Hello I'm Dr. Siddharth Prakash. I'm a cosmetic and plastic surgeon and I practice at Lilavati Ho...

Hello I'm Dr. Siddharth Prakash. I'm a cosmetic and plastic surgeon and I practice at Lilavati Hospital in Bandra and at my own clinic in Andheri Lokhandvala. Today I would like to talk to you a bit about plastic surgery in general because most people do not have a lot of information about this branch.

Plastic surgery involves a very wide variety of procedures. These procedures can vary from reconstructive procedures which came to correct abnormalities which come from cancer surgeries, from injuries, from bonds, from difficult wounds and it can also be cosmetic plastic surgery. Cosmetic plastic surgery aims to make you more beautiful to improve your appearance. The most common question, the most common inquiries that I receive in my own cosmetic surgery practice is about liposuction. Liposuction is a fat reduction and our body reshaping procedure. Then I'm asked about tummy, talk about abdominoplasty, about breast surgeries like breast augmentation, breast lift, breast reduction, male breast correction. I'm asked about facial plastic surgeries like face lifts and nose jobs, lip augmentation, chin and cheek augmentation. I'm asked about other skin-tightening procedures after weight loss and many more. The list is endless but if you are considering any of these procedures, I am sure you have a lot of questions. If I could just touch upon some of the questions today to be good. The most common question would be how do I choose my plastic surgeon.

Now you don't visit a plastic surgeon only because this clinic is very close to your home or he charges very less or his waiting room his always full. Those are the wrong reasons. The right reasons- you would have to be sure about his credentials, his qualification, is he a qualified plastic surgeon because unfortunately there are many general surgeons or even plain MBBS doctors who are doing a lot of cosmetic surgery. Now that is not safe and I would not advise that.

The other thing would be that we have additional training or fellowships in cosmetic plastic surgery because a good reconstructive surgeon may not always be a good cosmetic surgeon and your doctor should be trained in the procedure that you are planning to have.

Another question would be which hospital is your doctor attached to. Naturally the best hospitals in the city would employ be the best cosmetic surgeons as well so that becomes an automatic choice. Another important factor is what sort of a connection that you're able to you know have with your plastic surgeon. Does it give you time, does he listens to you, does he answer your questions. So in short, how comfortable is your plastic surgeon making you in your consultation. So that first consultation is an important time. I usually keep about one hour for my new patients because I need to know their expectation, I need to know what what they have come in with, what are their desire, you know what changes do they want in their appearance and they want to know from me which procedure would be exactly right for them, what could be the possible side effects or I need to know what concerns they have, so this involves time, it involves a building of a relationship and I need to be able to answer all your questions thoroughly, so that you are mentally and physically ready for your surgery.

The other questions that people have are regarding place, time cost. If it's a small cosmetic surgery procedure I can do it in my clinic but larger procedures need to be done in a fully equipped hospital. I would never advise anybody to get cosmetic surgery procedure in a small clinic, in clinic sort of a nursing home or place, simply to save some money. That would not be safe.

As regards time, if you come into the hospital in the morning you can be ready to go home by evening. You know more surgeries nowadays are day care procedures. If it's a bigger surgery when you might need to stay in overnight and if you are an overseas patient. If you are coming in from the city you can choose to stay back in the hospital for a few more days when that by choice. As regard cost depending upon how big the procedure is the cost is very variable and it can be something very little to something much more depending upon how much time the surgery is going to take but believe me cosmetic surgery is affordable. No it is not as expensive as some people would like you to think. Other questions that people have regards what results can I expect from plastic surgery. Now i can give you pretty impressive results and they can make a big difference to your appearance but if you believe that you are going to be able to look like that particular celebrity or somebody totally different then that fiction, that's only in movies and cinema is not true. Will these results last?  Yes most results from surgery will be lifelong and if your doctor does advise you some dietary or health modifications then you should keep that in mind but result will be permanent. Results will last and l these results are safe and these procedures. If they are very safe that's not a problem if you are in the in the right hands, in the right hospital. All surgical procedures have a small risk of bleeding or infection or scarring but what is more important is your doctor's ability to be to be able to keep the chance of anything like that happening to the minimum and if some rare thing occurs he should be confident enough to be able to handle it well. You know your doctor should be with you through your recovery period, through the entire healing process. It should be able to answer your questions and I aim to be there for everything. So that you are comfortable and you're well taken care of. So in general plastic surgery is very effective, very safe and I'm sure you have a lot more questions specific to certain procedures that you have in mind.

