Dr.RashiKhatri has done her BDS followed with an MDS in Oral and Maxillofacial Pathology Dentist. She is a Dental Surgeon who has 4 years of experience. Her graduation from one of the most reputed institutes of India viz. Government Dental College & Hospital, Ahmedabad is one of the earliest signs of her accomplishments. Later she also completed here Masters Degree (MDS) from the same institute viz. Government Dental College & Hospital, Ahmedabad and specialised in Oral and Maxillofacial Pathology. Orofacial Pain - Sources and Symptoms

An unpleasant experience which not only disturbs the usual routine of a person but also the psychological peace and causes extreme discomfort is called pain. When this pain involves the region above the neck, below the orbitomeatal line and in front of the ear is called as orofacial pain.

Multiple causes for orofacial pain may exist and the symptoms may include such diverse findings as headaches, neck pain, ear pain, dental pain, facial burning or stabbing sensations, and jaw joint pain.

The complaints may either develop gradually or have a rapid onset and can originate from musculoskeletal, neurovascular or neuropathic causes.

Sources of orofacial pain may include:

  1. Trigeminal Neuropathy – ant damage or degeneration to the trigeminal nerves that supplies mainly to the facial structures causes the orofacial pain. This is commonly referred as trigeminal neuralgia.

  2. Neurovascular Disorder – migraine is the most common neurovascular disorder that causes the radiating pain to the mouth eyes and other facial muscles.

  3. Temporomandibular Disorder – the major cause of orofacial pain is the temporo mandibular joint disorder which causes pain in the lower jaws. Orofacial pain may also be caused by any temporo mandibular joint surgeries and due to failure of joint replacement surgery of the jaw.

  4. Burning Mouth Syndrome – this condition is caused due to nerve damage, lack of adequate saliva production, fungal infection, certain medications and also noted for patients with diabetes. Orofacial pain is also noted in burning mouth syndrome.

  5. Cervicalgia – pain due to spinal cord injury, damage to spinal nerve, muscle and ligaments may radiate to face and cause orofacial pain.

  6. Trauma – accident is the most common cause of temporomandibular disorder and other spinal disorder that causes orofacial pain.

  7. Sleep disorders – sleep disorders include grinding or clenching of the teeth during sleep may also cause orofacial pain.

Symptoms

Though orofacial pain is caused by several factors, its symptoms includes dental pain, ear pain, neck pain, headache, pain in the jaw joint and stabbing sensation over the face. Some patients may also experience burning sensation over the face. Other symptoms include dizziness, insomnia, tinnitus (abnormal ringing or roaring sound in ears), poor muscle coordination, abnormal itching over the face and neck and tingling sensation. If you wish to discuss about any specific problem, you can consult a Dentist.

TMJ Symptoms, Treatment, and Causes

The human jaw is a very important part of human anatomy and within it, the mandible or the lower jaw is very important as it is the movable part which helps you chew and enables you to open or close the mouth. The lower jaw is connected to the skull via the TMJ or the Temporo-mandibular joint. It works like a hinge which slides as and when required to allow movement of the jaw. TMJ disorder can be quite painful and limit you from normal activities and cause disruptions in your daily life.

Causes of TMJ-

The TMJ works as a movable joint where the bone area in contact with each other is covered by cartilage and a small shock absorbing soft disk separates the two cartilages. The disk enables the movements to be smooth. Any changes in this arrangement will usually result in pain. The primary causes for TMJ disorders could be summed up as follows –

  1. Misalignment – If the TMJ moves out of alignment due to underlying factors, it will cause pain and the inability to move the joint.

  2. Erosion of the cartilage or the disk – If the cartilage or the disk separating them erodes, then TMJ disorders might occur.

  3. ArthritisJust as arthritis affects knees, wrists, elbows and finger joints, it could cause TMJ disorders as well.

  4. Impact – One of the most common causes of TMJ disorders, this may occur due to injuries sustained from forceful impacts.

Symptoms-

  1. Pain or tenderness around the jaws.

  2. You may experience pain on either or both sides of the temporo-mandibular joint.

  3. Even if you don’t have pain in the specific joint, you may have an aching facial pain.

  4. The most common and typical symptom of TMJ is of pain while chewing. Your pain may progressively get worse over time if you have a deteriorating condition.

  5. Pain in and around the ears may also be a symptom of TMJ disorders.

  6. Difficulty in opening or closing your jaw either due to stiffness, pain or both.

Treatment-

Treatments vary between various methods and approaches and the severity of the pain or condition and the underlying causes. Some of the options are:

  1. Oral Medications – Pain killers, muscle relaxants and anti–inflammatory medications are usually prescribed to reduce pain while the jaw may be healing or for underlying causes that may result in pain.

  2. Physical Therapies – This is another common method wherein the doctors may use a device to keep the jaw stiff or stop them from moving in a particular way in which it causes pain. Mouth guards or oral splints are such devices. Dislocation may have to be corrected by physical pressure and setting the bone back into the joint as well.

  3. SurgeryIn certain cases, surgery may also be required to correct chronic pain, especially wherein the underlying condition may keep causing problems. If you wish to discuss about any specific problem, you can consult a Dentist.

Diagnosis of headache is key to treatment. Taking painkillers without correct evaluation is harmful. If your headache arises from the neck it has a different treatment than if you have pain around your eye. So please get a thorough headache evaluation before starting any meds

Kindly get yourself evaluated if there is any sharp edge of the tooth aggravating the ulcer traumatically. Also you need an evaluation for iron and vit b12.

I have migraine trouble since long time . Took so many pills and try other methods bud no relief what should I do.

If your migraines are accompanied by pain in the back of the neck and top of the head, you should have your neck muscles and cervical spine evaluated. Normal pain killers will not work.

Migraines can be treated and need a thorough headache evaluation to determine the source and triggers for the same. It is possible to eliminate them with the correct medical treatment and lifestyle changes.

I am taking amitriptyline 10 mg for migraine. What is the duration of the treatment?

I am suffering by migraine from last 3 month. Which medicine are useful for this?

To treat any headache condition (including migraines) you have to get the exact nature of the headache diagnosed so that you can treat the headache from source rather than rely on common pain killers. Often tight muscles of the neck are common triggers asking with certain foods/smells and hormonal changes. I suggest a thorough headache evaluation.
