How Endometriosis Leads to Infertility and Miscarriages?

There can be a number of reasons that lead to infertility in women. Endometriosis is one of them. It is a benign condition where the endometrium, which includes the tissues that form the lining inside the uterus, grows outside the uterus. Apart from infertility, it may also cause pelvic pain in women.

There are no such symptoms of endometriosis apart from pelvic pain and it cannot be detected without surgery. Thus, it is very difficult to diagnose this disease and state how common it is. However, research says that the prevalence of this disease in women in their reproductive age is between 3 to 10 per cent.

It is found that endometriosis sometimes leads to infertility. Here is how:

  • Abdominal Adhesions and Infertility: Normally, the endometriosis implants develop in the abdomen. Then the body surrounds them with connective tissues. The reason for doing so is isolating the implants, so that they cannot harm your body. Now, sometimes these adhesions can create a blockage in the opening of the fallopian tube or may also pinch off the fallopian tube. This will result in obstruction in the fertilisation of egg and the sperm and thus fertilization will not take place; so there are no chances of conceiving. These obstructions when diagnosed can be treated with surgery.
  • Secretions from Implants: Endometrium plays a vital role in conception where the tissues secrete a wide variety of hormones and nutrients. Now, the endometrial implant also functions in the same way and secretes those hormones. The difference is that it does not deposit them in the lumen of the womb, and it is released in the abdominal cavity. In these substances, there may be some hormones that are responsible for infertility.
  • Prostaglandins: Prostaglandins are other hormones that are secreted by the endometrium normally. It helps in a lot of body processes that include various stages of menstrual cycle and pregnancy. This hormone is required for a lot of functions such as ovulation, sperm mobility, and regression of corpus luteum, immune interaction, menstrual cramps and contraction of the uterus at birth.

Just like the endometrium, the implants also secrete these hormones. And, the problem that is caused by the implant secreted prostaglandin hormone is that they are released in the abdomen and not in the womb. Moreover, they are not secreted at the right time, thus sending a wrong message to the brain.

A woman with a few days of pregnancy may face a miscarriage because the ovary will get a signal to start the menstrual cycle, and the womb lining where the egg is implanted will be expelled.

7 Diabetes Related Skin Problems

Diabetes is a metabolic disorder, chiefly caused by insulin fluctuation in the body. A study shows that, 1 out of 3 people suffering from diabetes are likely to develop a skin condition sooner or later; and in some cases, these skin problems serve as symptoms of diabetes. Although these skin problems can be cured easily if treated early, proper care must be administered at all times. Here are 7 skin conditions that affect diabetics:

1. Bacterial infection

Although anyone can get a bacterial infection, people suffering from diabetes are likelier to contract them. Usual bacterial skin problems that tend to trouble diabetics are boils, lumps near the eyelids, nail infections and carbuncles. Diabetics are also prone to infections deep in the skin and of the underlying tissues. The affected area is usually warm, red and swollen.

2. Fungal infections

People who have diabetes have an increased risk of contracting the fungal infection, especially the one known as candida Albicans. The fungus tends to create a red, itchy rash, which is surrounded by blisters and scales. These are usually found in warm places of the body like the armpits, or between the fingers of the hand or perhaps even the toes. Common fungal infections in diabetic people include athlete's foot, jock itch and ringworm.

3. Tingling and itching

Among the many causes of itching, diabetes is one. Yeast infection, dry skin and poor circulation are the primary causes of itching among patients of diabetes. Usually, the lower part of the leg tends to itch the most, if poor circulation is to be blamed.

4. Skin problems related to neuropathy

Diabetes can be the cause of nerve damage in some cases; and neuropathy is a common symptom of diabetes. This type of damage can cause a loss of sensation in a part of the body. On event of an injury, one may not be able to feel it. As a result, an open sore or wound, if left unattended, can develop into an infection.

5. Digital sclerosis

Digital sclerosis, a condition marked by tight, waxy, thick skin that develops on the back of the hand, is common among people who have type 1 diabetes. In this condition, the joints of the fingers get stiff and, thus, lose their mobility and elasticity. Skin of the toes, forehead and knees may also get affected.

6) Xerosis

Xerosis is another common skin condition that accompanies diabetes. In this condition, the skin becomes very dry and itchy.

7) Acanthosis nigricans

Acanthosis nigricans is the condition where diabetics get darkening of skin leading to skin tags around neck and armpits.

Mr. lybrate-user, Plantar fasciaitis is an inflammation at the insertion of that fascia in the heel. It needs certain regular exercise like walking on the toes, picking up a cloth with toes, using soft foot wear and applying ice to the heel twice a day. Plus there a few exercises suggested. That will help in controlling it. Occasionally local steroid injections are given if the pain is too bad. Thanks.