You could always contact me by Lybrate and come into my clinic for a personal one on one consultation. Look forward to see you. Thank you.

Body Contouring - For the Perfect Sculpted Look

Body Contouring - For the Perfect Sculpted Look

Not sure if there is anyone who is happy with their image, different people have different complaints. Fat thighs, fat upper arms, too thin legs, sagging breasts, pot belly, the list is just too long. While exercise and diet can be used to achieve overall fitness levels, they cannot produce the body shape that you have in mind (simulating your celebrity body part maybe). Almost all complaints revolve around the excess fat (in most cases) and about the quality of the skin.

However, there are various surgical methods which can produce results that are amazingly quick, effective, and safe. Body contouring is usually done by removing excess fat deposits and skin from various parts of the body. The skin also gets tightened during the process, also reducing cellulite. While most women complain about thighs and hips, men complain about their stomach area.

Some of the common body sculpting procedures includes:

  1. Arm lifts: With age, there is excessive fat accumulation and sagging of the upper arm, which can be corrected by removing excess fat. Contrarily, thin arms can be helped by using biceps implants.
  2. Tummy tucks: The excess skin and fat is removed from the tummy area and the muscles are tightened to produce a firmer look
  3. Thigh lift: Another common area of concern, the tone is improved by removing excess, loose skin.
  4. Pectoral implants: Can be used in men who desire muscular chests.
  5. Butt lifts and butt implants: These are used to alter the shape of the buttocks, either reducing it or firming it up.

In all these body contouring procedures, liposuction is the common technique.

Is it for you? If you are in overall good health with good thick and elastic skin, go for it. However, have realistic expectations by having a detailed discussion with the doctor. It will definitely help in fat removal, cellulite reduction and skin tightening; however, the key is to set a common goal and work towards it.

Gynecomastia Treatment

Gynecomastia Treatment

While women strive to enhance the way their breasts look, the development of breast tissue in men can be embarrassing. This condition is known as Gynecomastia. It can affect one or both breasts in a symmetric or asymmetric manner and is caused by an imbalance of estrogen and testosterone hormones. It can also be caused due to a side effect of medication or because of other health problems such as an overactive thyroid or tumor. In some cases, this condition can get resolved on its own while in other cases, medication and surgery may be recommended.

Medications: Before undergoing treatment for Gynecomastia, it is essential to ascertain that you are in fact suffering from this condition. This is because gynecomastia and the buildup of fat tissue in the chest can look similar, but are treated differently. When treating this condition, medication needs to be supplemented with controlled calorie intake and physical exercise. It is also cheaper than surgical treatment.

Surgical Treatment: Surgery is often the preferred mode of treatment for significantly enlarged male breasts. Surgical treatment for gynecomastia is of two types:

  • LiposuctionThis removes fat from the breast, but leaves the breast gland tissue in place. Liposuction can be performed in a number of ways. It involves making two or more incisions through which narrow tubes are inserted into the chest area. This tube is gently moved around to loosen the excess fat tissue that is then sucked out.
  • Mastectomy: This procedure removes the breast gland tissue itself. It is performed endoscopically and hence has a quicker recovery time and involves less scarring.

You may feel a slight discomfort after the surgery for a few days. You will also be advised to wear a chest support brace for a few days to allow your body to heal without putting any pressure on the site of the surgery. Normal activities can be resumed within 2 to 3 days but avoid strenuous activities of heavy lifting for a month after the surgery.

