (the bad cholesterol). The studies have reported that people who regularly and moderately comsume red wine, presents a lower risk of being diagnosed with cardiovascular disease. The moderate consumption of red wine reduces inflammation of chronic smokers blood vessels, prevents blood clots and reduces vessel damage caused by fat deposits.

3. Helps the digestion:

 It was noticed that the natural, untreated red wine helps digestion and prevents bloating. A glass of red wine, after a meal rich in meat and fats, helps in evacuation of waste from the intestine, thus reducing the risk of gastric or duodenal ulcer.

4. Skin care

 Also the red wine is ideal for skin care. The resveratrol from its composition helps the skin to remain young and firm, protecting it from free radicals and delaying the appearance of wrinkles. To enjoy an extra of beauty and freshness at skin level, add each time in the bath watter, one or two glasses of red wine.

5. Prevents breast cancer:

 The red wine prevents and keep the breast cancer under control. The scientists at harvard university have found that women who drink two glasses of red wine per day have a 50% lower chances to suffer from breast cancer, while specialists from the university of calabria, italy, found that women who have already been diagnosed with this disease can keep it under control if they drink a glass of red wine every day
 
6. Phytoestrogen effects:

 The resveratrol is a phytoestrogen. There are certain herbs that can play the estrogen role in organism, by having similar effects. Thus, it was sugested that the red wine can combat some problems caused by lack of estrogen, such as osteoporosis or premature menopause. However, these assumpsions have not been proven yet. For those who do not like red wine, the good news is that the resveratroul is also found in peanuts, blueberries and rosehips.

Consult an Infertility specialist. Go for follicular monitoring to check ovulation Get your husband's semen testing done. GO for laparoscopy to check if fallopian tubes are patent or closed. There can be observer variation in measuring length of cervix. As long as it is more than 2.5 cm there is nothing to worry. Also if you have not gone into preterm labour before chances are very low unless your pregnancy this time has developed any new complications. You can continue to take susten tablet as advised but there is no evidence to show that bed rest in pregnancy prevents preterm labour.