1. Do regular aerobic exercise for 1 hour daily (brisk walking, jogging, running, swimming, cycling etc) 2. Reduce weight if overweight 3control diet a) avoid sweets, sugar/honey, milk, milk product excess calorie and carbohydrate rich diet like white bread, cereal, rice, pasta b) avoid all refined food like maida, pasta, starchy food like rice, potato, bakery item, processed food, c) include more fiber like pulse, green leafy vegetable, cucumber, carrot, tomato, j legume. D) avoid sweetened fruit like banana, mango, litchi, chickoo, sapota, seethaphal jack fruit, grape etc. Can have fruits like guava, pineapple, papaya, orange, mousambi, strawberry, watermelon, pomegranate, jack fruit, sweet potato e) avoid white/polished rice, take unpolished rice like brown rice/wheat/jowar/ragi, moong and chana dal. F) avoid ghee/ butter./coconut oil/excess salt intake like papad/pickle. G) avoid red meat. Take fish/chicken. H) avoid alcohol and carbonated drinks like soft drink 4. Adhere to a strict diabetic diet by consulting dietitian 5. Monitor your fasting, pp blood sugar 3 monthly and hba1c six monthly 6. Consult diabetologist for advice and treatment.

Being pregnant her p. P. Blood sugar should be below 120 mg%. Please ask her to go for ogtt test. Based on result we first control by life style modification and brisk walking and if required metformin can be used. Other wise insulin is drug of choice.

What should be an ideal diet for 50 year male with activity living habit who is suffering from diabetics.
Mr. Lybrate-user, diet plan for a person with diabetes is not very different f than one for a normal person. He needs to take a balanced diet, with 50 to 55% of calories coming from carbs, 15% from proteins & 25 to 30 % from fats. But major difference is in relation to type of foods. He should avoid sweets, deep fried foods, junk foods, pastries, other bakery items, sweetened aerated drinks (colas), fructose corn syrup containing foods, highly processed food products and high fat milk products. He should consume more of green leafy vegetables, salads and permitted fruits (100 g a day). Always use whole grain products and flour. Use more of millets and fiber containing seeds like flax seeds, fenugreek etc. As regards a specific diet plan for you I need to know following details: 1) fasting & pp glucose levels, hbac1%, 2) present drugs with dosage, 3) extent of daily exercise & 4) present diet pattern. You will have to plan caloric intake based on above and ideal body weight (72 kgs). Thanks.

Dear lybrate user, one of the major nutritional substance required for human body is carbohydrate. We take carbohydrate in the form of rice, wheat, sugar etc. The complex molecules of carbohydrate are broken down into simple glucose molecules during digestion & released in the blood stream to be absorbed by the muscle cells. Muscle cells utilise the glucose molecules to release energy by breaking down those glucose molecules. Insulin hormone plays a vital role in the absorption of the glucose molecules by the muscle cells. In abnormal conditions, either due to less production of insulin or due to resistance to insulin, the process of absorption of glucose by the muscles comes to a stand still. The glucose molecules remains in the bloodstream which results in the increase in the blood sugar level. The failure of utilisation of the glucose molecules by the muscle cells results in production of less energy than normal condition. This is externally manifested as the fatigue & tiredness of the patient. This excess amount of blood sugar compels the kidney to function more than normal level in order to excrete the blood sugar molecules through urine. As a result the patient suffers from frequent urination. This abnormal condition is clinically known as diabetes mellitus. Once a patient is diagnosed with diabetes he has to take regular medicine to keep his blood sugar level within normal limits. Take homoeopathic mother tincture gymnema sylvestra q, 30 drops, thrice daily, after meals, in a cup of water. Also you need to walk at least 1 hour daily without any interval. Hello! if you want to know your diabetic profile you need to do simple blood test. Fasting and postprandial blood will be collected n you will be able to know it within 1-2 days depending on lab policy.

lybrate-user, Thanks for the query. Both hypothyroidism and hyperthyroidism can cause problems with menstruation, ovulation and hence leading to infertility. So maintaining normal thyroid function is very essential for getting pregnant and also for proper development of the fetus and desired outcome of pregnancy. Thanks.

Why do my lower body increases when my thyroid raise I have done thyroid and I lost, 34 kgs it happens with me after loosing weight I am very much depressed as my lower body increases and my thyroid raises every 3 months even on regular dose of tablet I take 75 mg and do regular exercise also my tsh is always 7.5 or something around that I am taking thyrox 75 mg.

Lybrate-user, if hypothyroidism is not well controlled then weight reduction is difficult. To achieve very good control tsh levels should come around 3 to 4 milli units /l. With 7.5 milli units/l weight reduction is not possible, because it shows that hypothyroidism is not in control. Once you achieve that then efforts to reduce weight will be successful. Plus you will have to take a restricted diet giving exact calories based on ideal body weight, plus regular exercise of over 1 hour a day. Thanks for the question.

Dear madam! U have what is called subclinical hypothyroidism. Nothing to worry. Pls monitor your thyroid function 3 monthly and for once get anti TPO titers done. It TSH keeps increasing we have to treat then. If TPO +ve you will need medicines then.