As long as you follow a healthy lifestyle and have a balanced diet, either of the above surgical procedures should have a lasting effect. This makes surgery a more preferred form of treatment than medication.

Apart from this, certain exercises can also help tone chest muscles and reduce fat tissue. This includes inclines dumbbell presses, push-ups and exercises that focus on pectoral muscles. However, this is a very gradual process and hence suitable only for mild cases of gynecomastia.

Facts About Body Contouring Surgery

Losing weight for an obese person is a big achievement, but mere weight loss may not give them the desire look. Dramatic weight loss often causes excess skin to hang in folds. This can be both embarrassing and unappealing. While exercising and dieting can reduce fat, there is no way to deal with this excess skin other than surgery. Body contouring refers to plastic surgery procedures that address this.

There are many type of body contouring surgeries. Face lifts, tummy tucks and breast lift procedures are common examples of such surgeries. Body contouring surgeries that follow weight loss usually address more than one area.  The lower body lift, which focuses on the tummy, midsection and thighs is one of the most common procedures. Though such a surgery is usually performed by a team of surgeons who simultaneously focus on different parts of the body, a body contouring surgery can take up to 8 hours. People who have undergone drastic weight loss may still need multiple surgeries to achieve the desired look.

The risks of such a surgery are similar to other procedures that are performed under anesthesia. The most common complications experienced after this surgery are infections of the wound, seroma formation, slow healing of the wound, excessive bleeding and reopening of the wound. Patients may also notice a difference in the quality of their skin. This can lead to slow wound healing, skin laxity and issues related to scarring. For the overall cosmetic results to be appreciated, the patient must wait for at least 3 months. The scars can take up to a year to heal and blend with the rest of the skin. Body contouring is also a surgery that is not covered by most insurance policies and can be quite expensive.

If you are considering this surgery, ensure that your weight remains stable for 3 to 6 months before the surgery. This will ensure that the surgery has long lasting results. Weight fluctuations, both gain and loss can lead to the need for additional procedures. If your weight loss is a result of bariatric surgery, you may need to wait a year before undergoing a contouring procedure. Instead of opting for an overall contouring procedure, prioritize your problem areas and address these first. You may realize that you do not need additional surgeries. You should also be aware that though this surgery removes excess skin, it will not leave you with flawless skin. the scars caused by this surgery may lighten over time but are permanent.

Rhinoplasty: Procedure and Recovery

Rhinoplasty: Procedure and Recovery

With cosmetic surgery, today, it is possible to reshape the contours of your face anyway you like. Rhinoplasty is one of the most common types of cosmetic surgeries. This is a surgery to restructure the shape and size of your nose. This procedure can be done under local or general anesthesia. Depending on the amount of restructuring needed, the surgery can be performed as an outpatient procedure or may require an overnight stay at the hospital.

To change the structure of the nose, the surgeon makes an incision on the nose to access the bones and cartilage. This incision can be made inside the nose to keep the scar from being visible after surgery. Depending on how the nose is to be structured, bone and cartilage may be removed or tissue from another part of the body may be added. Once this is done, skin and tissue is re-draped over the bone structure. A splint is added outside the nose to support it while it heals.

This splint and internal dressings remain in place for about a week. The area around your nose and eyes will also feel swollen and appear bruised. This swelling and bruising will take at least a fortnight to subside. The swelling may make your nose feel congested and cause slight bleeding and drainage of mucus. Cold compresses and pain medication can help relieve the discomfort.

Here are a few other tips to help speed up the healing process:

  • Keep your head elevated and relatively still for a few days after the surgery
  • Take bucket baths instead of showers while the nose is bandaged
  • Avoid blowing your nose
  • Avoid making extreme facial expressions
  • Limit movement of the upper lip by brushing your teeth gently
  • Eat fiber rich foods to avoid constipation
  • Avoid wearing clothes that need to be pulled over your head and instead wear front closing clothes
  • Avoid strenuous activities like aerobics and jogging
  • Do not wear sunglasses or spectacles that put pressure on your nose for at least a month
  • Protect your nose from the sun by using sunscreen whenever outdoors
  • Limit your salt intake

To be completely satisfied with the results of your rhinoplasty surgery, will require patience. It can take a few months for the cartilage and tissues to settle into place. It is also necessary for you and your doctor to be aligned as to the kind of correction needed.

