Testicular Cancer And Its Treatment!

MBBS, MD - Oncology, DNB - Super Speciality, Immuno Oncology

Testicular Cancer And Its Treatment!

Cancer that starts in the testicles is called testicular cancer. Testicular cancer is commonly seen in men who are 25 to 40 years of age. It is generally of two types.

  1. Seminomas: These grow and spread slowly and respond to radiation therapy.
  2. Non-seminomas: These grow and spread rapidly, do not respond to radiation, and often require surgery.

While the exact causes for testicular cancer are not clear, the following are considered to be the possible risk factors:

  1. Family history
  2. Prior history of testicular cancer
  3. Undescended testicles
  4. Congenital abnormalities of the penis or urinary tract

Symptoms: While testicular cancer is often not easy to diagnose, the patient will have symptoms, which he may ignore.

  1. Painful lump in one or both testicles
  2. Pain in the lower abdomen and groin area
  3. Enlargement of the testicles, due to fluid collection
  4. Back pain

Often, this type of cancer originates in the testicles and spreads to the adjacent groin lymph nodes. It can also reach distant organs like lungs, liver, and brain via metastasis.

The good news about testicular cancer is that the above symptoms would lead to early diagnosis, confirmed by ultrasound and other tests. Once confirmed, even if diagnosed in the late stages, treatment is quite effective and prognosis is good.

Stages of Testicular Cancer
There are four stages of testicular cancer – 0 to III, and treatment and prognosis would depend on the stage at which it is diagnosed.

Treatment: Various modalities are used depending on the stage and extent of spread.

  1. Chemotherapy: Chemotherapy drugs are used to kill the existing cancer cells and arrest the further growth of testicular cancer. These can be given as tablets or injected into the system.
  2. Radiation: Either internal or external radiation can be used. External is when a beam of radiation is directed at the scrotum to kill the cancer cells. Internal is when a needle, seed or wire is planted in the scrotum, which constantly gives out radiation to kill the cancer cells.
  3. Surgery: If one testicle is involved, then it is removed along with the neighbouring lymph nodes. This may be followed up by chemotherapy or radiation in most cases. If distant organs are involved, it may not be removed. Instead chemotherapy or radiation will help control the spread.
  4. Clinical trials: There are various drugs in final trial phase, and with the risks understood, some men may choose to be part of such trials.

With any of these treatments, surveillance follow-up is very essential, as testicular cancer is known for recurrence. If only one testicle was involved initially, the second one also could be affected later. Infertility is another issue, which needs to be managed in men with testicular cancer.

Stages and Symptoms of Breast Cancer!

Stages and Symptoms of Breast Cancer!

What do Sheryl Crow, Kylie Minogue and Angelina Jolie have in common; they are all breast cancer survivors. Being diagnosed with cancer is a scary thought but it should come as a relief that many types of cancer can be cured, breast cancer being one of them. However, this depends largely on the stage of diagnosis. Hence, it is essential to understand and recognize the symptoms of this disease.

Breast cancer can affect men and women. However, it is more common amongst women. A lump in the breast is usually the first sign of breast cancer. This may feel like a small pea or make part of the breast tissue denser thereby changing the contours of the breast. Some people may also notice a clear discharge from the nipple. This can be easily felt during a routine breast examination at home. If you feel something along these lines, you must consult a doctor immediately.

Along with a physical examination, your doctor may ask for a biopsy of the breast tissue, blood tests, X rays etc to determine a breast cancer diagnosis. If you do have breast cancer, these tests will also help the doctor find out what stage the cancer is at. There are 4 stages of breast cancer.

  1. Stage I: From this level onwards, the cancer is said to be invasive. In Stage IA the cancer cells spread to the fatty tissue while in stage IB, a few cancer cells can also be seen in a few lymph nodes.
  2. Stage II: In stage IIA, a small tumor may be seen in the breast. In Stage IIB this tumour may be as big as a walnut. Cancer may or may not affect the lymph nodes at this stage.
  3. Stage III: At this stage, cancer is considered to be in its advanced stage and is harder to treat. In Stage IIIA cancer cells can be found in up to 9 lymph nodes around the breast. In Stage IIIB the tumour advances into the chest wall and skin surrounding the breast. In Stage IIIC more than 10 lymph nodes may be affected.
  4. Stage IV: This is also known as the Metastatic stage. In this stage, the cancer cells would have spread beyond the breasts and lymph nodes. Common organs to be affected are the lungs, brain, bones and liver.

Treatment for breast cancer depends on many factors including the type of cancer, the speed at which it is growing, the stage of diagnosis and the patient’s overall health.

Uterine Cancer - Ways It Can Be Treated!

MBBS, MD - Oncology, DNB - Super Speciality, Immuno Oncology
Oncologist, Delhi
Uterine Cancer - Ways It Can Be Treated!

Uterine cancer is often treated with a combination of treatments. The recommendation of treatment option depends on the factors such as stage and type of cancer, age, the overall health of the patient, side effects, personal preference etc. Another important consideration that goes before taking a call on the treatment option includes the research on the patient’s ability to bear a child in future. But, overall, the treatment plan includes surgery, chemotherapy, radiotherapy, hormone therapy etc.

Surgery
Surgery is often the first step towards removing the tumor and a few healthy tissues around it. Some of the common surgical procedure include hysterectomy, lymph node dissection, and sentinel lymph node biopsy.

  1. Hysterectomy: The extent of the cancer spread decides the type of procedure a surgeon can perform. Hysterectomy is the removal of the cervix and the uterus. For patients whose cancer has spread significantly, a surgeon performs a radical hysterectomy- removal of the vagina, cervix, uterus and some healthy tissues from the nearby region. For patients who have had menopause in their health history, doctors choose to perform a procedure known as the salpingo-oophorectomy. This involves the removal of both the ovaries and the fallopian tube.
  2. Lymph node dissection: This is quite similar to hysterectomy. The surgeon might remove the lymph nodes to ensure that cancer doesn’t spread beyond the uterus.
  3. Sentinel lymph node biopsy: This is another process to determine whether cancer has spread to the lymph nodes and the surrounding areas. This procedure is effective in determining other cancers type as well such as cancer-related to the breasts.
  4. Radiation Therapy: This is a kind of therapy that involves the use of high-energy rays that could destroy cancer cells. Radiation is given at a fixed dosage over a limited number of days. This method of treatment is typically used in addition to a surgery. In most cases, a surgery followed by radiation therapy helps a patient to fight cancer more effectively. Many oncologists might also want a patient to take radiation therapy to shrink the size of a tumor before going for a surgery. The most common radiation therapy is external beam radiation therapy.
  5. Chemotherapy: This procedure involves the use of powerful drugs to destroy cancer cells. There are claims that it effectively reduces the ability of the cancer cells to grow and spread. Chemotherapy is usually suggested after a surgery or can be individually applied to a cancer patient. Chemotherapy can be applied in orally or in the form of IV. The period of chemotherapy like radiotherapy is applicable for a specified amount of cycle over a limited number of days. This form of treatment is most preferred when cancer recurs in a patient after a considerable amount of time. They are equally effective in uterine cancer as well. If you wish to discuss about any specific problem, you can consult an Oncologist.