Conservative measures to decrease straining and constipation in hemorrhoids/piles many patients see improvement or complete reduction of many symptoms with conservative measures which can be tried at home. Warm baths bathing in tubs with warm water alleviates pain and severity of piles. Ice can also relieve the pain of acute thrombosis. The sitz bath can be tried for symptom relief. Sitz bath is done by sitting in a bowl of warm water to which salt is added. It is usually done for about 20 to 30 minutes twice or thrice a day. High fiber diet a high fiber diet includes more than 25 g of fiber per day. Many hemorrhoidal symptoms reduce when dietary changes are made. Addition of fiber supplements to already existing diet is a good plan. Toilet habit retraining and stool softeners patients should sit on the toilet seat only for the time required to evacuate the lower bowels. Persistent straining or prolonged sitting can lead to increased size of hemorrhoids. Stool softeners can be taken to soften stools if there is hard stools which can cause undue straining while going to toilet. Topical agents topical ointments and creams can be used to decrease itching and inflammation. Topical hydrocortisone can reduce some symptoms of piles but prolonged use is not recommended. Adequate water intake. Patients suffering from hemorrhoids should take adequate amounts of water which can ease the passage of stools.

Hello dear High triglycerides and cholesterol can cause atherosclerotic heart disease or narrowed coronary arteries in the heart can cause the symptoms of angina, when theheart muscle is not provided with enough oxygen to function. Decreased blood supply to the brain may be due to narrowed small arteries in the brain or because the larger carotidarteries in the neck may become blocked. This can result in a transient ischmic attack or stroke. Peripheral artery disease describes gradual narrowing of the arteries that supply the legs. During exercise , if the legs do not get enough blood supply, they can develop pain , called claudication . Other arteries in the body may also be affected by plaque buildup causing them to narrow, including the mesenteric arteries to the intestine and the renal arteries to the kidney. Change life style .do meditation,yoga, Go walk daily, do any one physically activity daily i.e.play any game., Avoid fast foods,oily foods,junk foods,alcohol,smoking,fatty foods, non-veg.,avoid stress,take plenty of water in day time. And 1 glass of water before bed at night. use flax oil, omega-3 fatty meal or cap., take .green veg., whole grain. GARLIC,. Cod liver oil. Take anti cholesterol medicines . Cap. Ecosprin gold-10 at night daily.

Dear since you have diabetes you need exercises to keep fit and control your sugar. Since you have knee pain best exercise for your problem is cycling. Do buy a stationary bicycle and exercise for at least 20-30 minutes daily and then twice a day.

My mother had ultrasound of her neck today in which the doctor said her thyroid gland is increasing and its tissues are coming out. Can anyone please help me with this that why is it happening? My mom has to go for two blood tests as well yet then the doctor will tell about the treatment. Since 1.5 months my mom feels tired after working, has swelling on right side of her neck and it pains in the ear too when she tries to sleep that side. No internal pain but it pains outside and it has gradually increased. Please tell me the precautions that we should take and also is it serious? Would be very helpful. Thanks.

Hi, If thyroid gland size is increasing, it may be due to goitre formation. So it is necessary to get a thyroid assay done. The result will help in a correct diagnosis of the condition. It can be Hashimoto's Thyroiditis, a simple goitre or thyrotoxicosis. Only assay will help in finding out First get the thyroid assay done and come back with the results then only I will be able to guide. Thanks.

Hello Mr. Lybrate-user. Your creatinine clearance is 51.5 ml/min according to the information provided by you. 50 ml/min is borderline and you need to take care of it. You have not mentioned whether you have any other co morbidities like diabetes and hypertension etc. If do then you must keep them in control to reduce the damage to the kidneys. You have to avoid red meat completely. But can have sea food as suggested by your physician. You need to do regular exercise control your weight. A simple walking for 30 mins morning and evening is sufficient. Check your creatinine and urea levels regularly and don't hesitate to consult physician immediately. IN HYPOTHYROIDISM:--- where there is deficiency of thyroxine hormone leading to sluggishness, tired feeling, weight increase, slow heart beats, intolerance to cold, swelling of legs, feet and face, difficulty in concentrating and at times in speech. Treatment for hypothyroidism is giving thyroxine tablets. And dose are according to level of thyroid for life time. Avoid bad habits . Avoid stress. Some Relax during work. Go for walk, Take plenty of water, high-carbohydrate foods such as potatoes, rice, pasta and bread. Nutrient-rich foods that improve your health may also benefit your thyroid gland, antioxidant-rich fruits and vegetables more plenty of water: Blueberries, tomatoes, bell peppers, and other foods rich in antioxidants can improve overall health and benefit the thyroid gland. Eating foods high in B vitamins, like whole grains, may also help. flax oil, Selenium: Tiny amounts of selenium are needed for enzymes that make thyroid hormones to work properly. Eating selenium-rich foods, such as sunflower seeds or Brazil nuts, can be beneficial. Tyrosine: This amino acid is used by the thyroid gland to produce T3 and T4. Taking a supplement may help, avoid oily foods,junk foods, go for walk. Do Tests every 3 to 6 months. And consult doctor on time.