All You Need to Know about Breast Augmentation Surgery

All You Need to Know about Breast Augmentation Surgery

Breast Augmentation is the most popular plastic surgery procedure performed worldwide.

Breast augmentation can enhance your appearance and your self-confidence, enlarging your breasts and making them more proportional with the rest of your body. Breast augmentation can also be used to correct differences in the size of your breasts or sagging breasts, which can occur after pregnancy and breastfeeding.

Preoperative Planning

There are many choices and options in breast augmentation, which require careful planning to individualize what are the best options for you. All of these choices have advantages and disadvantages, which you should understand before you make a decision with your surgeon.

Breast augmentation does not correct severely drooping breasts. If you want your breasts to look fuller and to be lifted due to sagging, a breast lift may be required in conjunction with breast augmentation. Breast lifting can often be done at the same time as your augmentation or may require a separate operation. Your plastic surgeon will assist you in making this decision.

Most patients choose silicone implants rather than saline implants because they have a more natural look and feel. Modern silicone implants are substantially different from the old type of silicone implants used in the 1970s and 80s. Another advantage of the cohesive gel silicone implants is that they are made in a variety of round and tear-drop shapes, which means it is easier to choose an implant to achieve the breast shape that you desire.

Safety Profile

Misinformation emerged about silicone in the late 80's and early 90's. The effects of silicone implants have since been studied extensively these claims were made, looking at 100,000s of women with breast implants. In June 1999, The Institute of Medicine at the National Academy of Sciences in the U.S.A. released a report, which confirmed that there is no increased risk of the development of cancer, immunologic or neurological problems associated with the use of silicone breast implants, which had been claimed in the late 80's. Patients with implants can breastfeed normally as there is no effect on breast milk.

Surgery

Breast augmentation surgery requires a general anaesthetic, with the operation taking 1-2 hours, and is usually performed as a day-stay or overnight-stay procedure.

Implants are placed either behind the breast tissue or under the pectoralis muscle on the chest wall. Breast implants placed underneath the muscle have a more natural shape, a lower chance of capsular contracture (hardening) of the implant, and it is easier for mammography. There is also a choice of incision - in the crease under the breast, under the areola or in the armpit, all of which have advantages and disadvantages, which you should discuss with your surgeon.

Recovery

Following the surgery, it is important to have restricted light activities, with no lifting or heavy duties for 7-10 days. Most patients may drive again and return to work within 7 days after the surgery. Heavy activities and sport should not be undertaken for 3-4 weeks following the surgery.

I am 23 years old. My nose is not in correct shape. I want to reshape it. I consult with a doctor he said me to go for nose implant and it will cost around 3-4 lakh. But I read this in internet that it's better to have surgery from own rib cartilage tissue rather than nose implant. Since it do not last for lifetime. It can get infected also. I am soo confused what to do. please plz suggest.

Fellowship in Cosmetic Plastic Surgery, Fellowship in Breast and Body Contouring, Rio, Brazil, DNB (Plastic Surgery) -, DNB (General Surgery) -, MS (General Surgery) - , MBBS
Cosmetic/Plastic Surgeon, Mumbai
Hi, there are many different ways to correct the shape of the nose. Many surgeons do it using implants while other surgeons do it using grafts from the patients own rib cartilage. Both methods are widely used and have their own advantages and disadvantages. Your plastic surgeon would discuss all options with you in detail. You both and then decide which procedure to opt for depending upon the surgeon's and your own preference. You should ask all your doubts to your surgeon during the consultation and get them cleared. The total cost of surgery depends on a lot of factors and varies from hospital to hospital. I hope this helps you.
