Hi, thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. As you feel pain in knees, follow these usefultips for pain relief: * first check out bone mineral density (bmd). In case of low bmd, your calcium level in body are low. So take take calcium rich diet daily. Take a glass (300 ml) of full cream milk daily or 100 gm of cheese or 1-2 eggs regularly. * give a gentle oil massage to knees regularly and wrap your knees with cloth dipped in hot water for atleast 10-15 minutes. * wear knee cap or wrap crape bandage during walk for knee support. * sit atleast for 5-8 minutes in sunlight in morning or evening for vitamind. *avoid heal shoe and wear only flat shoes for balanced load on knees. * check out your blood sugar level and hemoglobin level in every 6 months. * if you have any history of rheumatoid arthritis in family then checkout your ra factor. * take guggalu tablets if himalaya one tablet daily for atleast 3 months. * take a piece of garlic, crush it and boil it a glass of milk and drink daily for fast pain relief.

FOR LOWER BACK PAIN EXERCISE DEAR U FOLLOW THIS EXERCISE……………………….LUMBAR EXTENSION STRETCHES Generally these are good for individuals who may have a disc herniation. They should be performed with caution for anyone who has spinal stenosis or any known fractures in their vertebrae. Prone Press Up: Begin by lying on your stomach with your elbows bent underneath you and palms flat on the surface. Keeping your hips and pelvis in contact with the surface, lift your upper torso off the mat with your arms, keeping your back muscles relaxed. Only go as high as you are comfortable. Perform 10 repetitions holding each one for 10 seconds each, working your way to 30 seconds Generally, the flexion based stretches are good for those with spinal stenosis or tightness through the lower lumbar musculature. Single Knee to Chest: Begin by lying on your back with both knees bent. Bring one knee up towards your chest. Perform 2-3 repetitions, holding each one for 15-30 seconds. You may feel a stretch along the lower back or buttocks area. You may also perform this with both legs up towards your chest if it is comfortable Prayer Stretch or Child’s Pose: This stretch is for the lower back muscles along the spine and is a very common yoga pose. On your hands and knees, sit back so your buttocks is resting on your heels. Reach your hands forward to lengthen your spine and feel a stretch in your middle back. You can reach your hands to either side to focus the stretch on the opposite side of your spine. Hold for 10-30 seconds and perform 2-3 repetitions. Angry Cat Stretch: This stretch can incorporate both an extension and a flexion component. You may perform either way or just one way if that is more comfortable for you. On your hands and knees, let your belly sag towards the table to increase extension through your spine (lumbar extension picture). To increase flexion through your spine, arch your back upwards, bringing your spine away from the table (lumbar flexion picture). You may hold each position for 5-20 seconds and repeat several times NERVE STRETCHES Just as you can stretch a muscle, you can stretch a nerve. Nerve stretches are very important to perform if you have any radiating pain from your lower back into your buttocks or legs. Nerve stretches are also very important to perform after lumbar surgery to ensure that there is no scar tissue adhering the nerve to any internal structures. Sciatic Nerve Stretch: This stretch should be performed if the majority of the radiating pain into the legs is felt in the buttocks, back of the leg, and/or through the calf and foot. This follows the pathway of the sciatic nerve. By stretching the nerve, it can help to desensitize it so that it will not cause as much pain. Perform this stretch by lying on your back with your hand behind one knee, preferable the leg with the sciatic pain. Straighten your knee then alternate flexing your ankle back and forth. Only hold your ankle in each position for a few seconds. Perform 10 repetitions on each side. As with beginning any new exercise, start off slowly with a very gentle stretch. Stop if you have any increase in lower back pain or sciatic pain down your leg. If unsure whether you should perform these stretches, consult your doctor or physical therapist.

Knee pain is a very common medical condition that occurs due to constant wear and tear of the knee joint. It can be experienced by older adults, young adults and children. Women are more vulnerable to knee pain than men. The exact location of the knee pain can vary. Pain can originate in any of the bony structures compromising the knee joint (femur, tibia and fibula), the kneecap (patella) or the ligaments and cartilage (meniscus). Knee pain can be localized or diffuse throughout the knee. Home remedies for knee pain. Applying cold compresses to the painful knee is one of the best and easiest ways to reduce pain and swelling. The cold will constrict the blood vessels, reducing blood flow to the affected area and thus reducing swelling. It will also give you relief from the pain. Wrap a handful of ice cubes in a thin towel. Apply the compress to the affected knee area for 10 to 20 minutes. Do this two or three times daily until your pain is gone. A bag of frozen peas also works well as a cold compress. You can opt for alternating hot and cold compresses as well for chronic pains. Mix two teaspoons of apple cider vinegar in two cups of filtered water. Sip this mixture throughout the day. Drink this tonic daily until you recover completely. You can also add two cups of apple cider vinegar to a bath tub of hot water. Soak the affected knee in the water for 30 minutes. Do this once daily for a few days. Also, you can mix together one tablespoon each of apple cider vinegar and olive oil. Use it to massage the affected knee once or twice daily until the pain is gone. Crush a small piece of fresh ginger root, add to one cup of water and boil it for 10 minutes. Strain and add a little honey and lemon juice. Drink two to three cups of this ginger tea daily until the pain is gone. You can also massage the affected knee with ginger oil two or three times a day until you are satisfied with the results Add one-half teaspoon each of ground ginger and turmeric to a cup of water and boil it for 10 minutes. Strain, add honey to taste, and enjoy twice daily. Boil one teaspoon of turmeric powder in a glass of milk. Sweeten with honey and drink it once daily. Another option is to take 250 to 500 mg turmeric capsules three times daily. Follow these remedies daily until the pain is gone completely.

One sided back pain can be due muscle strain, Facet arthropathy, Sacroilitis, lateral disc bulge. Since you are suffering from 2 years you should get MRI lumbo sacral spine done to find out the cause. Here are some general tips to help you. 1. Sit with straight back and change posture in between. 2. Take breaks in between work and walk. 3. Do back strengthening and stretching exercises daily. 4. Take simple painkillers like paracetamol if pain is persistent. 5. Apply analgesic cream over back followed by warm fomentation. 6. Keep pillow behind your knees at bedtime. 7. Do not bend and lift weights. 8. If back pain is accompanied with radiating leg pain or tingling , numbness or weakness, consult your doctor immediately

Please note the following in respect to your queries: Straight ahead running puts very little stress on the ACL so you can likely get back to some running when you are comfortable and your surgeon is satisfied you are healed. It is also important that you strengthen your gluts and butt muscles to help control your leg motion and reduce your risk of damaging the ACL in the future. Your PT should be helping you with that portion of your rehabilitation. It takes at least a year for the graft to revitalize to the point where the tissue can repair itself. What that means for you is that each time you stretch the ligament before it is revitalized, you are stuck with a new longer length ligament; so the graft can become sloppy and not as effective at keeping your knee stable when you make lateral or cutting movements. The lack of stability is likely why you went through the procedure in the first place.
