Your kidneys don't get flushed or cleaned because of water. For common thinking, it is ok to believe that since kidneys would shed more water, they will get cleaned. But this is not the way kidneys work. Kidneys work by a mechanism called re-absorption. Meaning by which, they first eliminate a lot of solute mixed water by simple filtering and then re-absorb the important things that should be retained. Proteins, sugars and electrolytes are things that re-absorbed back by the kidneys. If you want to ensure the health of your kidneys - take less salt (sodium), keep your blood pressure under control at all times and monitor your sugar levels.

Did You Know - Constipation Can Also be a Sign of Urinary Incontinence

Did You Know - Constipation Can Also be a Sign of Urinary Incontinence

Urinary Incontinence is the sudden urine loss that occurs involuntarily in women. Some of the factors which cause urinary incontinence are pregnancy, menopause (know more why Women Are More Vulnerable to Heart Diseases Post Menopause) and childbirth. It should be noted that urinary incontinence by itself is not a disease, but is a symptom of other underlying disorders such as diabetes, infections and other conditions.

Causes of temporary cases of urinary incontinence include:
1. Constipation (learn more that Work for Constipation Relief)
2. Urinary tract infections
3. Excess consumption of alcohol and caffeine
4. Consuming carbonated drinks
5. Use of artificial sweeteners
6. High doses of vitamin B and vitamin C
7. Being on sedatives, muscle relaxants, blood pressure and heart medications
8. Eatables which are too spicy, acidic or sugary

However, persistence of incontinence might be due to a more serious condition. Some of the causes are stated below:

  1. Age: The bladder muscles tend to weaken with age. This affects the bladder’s urine holding capacity.
  2. Pregnancy: Hormonal changes and increase in weight during pregnancy can cause incontinence.
  3. Menopause: Oestrogen is responsible for the healthy maintenance of the lining of the urethra and bladder. The onset of menopause causes the oestrogen hormone levels to drop. The gradual damage of the bladder tissues causes incontinence.
  4. Childbirth: The bladder control muscles are weakened during normal vaginal delivery, thus leading to incontinence.
  5. Obstruction: Occurrence of tumours in the urinary tract can block the normal flow of urine which can cause incontinence.
  6. Hysterectomy: The same ligaments and muscles support both the bladder as well as the urethra. With removal of the uterus by hysterectomy, the pelvic floor muscles are deteriorated. This leads to urinary incontinence.
  7. Neurological disorders: Conditions such as multiple sclerosis (autoimmune disorder in which the immune system destroys the protective sheath of the nerves), Parkinson’s disease (disorder affecting motor functioning of the body), brain tumours, spinal injury or strokes are capable of causing incontinence.

Different forms of urinary incontinence are classified on the basis of their symptoms:

  1. Urge incontinence: Urge or urgency incontinence occurs when there is a leak before the person reaches the toilet.
  2. Stress incontinence: Small amounts of urine leak out during normal physical movements such as sneezing, coughing and exercising. This happens because of the minimum stress exerted on the body, and hence, the name.
  3. Functional incontinence: Sudden leakage of urine occurs due to external deterrents or physical disabilities such as not being able to find a toilet.
  4. Overactive Bladder: This is characterized by frequent and urgent urination.
  5. Overflow incontinence: A full bladder, at times, leads to unexpected leakage.
  6. Transient incontinence: If urine leakage occurs due to temporary situations such as infections, or new medications.

A 7 mm stone in the ureter should be removed surgically to prevent kidney damage. Nowadays it can be done endoscopically. No incision/cut is required. And you need to stay in the hospital for a day or 2 only.

I am 59 years old male. I had undergone spinal cord operation in the year 1986. Three years back my creatine level raised to 1.4 and after urinating, I still had some residual inside. When urine stayed inside I use to get infection and get fever. Since then I either removed the urine by inserting tube or wearing a urine bag as advice since then. Since few months my creatine levels are increasing. Now my creatine level is 7. My legs have swelling.

You are rather in a very critical condition.I will suggest to to go for Homoeopathic treatment alongwith the present treatment. There are very effective homoeopathic medicines which will help in bringing down the creatine levels and have no adverse effects if taken along with allopathic medicines. It will improve quality of your life .I willnot say at this stage that I can cure you 100% but right now fingers crossed and just have faith in me and homoeopathy.

Good morning! I see no reason why you cannot go abroad and settle down there. Every place has medical facilities that can be used by you. However, it would only be judicious to visit your urologist and get his NOC for the same. He could also help you by online consultation in case you face any difficulty at any point of time. All the best. Take care.

