REHABILITATION FOLLOWING MENISCAL SURGERY

Meniscal injuries are common in today's exercise minded world. The medical meniscus is oval or C-shaped and is larger in diameter than the lateral meniscus. The entire border of the medical meniscus is attached to the tibia by thick coronary ligaments. Meniscal tissue is hydrated, soft, and fibrocartilagenous is slightly greater than that in articular cartilage. 

The menisci function to: 

1) distribute weight bearing loads 

2) increase joint congruency, thus aiding stability 

3) limit abnormal motions, and 

4) possibly improve articular nourishment.

A. MECHANISM OF INJURY

Non contact stresses are the most frequent mechanism of injury to the menisci. These stresses result from a sudden acceleration or decelerations in combination with a change in direction (eg. when a soccer or lacrosse player“plants and cuts” to elude an opponent). In jumping sports such as volley ball or basket ball, the additional element of vertical force with angular momentum upon landing can contribute to a Meniscal tear. Meniscal lesions may have clinical symptoms. 

Displaced tears, such as bucket handle tears, can produce locking and giving way. Non displaced tears can alter meniscus mobility and produce abnormal traction stresses to the capsule and synovium, accounting for the pain associated with such lesions.

B. TREATMENT PLAN

 The final component of implementing an individualized rehabilitation program is the formation of a treatment plan. The underlying message of the treatment plan should be a functional orientation to exercise. It is important that the rehabilitation program follow a functional progression. In association with a functional progression, the therapist should follow the SAID principle(specific adaptation to imposed demands). The body adapts to specific activities based on the type of stress experienced, and the type of stress experienced, and the type of adaption that takes place will be specific to the type of training performed. 

This concept is demonstrated throughout the rehabilitation process in various closed kinetic chain (body movement performed with one or both feet in a stationary position) exercises, which impose different forces at the knee in multiple planes. Therefore, in following a functional approach to rehabilitation, emphasis should be placed on treatment in a closed kinetic state when appropriate.To initiate the treatment plan, it is imperative to follow certain rehabilitation rules. 

These have been described as :

1) create a safe environment for optimal healing. 

2) don’t hurt the patient.

3) be as aggressive as you can without breaking rule. 

4) The treatment plan can be made effective by understanding the specific effects of the treatment, the cause of the patient’s symptoms, the functional biomechanics of the knee joint, stages of tissue healing, and the patient’s specific injury or surgery.

Wash d face 3/4 times with plain water. Change your shaving cream. Apply tumeric powder to itchy areas. It will help.

Hello. Gently move the razor on these areas. Medication: start immediately with homoeopathic medication - sulphur 200/ one dose daily in the morning for a week and a homoeopathic tincture called arnica montana and mix 1 tsp in 1/2 cup of water and apply upon the face on the beard and the mustache area with a cotton swab. Then use the razor movement technique as in 1 above. Repeat the above process every 3rd day and see the growth in 1 month. Take a red mushroom called ganoderma 5 gms. 2 times per day at 12 hours interval for 3 months.

Vitamin a, e, nutrition and zinc to be consider part of treatment. Treatment: 1. A teaspoonful of coriander juice mixed with pinch of turmeric powder is effective home remedy for pimples / acne. Mixture should be applied to the face at night after thoroughly wash every day. 2. If you have constipation or menstruation in case of female, then remove a same, or go for treatment 3. Cap. Neem guard - 1 daily in morning after breakfast 4. Cap garlic pearls - 1 daily after breakfast 5. Steam on face buy using neem patti / leaves thrice in a week 6. Avoid cosmetic cream available at otc and use pure chandan and other herbs made paste along with non side effect face wash 7. Stress management 8. Steam bath twice a week diet: 1. Upon arising: a glass of lukewarm water with half a freshly? squeezed lime and honey 2. Breakfast: any fresh fruit and fresh milk. 3. Lunch: a bowl of freshly-prepared steamed vegetable, one or two whole wheat chapattis, and glass of butter milk or curd. 4. Mid afternoon: a fresh fruit or juice 5. Dinner: a large bowl of raw vegetable salad, with lime juice dressing, sprouted mung beans 6. Bedtime snack: a glass of fresh milk or fresh fruit note: we have kit for pimples and acne contain home made pure chandan with kesar, kasturi, baras etc. Cream, saffron facial bar contain neem, turmeric, sandal wood & cow urine - made from cowpathy, neem - turmeric face wash? made from cowpathy, panchagavya moisturizing cream, 1- month pills of homoeopathy/ biochemic non side effect remedy. You need to take high fiber low fat diet followed by detox diet. Drink lot of water everyday. Take small and frequent meals at regular intervals of 2-3 hrs. Avoid outside food completely. Ask me privately for customized diet plan for you.

I am 24 year old, male, I am suffering from insomnia from one month and used melatonin medicine and not cured and no sleep from one month and what should I do?

Dear, Due to lack of sleep there are many complications arise such as lack of concentration ,irritability, fatigue . So these are some instructions & methods for good sleep : • Frequent oil massage of head with oil relax your mind and enhances sleep .You can also do foot massage daily it destress you . • Do not take stress . Do meditation and long breathing pranayams to relax the mind. • Do not take caffeine, tea & alcohol in excess and these are stimulant ,(activate) your brain result in lack of sleep. • Avoid drinking alcohol & smoking. • Avoid sleep during day time as it makes it harder to fall asleep at night. •Try to sleep early at night . Do not think during sleep . Do exercise and long walk daily . • Avoid to take sleeping pills because these are habit forming drugs and these drugs disturb your normal sleep pattern. • Take a bowl of curd daily in afternoon and one glass of milk at night daily .

Do oil massage with mahanarayana taila mixed with pindataila daily before taking bathing. Luke warm the oil before applying. Do atleast for 7 days continuously and check. Let me know if you still have this problem then.