Diabetic Nephropathy - Who is at Risk of Acquiring it?

MBBS, MD - Medicine, DM - Nephrology

Diabetic Nephropathy - Who is at Risk of Acquiring it?

Diabetes is a lifestyle disease that is very prevalent in the current generation. According to a report by the World Health Organisation (WHO), the number of people with diabetes has risen from 108 million in 1980 to 422 million in 2014. The global prevalence of diabetes among adults over 18 years of age has risen from 4.7% to 8.5% from 1980 to 2014. Diabetes should not be taken lightly as it can affect other body organs besides the pancreas, such as the kidneys. Diabetic nephropathy refers to damage to your kidneys caused by diabetes, and may also lead to kidney failure in various cases.

Causes of diabetic nephropathy
Not everyone who suffers from diabetes will suffer from diabetic nephropathy. The main function of the kidneys is to filter waste from your blood. However, this is not always the case. Due to high a concentration of blood sugar caused by diabetes, several blood vessels in the kidney are destroyed, thereby, preventing the kidneys from carrying out its job. Over time, the kidneys may stop working altogether, resulting in kidney failure. If you are diabetic, the probability of you suffering from diabetic nephropathy will increase if you smoke or have high blood pressure or high cholesterol.

Catching it early
Today, diabetic nephropathy is the leading cause of chronic kidney disease in the US and other western countries. Since the symptoms of this condition aren’t visible in its early stages, you need to undergo urine tests on a regular basis. If detected early, the effects of diabetic nephropathy can be reversed. Common symptoms of this disease include the following:

  • Swelling in legs and feet
  • Detection of albumin in urine
  • Decline in glomerular filtration rate (GFR)
  • Elevated arterial blood pressure

What are the treatment options?
Medication to lower the blood pressure is the most commonly prescribed form of treatment suggested by doctors to prevent or slow the damage to your kidneys. There are other lifestyle changes that you may have to undertake in order to keep a check on the condition:

  • Limit sugar intake and keep blood sugar levels within the target range
  • Exercise regularly and work towards keeping your heart healthy
  • Limit protein intake, as it could put extra pressure on your kidneys
  • Limit salt intake to control blood pressure
  • Quit smoking and drinking

Diabetes is an unfortunate condition to have to live with. But don’t let it bog you down! With these simple lifestyle changes and taking care of your general well-being, you can beat the disease and emerge a winner!

Darkening of urine has various reasons to blame. To know the exact reasons do a routine urine examination and urine microscopy. Unless you have fever too, urinary infection is less likely.

Instead of reducing your calcium intake, focus on limiting the sodium in your diet. Extra sodium causes you to lose more calcium in your urine, putting you at risk for developing another stone. Limit your sodium to 2, 000 milligrams each day. There are many sources of hidden sodium such as canned or commercially processed foods as well as restaurant-prepared and fast foods. Calcium oxalate kidney stones are the leading type of kidney stones. Oxalate is naturally found in many foods, including fruits and vegetables, nuts and seeds, grains, legumes, and even chocolate and tea. Some examples of foods that contain high levels of oxalate include: peanuts, spinach, beets, chocolate and sweet potatoes. Another common type of kidney stone is a uric acid stone. Red meat and shellfish have high concentrations of a natural chemical compound known as a purine. High purine intake leads to a higher production of uric acid which then accumulates as crystals in the joints, or as stones in the kidneys. To prevent uric acid stones, cut down on high-purine foods such as red meat, organ meats, and shellfish, and follow a healthy diet that contains mostly vegetables and fruits, whole grains, and low fat dairy products. Limit sugar-sweetened foods and drinks, especially those that contain high fructose corn syrup. Limit alcohol because it can increase uric acid levels in the blood and avoid crash diets for the same reason. Eating less animal-based protein and eating more fruits and vegetables will help decrease urine acidity and this will help reduce the chance for stone formation. You should also be sure to drink at least 6-8 glass of water a day to help reduce the risk for stone formation.

I have got small stones in kidney. So what precautions should I take to get rid of stones.

MBBS
General Physician,
I have got small stones in kidney. So what precautions should I take to get rid of stones.
There are several medicines for dissolving stones but treatment depend on size type and cause of formation of stone. Prevention of renal stones also depend on type of stone. Also there are various diseases and drugs that can predispose to stone formation. A usg, complete metabolic panel and urine cytology and ph check up is needed in young aged patients. To prevent oxalate stones thats very commonly found you need to restrict oxalate rich foods like: peanuts, rhubarb, spinach, beets, chocolate and sweet potatoes.
